BACKGROUND: Tube thoracostomy is the definitive treatment for most significant chest trauma, including injuries resulting in pneumothorax, hemothorax, and hemopneumothorax. However, traditional chest tubes fail to sufficiently remove blood up to 20% of the time (i.e., retained hemothorax), which can lead to empyema and fibrothorax, as well as significant morbidity and mortality. Here we describe the use of a novel chest tube system in a swine model of hemothorax.
METHODS: This was an intra-animal-paired, randomized-controlled study of hemothorax evacuation using the PleuraPath™ Thoracostomy S…

Both aortic dissection and tension pneumothorax are conditions that require urgent treatments. However, the diagnosis of these emergencies i … the patient's vital signs did not improve and reexpansion pulmonary edema developed following tube thoracostomy. Contrast-enhanced computed tomography revealed type A thrombosed aortic dissection with bullae in the upper lobe of the left lung. Therefore, the patient was admitted to the intensive care unit, conservatively treated, and discharged without any co …

Introduction Isolated tracheobronchial injury after blunt trauma of the chest is rare. Because of the high elasticity of the chest in children, they occur mainly in the pediatric population. Case Report We report a case of a 7-year-old girl who experienced complete avulsion of the right main bronchus at the level of the carina after a horse-riding accident. The patient presented with extensive emphysema of the upper chest, neck, and face and severe respiratory distress. Endotracheal intubation led to tension pneumothorax. After insertion of two 17-mm thoracostomy tubes, pneumothorax and a ma…

AIM: To show our experience in treatment of patients with penetrating chest injuries sustained during the Homeland War in Croatia
METHODS: It was a retrospective study based on the records of the Department of Surgery of the University Hospital of Osijek (Croatia). All patients surgically treated during the wartime period (1991.-1995.) were analysed with respect to death rate, causes of injuries, frequency of injuries of thoracic organs and frequency of combined injuries of thorax and abdomen. Most of our patients were treated with thoracotomy as opposed to the common protocol (thoracostomy) …

BACKGROUND: Bilothorax is defined as the presence of bile in the pleural space. It is a rare condition, and diagnosis is confirmed with a pl … y or procedure, and traumatic injury to the chest or abdomen was the second most common cause. Tube thoracostomy was the main treatment modality (73.83%), followed by serial thoracentesis. Fifty-two patients (51.30%) had associated bronchopleural fistulas. The mortality was considerable, with 18/115 (15.65%) reported death. Most of the patients with mortality …

Pleural sepsis stems from an infection within the pleural space typically from an underlying bacterial pneumonia leading to development of a parapneumonic effusion. This effusion is traditionally divided into uncomplicated, complicated, and empyema. Poor clinical outcomes and increased mortality can be associated with the development of parapneumonic effusions, reinforcing the importance of early recognition and diagnosis. Management necessitates a multimodal therapeutic strategy consisting of antimicrobials, catheter/tube thoracostomy, and at times, video-assisted thoracoscopic surgery.

Firecracker thoracic injuries are rare but critical, often requiring multidisciplinary management. We report a case of a teenage male patient who presented with hemothorax and empyema due to a firecracker-related chest wall injury. Initial management involved tube thoracostomy; however, the patient subsequently required thoracoscopic intervention for the removal of intrathoracic firecracker residue and decortication. This case emphasizes the significance of early intervention, tailored antibiotics, residue removal for recovery, and preventive measures for such injuries.

Bronchopleural fistula (BPF) is a sinus tract between a mainstem, lobar or segmental bronchus and the pleural space. We present a 68-year-old male with a 13 mm spiculated left lower lobe nodule who underwent video-assisted thoracoscopic surgery left lower lobe wedge resection followed by persistent BPF requiring open window thoracostomy. We present a step-up approach to management of persistent BPF with discussion of conservative, operative and reconstructive techniques for closure.

A case of bilateral extensive subcutaneous emphysema secondary to external cardiac massage is reported. This is a rare complication. It occurs with a pneumothorax after resuscitation maneuvers. The diagnosis is based on a bundle of anamnestic, clinical and radiological arguments. This case demonstrates the importance of performing a simple prehospital thoracostomy on the finger without chest tube placement and demonstrates the need for systematic investigation of possible complications in any patient who has undergone external cardiac massage.

Bronchial laceration is an uncommon complication of blunt trauma in children. Treatment of bronchial laceration has involved thoracotomy with primary repair of the bronchial injury or nonoperative management with tube thoracostomy. We report a 21-month-old boy who sustained a large tear of the right upper lobe bronchus after an automobile/pedestrian accident in whom nonoperative management resulted in a favorable outcome. The relevant literature is reviewed, and an algorithm for management is proposed.

A 22-year-old man with tetralogy of Fallot successfully underwent a corrective operation. Twenty years after total correction, large systemic collateral arteries (SCA) developed. The SCA arose from the right subclavian and internal mammary arteries. The SCA were not present at the time of total repair and thoracotomy had not been performed on this side. A closed-tube thoracostomy for postoperative pneumothorax may be responsible for the development of the large SCA.

Spontaneous pneumothorax is not uncommon and can be potentially dangerous, especially in older people. A series of 201 patients is presented. Conservative treatment with tube thoracostomy had a 19% failure rate. It seems nevertheless to be the treatment of choice, though with risk of serious complications. Among the 60 patients who underwent lateral thoracotomy, 30% had recurrence of pneumothorax or other complications. Reoperation because of inadequate pulmonary expansion was necessary in four cases.

Bilateral pneumothoraces complicating attempted bilateral subclavian vein catheterization culminated in the iatrogenic death of the patient. Complications of subclavian vein catheterization are reviewed briefly. This procedure should be limited to patients in whom its use is clearly indicated and should be performed only by individuals who are experienced in the anatomy of the region, who are trained in the technique of subclavian puncture, and who have the means and ability to perform immediate tube thoracostomy should pneumothorax occur.

BACKGROUND: The preclinical treatment of a traumatic or spontaneous tension pneumothorax remains a particular challenge in pediatric patients. Currently recommended interventions for decompression are either finger thoracostomy or needle decompression. Due to the tiny intercostal spaces, finger thoracostomy may not be feasible in small children and surgical preparation may be necessary. In needle decompression, the risk of injuring underlying vital structures is increased because of the smaller anatomic structures. As most emergency physicians do not regularly work in pediatric trauma care, de…

Gastropleural fistula (GPF) is a rare disease characterized by fistulous tract between gastric and pleural cavities via the diaphragm. The authors present diagnosis and comprehensive treatment of a patient with GPF. Endoscopic closure of gastric wall defect using the Apollo OverStitch device for intraluminal suturing followed by video-assisted argon plasma coagulation (VAP) of cavity walls through thoracostomy and placement of vacuum aspiration system resulted in a positive clinical outcome in a patient with delayed postoperative complication.
