First experience from 200 cases with a new breast tissue expander for multi-stage pre-pectoral breast reconstruction after mastectomy.

BACKGROUND: Breast cancer is the most common cancer globally, and mastectomy rates are increasing. Tissue expander-based breast reconstruction is used when direct-to-implant methods are unsuitable. A novel magnetic resonance imaging compatible breast tissue expander has recently been introduced. This study aims to evaluate its clinical use in pre-pectoral breast reconstruction and its complication profile. METHODS: A monocentric prospective cohort study included women undergoing expander-based breast reconstruction after mastectomy with the new tissue expander between August 2020 and October

Adverse Outcome of Two-Staged Breast Reconstruction: More Than One Culprit.

BACKGROUND: Two-stage prosthetic breast reconstruction with initial insertion of a tissue expander followed by change-over to an implant after a period of inflation has become a well-established option in breast reconstruction. Our aim was to analyze the feasibility of two-stage breast reconstruction in our center by following the surgical technique, number of complications and associated risk factors. PATIENTS AND METHODS: We studied 91 patients who underwent skin sparring mastectomy and tissue expander placement in our surgical unit, between January 2017 and December 2021. Axillary surgery

Optimization of In-Continuity Spring-Mediated Intestinal Lengthening.

BACKGROUND: Spring-mediated intestinal lengthening has been studied in numerous animal models to effectively achieve up to a 3-fold increase in length. In this study we are interested in optimizing this method of spring lengthening. METHODS: Juvenile mini-Yucatan pigs underwent laparotomy for spring implantation. Springs were secured by plicating the intestine around the springs. In one set of experiments, varying degrees of plication were compared to determine the necessary narrowing needed to confine the spring. In another set of experiments, dissolvable sutures were used for the plication

Cleft palate repair with the use of osmotic expanders: a preliminary report.

A new method of cleft palate repair by expansion of tissue by means of osmotic expanders implanted in the first stage of treatment is descri … tive concept. Despite some technical problems related mostly to still tested optimal filling phase, tissue expansion makes palate repair easier, probably without relaxing incisions and bone denudation. Consequently, some adverse effects on facial growth may be reduced. So far, there is no evidence for it, however, and since this is a preliminary report, there is a …

The surgical treatment of cicatricial alopecia.

Surgical correction of cicatricial alopecia can yield exceptional results when performed in the appropriate clinical scenario. To facilitate determination of the most suitable corrective therapy, we propose two new categories of cicatricial alopecia: "unstable" and "stable." Unstable cicatricial alopecia is intermittent and results in possible subsequent scarring hair loss in either new or old areas. Stable cicatricial alopecia, on the other hand, refers to fixed permanent scarring. While surgical excision is preferred to hair transplantation for both categories of cicatricial alopecia, this p

Multimodality imaging of breast tissue expanders: types, MRI safety and complications.

Mastectomy remains a common surgical treatment for breast cancer, often followed by breast reconstruction to mitigate the psychological impact. Among reconstruction techniques, the use of tissue expanders is a safe, cost-effective, and widely adopted method, typically involving a two-stage surgical approach. These devices gradually stretch the skin and tissues post-mastectomy to prepare for permanent implant placement. This review outlines the imaging characteristics of breast tissue expanders, emphasizing their types-based on positioning, lumen configuration, and valve design-and associated c

Effect of Operative Plane on Anesthetic Requirements in Tissue Expander Breast Reconstruction Patients.

BACKGROUND: Initially subpectoral, the plane of choice for tissue expander placement in breast reconstruction has evolved over the past decade, with more surgeons moving to prepectoral placement. In making this transition, our institution noted a subjective decrease in the anesthetic requirements of patients undergoing the second stage of their prepectoral reconstruction. Despite this, there is little published literature on the topic. Therefore, our study aims to examine the differences in anesthetic requirements among patients with subpectoral versus prepectoral expanders at the time of exch

Expanded occipito-cervico-pectoral flap for reconstruction of burned cervical contracture.

Postburn neck contracture and hypertrophic scarring can cause functional limitation and aesthetic disfigurement. Reconstruction of severe deformities and scar of neck following healing from burns confronts the surgeon with some of the most challenging problems in reconstructive surgery. Through knowledge of available reconstructive technique accurate diagnosis of tissue deficiency and secondary distortion, imaginative planning and definitive, careful execution of ones surgical plan are the bare minimum items for achieving improvement in a burned deformed neck. The aim of this article is to ass

Evaluation of Sidestream Darkfield Microscopy for Real-Time Imaging Acellular Dermal Matrix Revascularization.

