Ulnar nerve lesion at the wrist related to pisotriquetral joint arthropathy.

INTRODUCTION: Ulnar nerve lesions at the wrist (UNLW) are always difficult to localize clinically and sometimes electrophysiologically. Finding conduction block when studying ulnar motor nerve conduction (CB) across the wrist is sometimes the only way to demonstrate that the ulnar deep motor branch (UDMB) is entrapped. METHODS: An elderly woman who had bilateral carpal tunnel syndrome (CTS) and thumb osteoarthritis for many years experienced worsening of left hand impairment recently. RESULTS: Electrodiagnostic and ultrasound examinations revealed an acute and severe UDMB lesion related to p

First Case of Combined Carpal and Cubital Tunnel Syndromes in an Adolescent: A Case Report and Literature Review.

Carpal and cubital tunnel syndromes are the two most common compressive neuropathies, but both carpal and cubital tunnel syndromes are extremely rare in children. Therefore, the combination of carpal and cubital tunnel syndrome in the pediatric population is even more uncommon. These neuropathies have multiple causes, with the three main categories being mechanical injury, metabolic, and idiopathic. Here, we present the unique case of a 15-year-old female with no known genetic or physical risk factors who was diagnosed with atraumatic combined carpal and cubital tunnel syndrome with severe, ch

Ulnar neuropathy with and without elbow instability.

Increased valgus stresses on the medial aspect of the elbow can result in a number of bony and soft-tissue abnormalities. The most common abnormalities are ulnar neuropathy and valgus instability secondary to injury of the medial collateral ligament. Diagnosis and treatment of these disorders are discussed in this article.

[Bicycle rider's ulnar nerve paralysis].

The ulnar neuropathy in bicycle riders is a less common occurrence, due to a local damage of the nerve on the level of the ulnar tunnel. It has been described in neurological literature only a few times, and hence its good prognosis is not sufficiently known. Within a short time two patients of this disease were transferred to our clinic for operation. After a local space-occupying growth in the tunnel could be excluded by MR or sonography of the wrist, we discouraged performance of an operative procedure. The patients' complaints and the neurological deficits vanished in a few weeks. Ulnar ne

Role of surgical decompression in ulnar neuritis of leprosy.

The present study was undertaken to evaluate the effect of neurolysis in relieving the pain of acute painful neuritis and assess its effect on the sensory motor deficit. Thirty eight ulnar nerves were operated and followed up for 8 to 125 weeks. Following surgery relief from pain was gratifying. However the pain recurred in cases of lepromatous leprosy in subsequent episodes of ENL reaction. Sensory recovery was appreciable and occurred within 8 weeks. Four cases had 75% recovery; five had 50 to 75% recovery and 11 cases had up to 50% recovery. Motor recovery took a longer time to appear. Prox

The acute carpal tunnel syndrome: nine case reports.

Nine cases of acute carpal tunnel syndrome are reported. Etiologies include: bleeding secondary to chronic lymphatic leukemia; Colles' fracture of the wrist (2 cases); Epiphyseal fracture (Salter II) of the distal radius; Bleeding secondary to giant cell tumor of the tendon sheath; Unstable distal radio-ulnar joint; Displaced intra-articular fracture of the distal radius; Rheumatoid synovitis and vasculitis; Trans-scaphoid, perilunar fracture dislocation of the wrist. Early recognition of median nerve compression in the carpal tunnel is vital. The signs of median nerve compression should be lo

Ultrasound-guided percutaneous release of the ulnar nerve at the elbow.

INTRODUCTION: Surgical treatment of ulnar tunnel syndrome is controversial, with no consensus on the ideal procedure. Nevertheless, in-situ decompression seems to provide faster recovery with less morbidity than neurolysis-transposition. The aim of this retrospective study was to validate a new percutaneous technique using ultrasonography. METHODS: A retrospective 2-center study included 40 patients with McGowan stage 1 or 2 ulnar nerve entrapment at the elbow treated by a new limited ultrasound-guided percutaneous technique. Patients with McGowan stage 3, osteoarthritis at the elbow or insta

Electrodiagnostic evaluation of carpal tunnel syndrome and ulnar neuropathies.

