Results of transcervical thymectomy for myasthenia gravis in 100 consecutive patients.

OBJECTIVE: To review the results of the authors' most recent 100 consecutive cases of transcervical thymectomy for myasthenia gravis (MG) in terms of complications and outcome in comparison with other reported techniques. SUMMARY BACKGROUND DATA: Myasthenia gravis is believed to be an autoimmune disorder characterized by increasing fatigue with exertion. The role of thymectomy in the management of the disease remains unproven, but there is widespread acceptance of the notion that complete thymectomy improves the course of the disease. Complete excision of the thymus is the goal in all cases; …

Long-term immunologic effects of thymectomy in patients with myasthenia gravis.

BACKGROUND: Thymectomy (Tx) is a common therapeutic option to treat myasthenia gravis (MG), but its effects on the immune system are still obscure in humans. OBJECTIVE: We sought to evaluate long-term immunologic effects of therapeutic Tx in patients with MG. METHODS: T- and B-cell subsets and T-cell repertoire were analyzed in 35 patients with MG, 16 with previous Tx (at least 8 years before), 6 with recent (<1 year) Tx, and 13 without Tx, as well as in 32 healthy subjects used as normal control subjects. Serum immunoglobulins and a variety of autoantibodies were also measured. A subsequent…

Sleep apnoea in well-controlled myasthenia gravis and the effect of thymectomy.

We conducted overnight polysomnographic sleep studies of 16 patients (5 men and 11 women) with clinically well-controlled myasthenia gravis … ended to have more SA. In 9 of the 12 SA patients, polysomnographic studies were repeated following thymectomy. SA had resolved in 6 patients, but persisted in 3. These findings suggest that SA is a possible clinical manifestiation of MG and that nocturnal dysfunction of both peripheral and central colinergic systems may be involved.

Management of myasthenia gravis by extended thymectomy with anterior mediastinal dissection.

BACKGROUND: Thymectomy has continued to gain acceptance as definitive treatment for myasthenia gravis. Because of the nature of thymic embryology with scattered rests throughout the anterior mediastinum, we advocate a transsternal thymectomy with extended anterior mediastinal dissection. METHODS: A series of 48 patients with myasthenia gravis treated by thymectomy between 1979 and 1991 were reviewed. RESULTS: The mean length of duration of disease from onset to operation was 48.7 +/- 11.3 months, and the mean length of follow-up was 51.6 +/- 6.5 months. The operation was associated with a 21…

Associated disorders in myasthenia gravis: autoimmune diseases and their relation to thymectomy.

The prevalence of myasthenia gravis (MG) in the counties of Hordaland and Sogn & Fjordane on January 1, 1984 was 9.6 per 100,000 inhabitants. Other autoimmune diseases were found in 11 out of 48 MG patients. The occurrence of autoimmune thyroiditis (5 patients, 10.4%) and systemic lupus erythematosus (4 patients, 8.3%) in the MG patients was clearly higher than that reported in the general population. Rheumatoid arthritis was found in 2 patients (4.2%). The autoimmune diseases were mainly recorded among the nonthymectomized MG patients. In addition to those with definite diseases of autoimmune

Response of agranulocytosis to thymectomy in a patient with thymoma and chronic lymphocytic leukemia.

A patient with chronic lymphocytic leukemia presented with recurrent episodes of severe neutropenia. This neutropenia was initially responsive to prednisone but eventually became refractory. A spindle-cell thymoma was diagnosed and resected with dramatic improvement in his granulocytopenia. Both autologous and heterologous granulocyte macrophage colony growth were clearly inhibited by the patient's serum. This is the first reported case of severe granulocytopenia associated with a thymoma that seemed to be reversed following thymectomy.

[Cervical thymectomy as an expanded surgical method in primary and secondary hyperparathyroidism].

From 1964 to 1979 neck explorations were performed on 135 patients with primary and 30 patients with secondary hyperparathyroidism. In 28 of 165 patients (16,9%) a cervical (23 patients) or a direct (5 patients) thymectomy after sternum splitting was carried out. In 22 of 28 patients, parathyroid tissue was found within or adjacent to the thymus (13,3%). Cervical thymectomy is a safe and effective procedure when an exact indication is present and if certain rules of operative technique are followed.

Distribution of thymic tissue at the anterior mediastinum. Current procedures in thymectomy.

The distribution of thymic tissue at the anterior mediastinum was examined histologically in 18 cases following the removal of the adipose tissue which was located outside the thymic capsule at the time of thymectomy for myasthenia gravis. The gross adipose tissue revealed the presence of histological thymic tissue, including Hassall's bodies, in 13 of 18 cases. In order to be sure of extirpating all of the thymic tissue, the adipose tissue at the anterior mediastinum as well as the gross thymus should be removed.

Bilateral video-assisted thoracoscopic thymectomy has a surgical extent similar to that of transsternal extended thymectomy with more favorable early surgical outcomes for myasthenia gravis patients.

BACKGROUND: This study aimed to introduce the authors' surgical technique of bilateral video-assisted thoracoscopic thymectomy (BVT) and to compare their early surgical results between BVT and transsternal extended thymectomy (T3-b thymectomy) for patients with myasthenia gravis (MG). METHODS: The records of 114 patients who underwent either BVT (n = 55) or T3-b thymectomy (n = 59) for MG were reviewed retrospectively between January 2006 and November 2009. RESULTS: No surgical mortality or major morbidity occurred in either group. There was no conversion to open thymectomy, and no statistic…

The Incidence of Malignancy and Autoimmune Disorders After Thymectomy: An Exploratory Study.

