Success of transition to adult care in patients with pediatric-onset chronic liver disease.

OBJECTIVES: Previous studies on chronic pediatric-onset conditions have highlighted the risks of loss to follow-up, disease progression, or therapeutic nonadherence during transition. However, very few studies have focused on liver diseases. We aimed to evaluate the rate of successful transition in a cohort of patients with chronic liver disease, and assess its evolution over time and factors associated with transition failure. METHODS: We conducted an observational, retrospective, monocentric study at our tertiary hospital. Ninety-three patients attending the pediatric hepatology clinic who

Understanding Transition From Home to Inpatient Setting in Palliative Care: A Qualitative Synthesis.

CONTEXT: Patients receiving palliative care often experience multiple transitions among care settings at the end of life, with transitions from home to inpatient settings being the most common. However, how these transitions occur and how care needs and preferences change over time have not been systematically synthesized. OBJECTIVE: To synthesize the experiences of patients receiving palliative care and their caregivers regarding transitions from home to inpatient settings. METHODS: A qualitative evidence synthesis was conducted following JBI guidance and reported in accordance with ENTREQ.

Navigating the Continuum: System-Level Requirements for High-Quality Dementia Care Transitions.

The worldwide rise in dementia cases is creating significant pressures on healthcare systems, clinicians, and decision-makers. As dementia does not follow a single predictable pathway, individuals commonly move across different care settings as their needs evolve, requiring coordinated and integrated responses across the healthcare system rather than isolated efforts within single services. This perspective study explores service quality and underlying structural challenges throughout the dementia care pathway with a particular concentration on transitions between various stages. Key barriers

Inpatient Pediatric-to-Adult Transition of Care Consultation Delivery: A Mixed-Methods Study.

AIM: To investigate the effect of carvedilol on liver fibrosis and hepatic sinusoidal capillarization in mice with carbon tetrachloride (CCl4)-induced fibrosis. METHODS: A liver fibrosis mouse model was induced by intraperitoneal CCl4 injection for 8 weeks. The mice were divided into five experimental groups: the normal group, the oil group, the CCl4 group, the CCl4+carvedilol (5 mg/kg/d) group, and the CCl4+carvedilol (10 mg/kg/d) group. The extent of liver fibrosis was evaluated by histopathological staining, and the changes in fenestrations of hepatic sinus endothelial cells were observed

Quality of Care Transition and Patient Safety: An Investigation Into Patients After Bariatric Surgery.

BACKGROUND: High quality care transition (QCT) is key to mitigating patient harm after hospital discharge to home. Nevertheless, patients' experiences of QCT are rarely investigated, particularly in acute surgical settings. Given the high relevance of bariatric surgery as the gold standard in the treatment of severely obese patients, mitigation of patient harm is gaining traction. Therefore, we aimed to determine the association between patient-experienced QCT and safety incidents and health complications after bariatric surgery. METHODS: Cross-sectional survey study. Using standardized instr

Three Decades of Health Care Transition in Rheumatology: Progress, Challenges, and Future Directions.

The revolution of treatment advances for children with rheumatic diseases since the advent of pediatric rheumatology has coincided with the growing recognition that many of these children will continue to require long-term rheumatology care after reaching adulthood. Moreover, it has become evident that effective, systematic approaches are needed to help adolescents and young adults (AYAs) with pediatric rheumatic diseases learn to manage their health independently and succeed in an adult-oriented model of health care. In this narrative literature review, we provide a historical perspective on

[Transition of Patients with Neurogenic Bladder Dysfunction: Challenges and Unmet Needs in Adult Care].

Transition describes the structured process by which adolescents are transferred from pediatric and family-centered, and thus predominantly externally determined, care to patient-centered, self-determined adult care. In patients with spina bifida (SB), transition is particularly relevant from a urological perspective, as neurogenic bladder and bowel dysfunction frequently require chronic pharmacological treatment, permanent bladder management (including intermittent self-catheterization), and lifelong monitoring of kidney and bladder function. At the same time, physical limitations and frequen

Transitioning care in cystic fibrosis: a comprehensive review of reviews of clinical and psychosocial outcomes.

BACKGROUND: With rising life expectancy in cystic fibrosis (CF), effective transition from pediatric to adult care is essential. Structured transition models are thought to improve continuity of care, clinical stability, and psychosocial outcomes. This review of reviews synthesizes systematic review evidence on the effectiveness of these models across healthcare systems. METHODS: Reviews and systematic reviews with or without meta-analysis published between 2005 and 2025 were identified through comprehensive searches. Methodological quality was assessed using AMSTAR 2, and primary study overl

Advancing Diabetic Foot Ulcer Care: Focus on the Post-Healing Transition Phase.

