Mandibular central mucoepidermoid carcinoma with distant metastasis.

Mucoepidermoid carcinoma is the most common malignant salivary gland tumour. Central mucoepidermoid carcinomas (CMC) are an exceedingly rare subgroup, representing 2-4% of all cases. The authors report on a case of CMC in a patient who presented with trigeminal neuropathy, trismus and distant metastasis. The possible pathogenesis of CMC and criteria for diagnosis are discussed.

Cephalic tetanus with left infranuclear facial palsy.

We report a case of 11 years old male who developed trismus associated with left infranuclear facial palsy following injury over forehead. Cephalic tetanus was diagnosed. Rarity of the case as well as mild course it ran, though the incubation period was less than seven days, is the reason for this case report.

Prophylactic use of indomethacin for prevention of postsurgical complications after removal of impacted third molars.

Indomethacin (100 mg. per day for 3 days) was tested for its prophylactic effects on the postsurgical complications of pain, swelling, and trismus after removal of impacted teeth. The drug resulted in a significant reduction of the swelling and an analgesic effect which was equal to that of acetaminophen and codeine. Only two minor side effects were encountered.

Horner's syndrome in severe tetanus.

A 66 year old man was admitted with a left Horner's syndrome, and trismus due to tetanus. Three days later he had respiratory arrests, classical tetanic spasms and was ventilated. He had associated severe autonomic dysfunction, tachyarrhythmias, hypotension, sweating and constipation. There was complete resolution of the left Horner's syndrome with recovery from tetanus.

Tunnelling Versus Non-tunnelling Techniques in Posterior-based Buccinator Myomucosal Flap Reconstruction - A Prospective Study.

INTRODUCTION: The posterior-based buccinator myomucosal flap is a reliable option for reconstructing posterior oral defects, though pedicle transfer methods vary. This prospective study compared functional outcomes, complications and burden of care between tunnelling and non-tunnelling techniques. MATERIALS AND METHODS: Patients were randomised into Group A (tunnelling, n = 20; single-stage) and Group B (non-tunnelling, n = 20; two-stage). Primary outcomes were flap survival and need for secondary surgery. Secondary outcomes included trismus, oral opening and mobility. Data were analysed usin…

Clinical and magnetic resonance imaging findings in patients with and without improvement after mandibular manipulation and conservative physical therapy for temporomandibular joint disorders.

OBJECTIVE: This study compared clinical and magnetic resonance imaging (MRI) findings in patients with anterior disk displacement without re … ch group. RESULTS: Significant differences between the groups included age, period of awareness of trismus, and maximum mouth opening (MMO). The MRI findings revealed significant differences in the degree of ADD and morphology of the disk and condyle. MMO significantly improved in the WOR-WR group between initial and follow-up visits. CONCLUSION: Patien …

Oral Care in Head and Neck Radiotherapy: Proposal for an Oral Hygiene Protocol.

This review aims to provide a comprehensive overview of the literature on the oral side effects caused by radiotherapy for head and neck cancers. Various treatments are examined to mitigate these sequelae, and a protocol is proposed for dentists and dental hygienists to manage oncological patients. A literature search was conducted to select relevant articles addressing the effects of radiotherapy treatments on the oral cavity, with a particular focus on the development of mucositis, candidiasis, changes in salivary pH, trismus, fibrosis, and alterations in the oral biofilm. PubMed and MedLine…

A prospective study examining the effects of treatment timing in the management of mandible fractures.

The ideal timing for treatment of mandible fractures has not been well established. The objective of this study was to analyse the effects of treatment timing in the surgical management of mandible fractures. A prospective evaluation of 215 continuous patients with a total of 359 mandible fractures was undertaken. Nine outcome variables were analysed in relation to treatment delay by logistic regression modelling: wound dehiscence, hardware exposure, local postoperative infection, malocclusion, trismus, nerve damage, fracture non-union, return to theatre, and radiographic outcome. Nineteen add…

[Cephalic tetanus and Bell's palsy in an elderly man].

Cephalic tetanus is a rare form of tetanus defined as trismus plus paralysis of one or more cranial nerves. The seventh cranial nerve is most commonly involved. The process can remain localized, or become generalized. We describe the case of an elderly Caucasian man who developed cephalic tetanus after head trauma with a facial wound. Bell's palsy was inaugural.

Postoperative effects of horizontal platelet-rich fibrin after impacted third molar extraction: a randomized controlled trial.

This double-blind randomized controlled trial was performed to evaluate the effects of horizontal platelet-rich fibrin (H-PRF) and leukocyte-platelet-rich fibrin (L-PRF) on postoperative outcomes after mandibular third molar extraction. Seventy-five patients (25 per group) were allocated to H-PRF, L-PRF, or control (no PRF). Pain was assessed on postoperative days 1-7, while swelling, trismus, and soft tissue healing were assessed on days 2 and 7. Quality of life (QoL) was assessed by questionnaire on day 4, and patients recorded the duration of QoL impacts during the first postoperative week,…

[Lockjaw in progressive myositis ossificans].

Frequency, symptomatology, course and therapy of myositis ossificans progressiva are described by means of literature references and personal observation in a patient. The patient was born in 1935 and the disease manifested itself for the first time in 1956 in the right sternocleidomastoid muscle. Despite surgery total trismus could not be prevented and later-on muscles and connective tissue of other areas ossified, too.