Carpal tunnel syndrome (CTS) and ulnar mononeuropathies at the elbow and wrist are the most common nerve entrapments in the upper extremities. Electrodiagnostic studies are a valid and reliable means of confirming the clinical diagnosis. This review addresses various electrodiagnostic techniques to evaluate the median and ulnar nerves at the wrist and elbow. It also discusses the limitations of electrodiagnostic studies with regard to the sensitivity and specificity of such testing.

[Clinical and electrophysiological findings in carpal tunnel syndrome].

Carpal tunnel syndrome (CTS) is the most common nerve entrapment disorder. The clinical features of CTS are variable, but usually include pain and paresthesia in the thumb, first two fingers, and the radial-half of the ring finger. Paresthesia and sensory deficits might involve the entire palm area in some cases. Pain frequently radiate proximally into the forearm, and occasionally to the shoulder. Many patients experience pain at night and are awakened by abnormal sensations. Shaking hand relief the symptom. The two classic tests for nerve compression at the wrist are the Tinel test and the P

Proximal versus distal slowing of motor nerve conduction velocity in the Guillain-Barré syndrome.

Using the F wave, a simple equation was devised to calculate the ratio between motor nerve conduction time from the spinal cord to the stimulus site and that of the remaining nerve segment to the muscle (F ratio). In 33 healthy subjects the F ratio (mean +/- SD) was close to unity for the median nerve (1.04 +/- 0.09) with stimulation at the elbow and for the peroneal (1.11 +/- 0.09) and tibial nerves (1.17 +/- 0.10) with stimulation at the knee. Thus, in these nerves, the time required for the passage of impulses from the cord to the site of stimulation is approximately the same as that from t

Nerve entrapment syndromes.

Peripheral nerve entrapment syndromes may be caused by inflammation, trauma, or congenital deficits. Some entrapment neuropathies are overdiagnosed, particularly carpal tunnel syndrome. Other nerve entrapment syndromes, such as the thoracic outlet syndrome, posterior interosseous nerve syndrome, and anterior interosseous nerve syndrome, are rare. In most cases of an entrapment neuropathy, the diagnosis and management are straightforward, but much remains to be learned about the natural history, course, and prevention of these clinical conditions.

Hypothenar hammer syndrome: A case of a late complication after surgery.

Hypothenar hammer syndrome is a rare condition secondary to ulnar artery damage in Guyon's canal, affecting mainly those exposed to repeated palm trauma. Surgery is discussed in cases of severe symptoms that are resistant to conservative treatment, and/or when anatomical lesions with high embolism potential are discovered during imaging exams. Resection of the pathological zone with revascularization by autologous vein graft is the best option. We report the case of a 60-year-old patient who had a recurrence of symptoms more than 10 years after this type of surgical treatment was performed. Th

[Compression of the ulnar nerve at the elbow. Results of a series of 51 medial epicondyle osteotomies associated with decompression].

PURPOSE OF THE STUDY: Despite many publications concerning the physiopathology and the treatment of Ulnar tunnel syndrome treatment remains controversial. MATERIAL AND METHODS: The authors reviewed 51 patients operated on for ulnar nerve entrapment at the elbow by neurolysis combined with medial epicondylectomy in case of luxation or subluxation of the nerve. Average was 39 years, 74 per cent being males, 53 per cent manual workers. According to Mc Gowan's classification, 39 per cent were grade I, 12 per cent grade IIA, 20 per cent grade IIB and 29 per cent grade III. RESULTS: Few postoperat

Problems With Medium-Sized Joints: Wrist Conditions.

Common wrist conditions include fractures and other injuries, osteoarthritis, radial epiphysitis, joint instability, de Quervain tenosynovitis, carpal tunnel syndrome, ganglion cyst, and ulnar neuropathy. The initial history and physical examination, with particular focus on the anatomic structures of the wrist, can narrow the differential diagnosis. Magnetic resonance imaging study can be used to identify soft tissue masses and occult osseous processes, particularly with scaphoid fractures. Computed tomography scan is useful in cases of bony abnormalities, high clinical suspicion of occult fr
