A comparison of lumbar epidural and intravenous fentanyl infusions for post-thoracotomy analgesia.

This double-blind randomised study compared the analgesic efficacy, respiratory effects, side effects, and pharmacokinetic disposition of 24 hr lumbar epidural and intravenous infusions of the same dosage regimen of fentanyl (1.5 micrograms.kg-1 bolus then 1 microgram.kg-1.hr-1 infusion) in 50 patients after thoracotomy. Patients received either epidural fentanyl and intravenous normal saline, or epidural normal saline and intravenous fentanyl, for postoperative analgesia, after a standard low-dose alfentanil and isoflurane general anaesthetic. Visual analogue pain scores were lower in the epi…

Management of panesophageal cancer by blunt resection without thoracotomy and reconstruction with stomach.

Encouraged by their experience since 1978 with blunt esophagectomy and gastric reconstruction in the management of pharyngolaryngeal malignancy, the authors have extended their use of this technique to the management of thoracic esophageal cancer. A study of 43 blunt esophageal resections is presented; 23 were performed in the management of pharyngolaryngeal cancer, and 20 were performed (22 attempted) for the resection of intrathoracic esophageal cancer. In the first group, two deaths occurred secondary to liver failure in cirrhotic patients with moist ascites. One death occurred due to anast

Breast and pectoral muscle maldevelopment after anterolateral and posterolateral thoracotomies in children.

Anterolateral and posterolateral thoracotomies, involving an incision through the third and fourth intercostal space, have been used for the repair of congenital heart lesions in children during the past thirty years. We examined 28 patients who had undergone repair of atrial septal defect, patent ductus arteriosus, and coarctation of the aorta as children, through the anterolateral or posterolateral chest approach. Volumes of the breasts and pectoral muscles were obtained by a method using plaster molds, and linear dimensions of each chest side were measured. Questionnaires were completed tha

A Mini-Thoracotomy Retropleural Microdiscectomy for Thoracic Disc Herniation: A Case Report.

Symptomatic thoracic disc herniation (TDH) is relatively rare, most commonly occurring below the T8 level due to increased spinal mobility. A small herniation could cause significant spinal cord compression due to the narrow thoracic spinal canal. A 62-year-old Malay man presented with thoracic myelopathy and was unable to walk. Magnetic resonance imaging (MRI) demonstrated a large paracentral disc herniation at T9/T10 compressing the spinal cord. He underwent a lateral mini-thoracotomy retropleural microdiscectomy. Both lungs were ventilated throughout the procedure. A lateral mini-open incis…

Right thoracotomy with central cannulation for valve surgery: 10 years of experience.

BACKGROUND: The aim of this study is to report the early outcomes of valvular heart surgeries performed via the right thoracotomy approach. While thoracotomy with femoro-femoral bypass is an established method for minimally invasive open-heart surgeries, thoracotomy with conventional cannulation is still being explored. In our center, we conducted 958 valvular heart surgery cases using the right anterolateral thoracotomy approach with central cannulation and data were analyzed. METHODS: This is a retrospective observational study based on prospectively collected data from patients who underwe…

Results of Coarctation Repair by Thoracotomy in Pediatric Patients: A Single Institution Experience.

BACKGROUND: Aortic coarctation is among the most common cardiovascular congenital abnormalities requiring repair after birth. Besides mortality, morbidity remains an important aspect. Accordingly, we reviewed our 20-year experience of aortic coarctation repair by thoracotomy, with emphasis on both short- and long-term outcomes. METHODS: From 1995 through 2014, 214 patients underwent coarctation repair via left thoracotomy. Associated arch lesions were distal arch hypoplasia (n = 117) or type A interrupted aortic arch (n = 6). Eighty-four patients had isolated coarctation (group 1), 66 associa…

Jerks of the latissimus dorsi muscle and intercostal neuralgia after posterolateral thoracotomy.

INTRODUCTION: Post-thoracotomy pain syndrome (PTPS) is a common complication related to intercostal nerve injury. During this type of surgery, although less frequently, thoracodorsal and long thoracic nerves can also be injured, and jerks of peripheral origins may appear. We report a case with intercostal neuralgia and latissimus dorsi muscle jerks after posterolateral thoracotomy. CASE REPORT: A 55-year-old woman with Ehlers-Danlos Syndrome presented with a typical picture of PTPS along the right T5 dermatome following posterolateral thoracotomy at the level of the fifth intercostal space. A…
