A new theory on the pathogenesis of acquired cholesteatoma: Mucosal traction.

OBJECTIVES/HYPOTHESIS: Although the migration of its squamous outer surface of the tympanic membrane has been well characterized, there is a … of the anterior pars tensa migrated radially toward the annulus. CONCLUSION: Mucosal coupling with traction generated by interaction of migrating opposing surfaces provides the first comprehensive theory that explains the observed characteristics of primary acquired cholesteatoma. The somewhat counterintuitive hypothesis that cholesteatoma is fundamentall …

[The preventive effect of flurbiprofen on preventing mesenteric traction syndrome].

BACKGROUND: Mesenteric traction syndrome (MTS) is associated with facial flushing, hypotension and tachycardia. The ways to treat MTS are fluid replacement, administration of vasopressors and nonsteroidal anti-inflammatory drugs (NSAIDs) such as flurbiprofen. In order to stabilize the hemodynamics during operation, preventing MTS is more reasonable. Thus, we investigated the preventive effect of flurbiprofen on MTS, the only injectable formulation which can be used in Japan. METHODS: In a prospective randomized study, 24 patients undergoing elective colorectal surgery were assigned to two gro…

Physiological changes as a result of hip arthroscopy performed with traction.

PURPOSE: To evaluate the physiological effects of hip arthroscopy using traction on venous blood flow, nerve conduction, soft-tissue injury, fibrinolysis, and patient pain. METHODS: Thirty subjects were prospectively analyzed in an institutional review board-approved study. The visual analog scale pain score, creatine phosphokinase (CPK)-MM level, and D-dimer test were obtained preoperatively, postoperatively, and 5 days postoperatively. Doppler ultrasound (group A) (n = 15) of femoral and popliteal venous blood flow and somatosensory evoked potentials (SSEPs) (group B) (n = 15) of the poster…

Gas gangrene as a result of femoral traction pin placement.

Treatment of adult femoral shaft fractures typically involves operative stabilization with intramedullary implants, external fixation, or a plate and screw construct. However, when stabilization is delayed for any reason, use of a traction pin is recommended to stabilize the fracture, prevent significant shortening, as well as to help with pain control. In this paper, we present the rare complication of a severe gas gangrene infection caused by Clostridium perfringens that led to several amputations and ultimately death. We also discuss risks of temporary skeletal traction and techniques to ov…

Nonprogressive tractional inferior retinal elevation in intermediate uveitis.

PURPOSE: To describe inferior peripheral retinal elevations associated with intermediate uveitis. METHODS: Retrospective review. RESULTS: Eleven eyes of 7 patients developed inferior retinal elevation secondary to intermediate uveitis. Six eyes (54.6%) were believed to have tractional retinoschisis, 2 (18.2%) had tractional retinal detachment, and the remaining 3 (27.3%) had flat retinal elevation of indeterminate type. In 10 eyes there was no evidence of progression during a mean 4-year follow-up. One eye developed extended tractional elevation with macular pucker and failed to respond to s…

Improvised skeletal traction in the management of ballistic femoral fractures.

Penetrating limb injuries are common during conflict, and in many there will be an associated fracture. Treatment of ballistic femoral fractures would usually be with by intramedullary nail; however, within the resource-constrained environment during conflict this is rarely possible. This report illustrates what can be achieved at a Role 2 facility to provide skeletal traction with the equipment and skills available. We discuss the history of skeletal traction and its use in ballistic femoral fractures, and believe that skeletal traction is still a valuable technique that we shouldn't ignore. …

Study of the traction resistance of mitral valve chordae tendineae.

OBJECTIVE: To determinate the extension and the resistance of the primary mitral valve chordae tendineae when submitted to traction. The importance of keeping the integrity of papillary muscle, chordae tendineae, and mitral valve cuspid when the replacement of this valve occurs is clear, but the knowledge of the maximum resistance that a primary tendinea chorda can withstand is not known. METHODS: Eight hearts were dissected, and one hundred and thirty two primary human chordae tendineae were measured (length and thickness) and submitted to traction under controlled conditions so that the abs…

Interexaminer differences in the traction test of the superior oblique tendon.