OBJECTIVES: Thymectomy reportedly increases the risk of cancer, autoimmune disease, and death. Studies are few and data limited. We sought to determine the incidence of postoperative malignancy and autoimmune disease in patients who underwent thymectomy. METHODS: We conducted a retrospective review of adult patients undergoing thymectomy between 2008 and 2020. Postoperative cancers and autoimmune disease were diagnosed after thymectomy. Incidence rates were calculated using patient-years and compared to the National Cancer Institute's (NCI) Surveillance, Epidemiology, and End Results (SEER) p…

A Case of Cryoglobulinemic Vasculitis: Linking Thymectomy, Autoimmune Disease, and Lymphomagenesis.

This case illustrates the possible interplay between autoimmunity, chronic antigenic stimulation, cryoglobulinemic vasculitis, and lymphoproliferative disorders. The patient's progression from myasthenia gravis and total thymectomy to systemic lupus erythematosus, cryoglobulinemic vasculitis, and marginal zone lymphoma demonstrates a continuum of immune dysregulation. Total thymectomy may impair central tolerance, predisposing to secondary autoimmunity and chronic B-cell activation. A 39-year-old woman presented with acute kidney injury, nephrotic-range proteinuria, and pulmonary edema. Labora…

Surgical and Neurological Outcomes in Robotic Thymectomy for Myasthenic Patients with Thymoma.

BACKGROUND: While the safety and feasibility of robotic thymectomy have been well documented through several studies, the surgical and long-term neurological outcomes in patients with thymomatous myasthenia gravis (MG), particularly in advanced stages, remain scarce. This study aims to evaluate the surgical outcomes in patients undergoing robotic-assisted thymectomy (RATS) for thymoma and to analyze neurological outcomes in patients with myasthenia. MATERIAL AND METHODS: Out of 128 robotic thymectomies performed at our institution between October 2013 and January 2022, clinical and pathologic…

A Case of Isaacs Syndrome Developed After Thymectomy for Myasthenia Gravis.

A 54-year-old man was referred to our hospital due to calf pain and gait disturbance. Two years earlier, he had developed ptosis of the left eye. A routine health check conducted two years later revealed a thymoma with positive anti-acetylcholine receptor antibodies. He was diagnosed with myasthenia gravis and underwent thymectomy. Eight days after the surgery, he experienced involuntary calf muscle movements and pain, leading to worsening gait disturbance. Upon admission, he presented with cold toes, ptosis of the left eye, and continuous myokymia of both legs, which persisted during sleep. L…

Nomogram for predicting the risk of postoperative myasthenic crisis in patients with thymectomy.

OBJECTIVE: This study aimed to develop and validate internally a clinical predictive model, for predicting myasthenic crisis within 30 days after thymectomy in patients with myasthenia gravis. METHODS: Eligible patients were enrolled between January 2015 and May 2019. The primary outcome measure was postoperative myasthenic crisis (POMC). A predictive model was constructed using logistic regression and presented in a nomogram. The area under the receiver operating characteristic curve (AUC) was calculated to examine the performance. The study population was divided into high- and low-risk gro…

Attitudes and Beliefs Toward Thymectomy in the Myasthenia Gravis Patient Registry.

OBJECTIVES: To evaluate patient attitudes and beliefs toward thymectomy for myasthenia gravis (MG). METHODS: The Myasthenia Gravis Foundation of America administered a questionnaire to the MG Patient Registry, an ongoing longitudinal survey of adult MG patients. Questions assessed reasons for or against thymectomy and how hypothetical scenarios would have affected their decision. RESULTS: Of 621 respondents, 190 (31%) reported a history of thymectomy. Of those who underwent thymectomy for nonthymomatous MG, 97 (51.6%) ranked symptom improvement as most important and 100 (53.2%) ranked reduci…

A novel method of subxiphoid video-assisted thoracic surgery for thymectomy.

BACKGROUND: With advances in thoracoscopic surgical instruments and techniques, subxiphoid video-assisted thoracic surgery (S-VATS) has become the main approach for anterior mediastinal tumor resection under thoracoscopy. However, the drawbacks of S-VATS, including it being a relatively unfixed surgical procedure, make it complicated and difficult for unexperienced surgeons to master. METHODS: This study retrospectively reviewed and analyzed consecutive patients with anterior mediastinal tumor or myasthenia gravis (MG) who underwent S-VATS at the Fujian Medical University Union Hospital, Chin

Clinical Outcomes and Technical Approach of Thymectomy in the Veterans Health Administration.

BACKGROUND: Thymectomy is traditionally performed through a transsternal incision, but less invasive modalities have emerged, including transcervical, thoracoscopic, and robotic approaches. Despite the advantages of video-assisted thoracoscopic surgery (VATS) over thoracotomy, most thymectomies are performed through sternotomy. This study compared the use and 30-day postoperative outcomes of transsternal, transcervical, and VATS thymectomy in the Veterans Health Administration. METHODS: This was a retrospective review of veterans who underwent thymectomy through the Veterans Affairs Surgical …