BACKGROUND: Acellular dermal matrices (ADMs) serve as a regenerative framework for host cell integration and collagen deposition to augment the soft tissue envelope in ADM-assisted breast reconstruction-a process dependent on vascular ingrowth. To date noninvasive intra-operative imaging techniques have been inadequate to evaluate the revascularization of ADM. METHODS: We investigated the safety, feasibility, and efficacy of sidestream darkfield (SDF) microscopy to assess the status of ADM microvascular architecture in 8 patients at the time of tissue expander to permanent implant exchange du

Alopecia secondary to repaired occipital encephalocele - role of tissue expander in hair restoration.

Partial scalp alopecia is a common problem that can lead to severe social and psychological problems. Tissue expansion, although an old concept, provides a surgical alternative to manage areas of alopecia. We describe a case of alopecia secondary to repaired occipital encephalocele that was successfully treated using tissue expansion technique.

Outcomes of Air Versus Saline-filled Breast Expanders: A Systematic Review and Meta-analysis.

BACKGROUND: Among the possible implant-based reconstructive strategies, the two-stage tissue expander-to-implant procedure is one of the most common options in patients not ideal candidate to direct-to-implant reconstruction. Recently, other filling options such as air or carbon dioxide (CO2) have been reported as alternative fill media than saline for tissue expansion. The aim of this systematic review was to qualitatively and quantitatively synthetize the available evidence on the topic. METHODS: A systematic review and meta-analysis were conducted, and they were reported according to PRISM…

Post-mastectomy Breast Reconstruction With Gas vs Saline Tissue Expanders: Does the Fill Type Matter?

The most common reconstruction technique following mastectomy is a 2-stage technique that involves tissue expansion followed by definitive implant-based reconstruction (IBR). Tissue expanders (TEs) have classically used saline for initial fill; however, TEs with an initial gas fill (GTE)-including the CO2-based AeroForm (AirXpanders, San Francisco, CA) TE and TEs initially filled with atmospheric air-have been increasingly used in the past decade. We aimed to compare the outcomes in breast reconstruction for tissue expanders initially filled with saline vs gas. PubMed was queried for studies c…

Osmotic expanders in children: no filling--no control--no problem?

BACKGROUND: Self-filling, hydrogel-based osmotic tissue expanders have been successfully used for several years, mainly in adult patients. We wanted to test this novel device in pediatric plastic and reconstructive surgery. MATERIAL AND METHODS: Between November 2004 and September 2009, we implanted 53 osmotic tissue expanders following standard surgical principles in a total of 30 children and adolescents with burn scars, congenital nevi, alopecia, or foot deformities. RESULTS: All expanders reached their predicted volume within 6 weeks and 51 (96.2%) produced a sufficient amount of additio

Does the Use of Incisional Negative-Pressure Wound Therapy Prevent Mastectomy Flap Necrosis in Immediate Expander-Based Breast Reconstruction?

BACKGROUND: Mastectomy flap necrosis is one of the most common and significant complications in immediate expander-based breast reconstruction. Negative-pressure wound therapy is widely used for open wounds but is not commonly used for closed incisional wounds. However, the postoperative use of incisional negative-pressure wound therapy is demonstrated to reduce complication rates. The authors evaluate the incidence of mastectomy flap necrosis in patients with incisional negative-pressure wound therapy after immediate expander-based breast reconstruction compared with the incidence in patients

[A comparative study of the skin expansion rate and instantly expanded skin retraction rate between implantation of double overlapping expanders and implantation of one single expander].

OBJECTIVE: To comparatively study the difference of the skin expansion rate and instantly expanded skin retraction rate between implantation of double overlapping expanders and implantation of one single expander. METHODS: From Mar. 2009 to Mar. 2012, 22 cases with 39 sites for skin expansion, received double overlapping expanders in 24 sites, single expander in 15 sites. The area of original skin and expanded skin was measured by "wet-cloth sampling". Then the skin expansion rate was calculated. A distance of 5 cm at the center of expanded skin was re-measured after taking out the expanders.