The recurrence of diabetic foot ulcers (DFUs) brings significant morbidity to people with diabetes and adds to healthcare costs. According to current guidelines, a DFU is considered healed when it is re-epithelialized, and the wound closure is maintained for around 2 weeks. However, recent literature suggests that the mechanical properties of the underlying plantar tissues remain altered, thus making the foot vulnerable to recurrence. The period lasting several weeks post-DFU healing can be termed the 'transition phase'. This review aimed at exploring this critical phase, with an emphasis on t

From foster care to incarceration: A prospective analysis of the National Youth in Transition Database.

BACKGROUND: Children in foster care often face significant adversity in adulthood, including a heightened risk of incarceration. Yet, it is uncertain whether adult incarceration rates differ between youth who age out of foster care, youth who are reunified with their families, and those adopted or placed with a legal guardian. OBJECTIVE: This prospective study investigates the prevalence of adult incarceration for youth in care at age 17 and examines whether the risk of incarceration varies by foster care exit type, both overall and among different racial/ethnic groups. Family perceptions of the transition from hospital to home palliative care: a phenomenological study.

PURPOSE: Home palliative care allows people in a palliative situation to remain in the comfort of their own homes. However, this transition poses significant challenges to family members, requiring logistical, physical, and emotional adaptations. This study aimed to explore the lived experiences of family members caring for a person in a palliative situation at home, focusing on the challenges, emotions, and coping strategies during the transition from hospital to home. METHOD: A descriptive phenomenological study was conducted with 20 family members involved in the care of patients followed

Transitioning from pediatric- to adult-focused dialysis care: A systematic review with recommendations.

Several andrological diseases require surgical repair or reconstruction of tunica albuginea, which envelops the corpora cavernosa penis. Despite intense research efforts involving a variety of biological materials, such as skin, muscle aponeurosis, human dura mater, tunica vaginalis, and pericardium, engineered tunica albuginea suitable for graft use is yet to be obtained. The study investigates microsurgical tunica albuginea allotransplantation in an animal model with the purpose of creation of an organ-specific tissue bank to store penile tissue, from cadaveric donors and male-to-female tran

Health services' responses to transitioning adolescents to adult HIV care in South Africa.

BACKGROUND: Adolescents living with human immunodeficiency virus (HIV) aged 10-19 years account for 1.7 million globally, with 82% residing in sub-Saharan Africa. Older adolescents (15-19 years) assume greater responsibility for their own care, often leading to reduced adherence, lower retention, and poorer health outcomes. Understanding the role of healthcare workers (HCWs), key stakeholders in the HIV care continuum, is essential to strengthening transition practices and health system responses. AIM: To describe HCWs' perspectives on transition practices for adolescents living with HIV in t

Transitions between care networks: a prospective study among older adults in the Netherlands.

As health impairment increases, older adults utilize care from different types of caregivers, but little is known about changes in the composition of care networks. We mapped the transitions between different care networks to gain insight into which people develop care networks that include informal, privately and publicly paid care. We used three waves (2012-2015-2018) of the Longitudinal Aging Study Amsterdam with 1413 Dutch community-dwelling adults, aged 64-100. Network types were identified using six types of caregivers: (1) co-resident, (2) non-co-resident children, (3) other kin, (4) ne

Navigating the Journey: Transitioning Spinal Cord Injuries from Acute Care to Rehabilitation.

PM&R plays an essential role in managing the individual with a spinal cord injury (SCI). Receiving care at an acute hospital and a rehabilitation center familiar with SCIs is critical. PM&R can guide the health care team and the patient/family through such a potentially life-changing event. Early consultation can help prevent complications and decrease stay length and mortality. A life-long relationship with the rehabilitation team is often recommended; team members such as navigators and multiple inpatient rehabilitation stays can help enhance care and opportunities for individuals with SCI.

Development of a Standardized Process for Transition to Adult Care in a Pediatric Colorectal Surgery Clinic.

BACKGROUND: As pediatric patients with colorectal diseases grow, it is important to address transition to adult practice. We aim to describe our center's transition process and early outcomes. METHODS: We developed a standardized process for transition to adult practice. An annual survey is given to parents and caregivers starting at age 12 that assesses knowledge of disease, independence with healthcare tasks, and confidence and interest regarding transition. After multidisciplinary review, those eligible are recommended for transition. Those not referred are provided with tools to help with

Markers of Fertility in Adolescents With Chronic Endocrinopathies at Transition From Paediatric to Adult Care.

OBJECTIVE: During the process of transition from paediatric to adult health care, counselling concerning fertility is an important issue and is based mainly on serum markers of gonadal function. Here, we analysed these markers in adolescents with various underlying endocrine diseases at the time of transition. METHODS: After reaching near adult height and late puberty (girls: bone age [BA] ≥14 years, and boys: BA ≥16 years), we assessed stages of puberty according to Tanner and measured testes or ovarian volumes and serum markers of gonadal function (anti-Mullerian hormone [AMH], inhibin B, 1