PURPOSE: The traction test of the superior oblique tendon is used for the manual evaluation of the looseness or tightness of the tendon. Because of the subjective nature of this test, knowledge of other clinical findings may lead to a bias in its interpretation. We studied interexaminer differences in the results of the test to assess the reliability of this traction test. METHODS: Examiner A examined all of the patients and collected all clinical findings, including the results of magnetic resonance imaging. Each of the nine noninformed examiners participated in one or more traction tests wi…

Tractional retinal detachment in Usher syndrome type II.

Retinal detachment is a rare complication in patients with retinitis pigmentosa. A case is reported of tractional retinal detachment in a patient with retinitis pigmentosa and sensorineural hearing loss, which was diagnosed as Usher syndrome type II. Because of the poor visual prognosis, the patient refused surgery in that eye. Tractional retinal detachment should be added to the differential diagnoses of visual loss in patients with retinitis pigmentosa.

Closed reduction of developmental dislocation of the hip by prolonged traction.

The efficacy of traction before an attempted closed reduction for patients with developmental dislocation of the hip remains controversial. We treated 55 children (62 dislocations of the hip) by preliminary, prolonged traction for a mean of eight weeks. All were followed up for at least two years in order to observe the development of any avascular changes within the femoral head. Of the 55 children, 27 (31 dislocations) were followed up until they were over six years of age. Fifty-seven of the 62 hips (92%) showed a successful closed reduction. Only one had radiological evidence of avascular …

Scar traction appliance for a patient with microstomia: a clinical report.

This clinical report illustrates how an active traction appliance was made for a patient with severe microstomia by attaching custom-made lip retractors to an orthodontic headgear. A steady, light tensile force was imparted to the lips with the elastic bands between the retractors and the headgear.

Guided abduction traction in the treatment of congenital hip dislocation.

Guided abduction is a form of overhead traction conceptually similar to the Pavlik harness. It is used in older children or in children in which the Pavlik harness has failed. The results of 27 congenitally dislocated hips treated by guided abduction traction from December 1979 to June 1989 were reviewed. Ages ranged from 1 month to 28 months. Twenty (74%) of the 27 hips underwent a gentle, often spontaneous, closed reduction, followed by abduction casting and bracing. Two hips developed radiographic evidence of avascular necrosis; five hips developed temporary irregular ossification. Closed r…

[Bilateral traction detachment in necrotizing retinitis as a sequela of toxoplasmosis].

A 35-year-old patient with systemic lupus erythematodes is presented. Two weeks after initiation of an immunosuppressive therapy with steroids and cyclophosphamide she developed a bilateral acute necrotising retinitis. Although the infiltration of the vitreous body was only slight the right eye developed a traction retinal detachment within a few days. A vitrectomy was performed but the situation was found to be inoperable due to central necrotic areas. Two days later serological specimen revealed an acute toxoplasmosis as the cause of the bilateral retinitis in this patient. A therapy with su…

[The results of using extraoral traction in children with a prognathic bite].

Primary and repeated lateral teleroentgenograms of the head of 37 children aged 4-8 with prognathic occlusion were analyzed, recorded on an average in 2 years after employment of a chin sling with extraoral traction. Analysis has shown normalization of the jaws and occlusion in the sagittal plane, resultant from limitation of mesial transposition of the mandible in the skull in the course of facial skeleton growth. This has been achieved at the expense of inhibition of longitudinal growth of the mandibular body and ascending branches more than twofold as against the reference values and was al…

Preoperative intermittent manual traction in congenital dislocation of the hip.

To avoid prolonged hospitalization and the complications of skin and skeletal traction, we have devised a method of intermittent preoperative manual traction in the treatment of congenital dislocation of the hip. Of 60 hips that were followed for more than 1 year, 88.3% had Class I or Class II results according to Severin's criteria. No hip in this series was complicated by avascular necrosis of the proximal femoral epiphysis.

Computerised axial tomography in traction injuries of the brachial plexus.

Severe traction injuries may damage the brachial plexus at any level from the spinal cord to the axillary outlet. Investigation aims to determine the level of the injury for each of the nerves, trunks or cords, with particular reference to obtaining firm evidence of any intradural (pre-ganglionic) damage. We report the results of computerised axial tomography of the cervical spine in comparison with conventional myelography and with surgical exploration of the plexus. CT scanning with contrast enhancement greatly improves diagnostic accuracy, particularly at C5 and C6 root levels.
