Cicatricial pemphigoid is a rare autoimmune blistering disease involving predominantly mucosal areas in elderly patients. It runs a chronic relapsing course. Immunosuppressive treatments are frequently complicated by adverse effects and require careful monitoring. A 69-year-old woman is reported who responded well to a simple regimen of combined tetracycline and nicotinamide after 15 years of inadequate disease control and numerous adverse effects from immunosuppressive agents.

In conclusion, tetracycline hydrochloride is an effective and safe sclerosant for the treatment of hydrocele with entailing minimal side effects and low recurrence rate. However, it should not be recommended for the treatment of spermatocele in younger men who still wish to have children, because of the danger of drug epididymitis and resulting infertility. Hydrocele treatment can be performed in younger men who still wish to have children.

We compared the effects of doxycycline and tetracycline hydrochloride on the subjective symptoms in ocular rosacea. Twenty-four patients with symptomatic ocular rosacea were randomly assigned to two groups and treated with doxycycline 100 mg/day (group 1, 16 patients) or tetracycline hydrochloride 1 g/day (group 2, eight patients). Twenty-six patients, aged 38 to 78 years, with testicular hydrocele were treated by aspiration, punction and tetracycline instillation. The cure rate was 79%. Inflammation of the scrotum was observed in all of the cases. Due to recurrence two patients underwent surgery, which revealed a hematocele.

Tetracycline (Achromycin, Lederle) solution was used to produce sclerosis of 37 hydroceles and 18 epididymal cysts in patients over the age of 50 years. All but one of the epididymal cysts were sclerosed satisfactorily at 9 months; 35 hydroceles sclerosed satisfactorily, although four patients required re-treatment at 3 months. One patient required orchiectomy for an infected haematocele. Pain after instillation of tetracycline was an occasional complication which could be prevented by prophylactic spermatic cord block. Fixed drug eruptions are a rarely reported complication of tetracycline therapy. For poorly understood reasons, the epithelium of the glans penis seems unusually susceptible to this type of reaction. We report 3 additional cases of balanitis secondary to orally administered tetracycline and suggest that there may not be cross sensitivity to doxycycline.

We found that naturally occurring tetracycline resistance in streptococci is encoded by more than one genetic determinant. We treated six patients who had recurrent episodes of nontuberculous phlyctenular keratoconjunctivitis and progressive corneal vascularization and scarring with oral tetracycline.
