A subjective and urodynamic assessment of unilateral bladder neck incision for bladder neck obstruction.

Thirty-eight patients with bladder neck obstruction were treated by unilateral bladder neck incision. Good subjective and urodynamic results were obtained in 33 patients. Retrograde ejaculation occurred in 16% of the patients on recommencing sexual activity. The value of combined videocystography and pressure flow studies is discussed. The technique is not recommended for patients presenting with acute retention.

The adynamic terminal ureteral segment.

Recently, we observed hydroureteronephrosis owing to an adynamic transmural distal ureteral segment associated with a thickened bladder, consequent to bladder outlet obstruction. Histological examination disclosed a loss of intrinsic musculature of the terminal ureteral segment with fibrosis. The importance of this entity is in the fact that upper tract dilatation and deterioration may progress, despite apparently successful correction of the bladder outlet uropathy.

The measurement of urethral resistance and mural tension using simple programmable modules.

The theory of urethral resistance and the derivation or mural tension have been discussed. A method for the continuous calculation and presentation of these parameters has been described. Urethral resistance and mural tensions are altered by the presence of bladder outflow obstruction and have been shown to return to normality after the obstruction is removed.

From black urine to bladder obstruction: multidisciplinary management of extensive stone formation in alkaptonuria.

Alkaptonuria, although rare, can lead to severe multisystemic manifestations, including progressive ochronosis and extensive urological complications. This case highlights an unusually diffuse stone burden involving the entire urinary tract, emphasising the importance of early recognition of classic signs such as dark urine and joint stiffness. Timely biochemical confirmation and imaging can aid in the appropriate diagnosis. Management requires a multidisciplinary approach encompassing metabolic control, surgical intervention and long-term follow-up. Greater awareness of this condition among c

Surgical management of bladder outlet obstruction due to functional and anatomical etiologies in women.

INTRODUCTION: Bladder outlet obstruction (BOO) in women includes functional and anatomic etiologies. Primary bladder neck obstruction (PBNO), Fowler's syndrome (FS), and dysfunctional voiding (DV) are some examples of functional obstructions, whereas pelvic organ prolapse (POP), periurethral masses, and intragenic causes are some of the anatomic causes. METHODS: This literature review describes the etiologies of female BOO, unique aspects of the workup and diagnosis, and the data for the standard surgical treatments and newer surgical techniques to treat women. Urethral stenosis and sling-rel

Nonneurogenic female bladder outlet obstruction: Conservative and medical management.

INTRODUCTION: In nonneurogenic female bladder outlet obstruction (BOO), management goals include reduction of outlet resistance to increase urinary outflow and improve bladder voiding to prevent or reduce lower and upper urinary tract (LUT and UUT) function deterioration, by correcting the underlying etiology. As significant progress has been achieved in the conservative and pharmacological management of nonneurogenic female BOO, the purpose of this article is to review and summarize the current literature. MATERIALS AND METHODS: For this narrative review, a PubMed® search was performed by cr

Management of Bladder Neck Contracture in the Age of Robotic Prostatectomy: An Evidence-based Guide.

The incidence of bladder neck contracture (BNC), or postprostatectomy vesicourethral anastomosis, has declined since the advent of robotic surgery. However, men with peripheral vascular comorbidities, among other factors, still have a high risk of developing this complication after any surgery that involves manipulation of the prostate. The best strategy for BNC management remains uncertain because of inconsistency in success for different approaches across studies. We reviewed the available evidence on BNC, including the results for endoscopic treatments, scar modulation therapies, and open a

Diagnosing Bladder Outlet Obstruction Using Non-invasive Decorrelation-Based Ultrasound Imaging: A Feasibility Study in Healthy Male Volunteers.

A feasibility study on the applicability of an ultrasound decorrelation method to urinary flow imaging was carried out in 20 healthy male volunteers, to provide a basis for a non-invasive approach to diagnose bladder outlet obstruction. Each volunteer voided five times in a flow meter in standing position. During each voiding, ultrasound radiofrequency frames were acquired transperineally at different flow rates. The results indicated that the decrease in correlation (decorrelation) of ultrasound radiofrequency signals had no unique relation with flow rate, but decreased distinctively with uri

The effect of partial outlet obstruction on calpain and phospholipase-2 activities: analyzed by severity and duration.

In an attempt to better understand the two pathways that lead to bladder decompensation following partial obstruction in rabbits one of which is caused by calcium-activated enzymes and the other by oxidative stress, calpain and phospholipase A2 (PLA2) biochemical assays were conducted to see how bladder decompensation is mediated by these two calcium-activated enzymes. Partial outlet obstructions of varying durations (4, 8, and 12 weeks plus controls) were performed on 32 New Zealand white rabbits. The rabbits were also grouped by severity: control, mild, intermediate, and severe. The activiti

Protective effects of inosine on urinary bladder function in rats with partial bladder outlet obstruction.

OBJECTIVES: To evaluate the protective effects of inosine on bladder dysfunction, nerve density, and oxidative damage after partial bladder outlet obstruction in rats. METHODS: A total of 60 adult male Sprague-Dawley rats were divided into 5 groups. Groups 1-4 underwent bladder outlet obstruction. Groups 1-3 were treated with inosine at 100, 150, or 225 mg/kg/d intraperitoneally and group 4 with saline. Group 5 consisted of sham-operated control rats. At 21 days postoperatively, cystometry were performed in 30 rats (6 per group). In the remaining 30 rats (6 per group), the bladders were excis

Effect of partial outlet obstruction on nitrotyrosine content and distribution within the rabbit bladder.

PURPOSE: Evidence indicates that free radicals are etiological factors in obstructive bladder disease. However, it is not clear which species of reactive oxygen or nitrogen species mediate the damage. The current studies were designed to determine if partial outlet obstruction in rabbits results in the generation of nitrotyrosine (NT). MATERIALS AND METHODS: Sixteen rabbits were separated into four groups of four. The rabbits in groups 1 and 2 underwent sham operation while rabbits in groups 3 and 4 underwent partial outlet obstruction. The rabbits in groups 1 and 3 were evaluated after 1 wee

Bladder obstruction in Hunter's syndrome.

We report a case of bladder obstruction in a patient with Hunter's syndrome, presenting with acute painful symptomatology, due to the impossibility of voiding, which was diagnosed with ultrasonography and cystometrography. Intermittent catheterization with intravesical oxybutynin chloride lead to successful functional resolution of the obstruction.

Filling mechanics of obstructed and de-obstructed rat urinary bladders.

Chronic bladder distension occurs after partial outlet obstruction and can lead to decompensation and impaired function. To quantify the degree of chronic bladder distension, we previously defined the zero pressure volume (ZPV), the largest contained volume at zero transmural pressure. In the current study, we investigated the short- and long-term effects of outlet obstruction and de-obstruction on chronic distension and passive bladder filling mechanics. Voiding patterns were measured 10 days (short term) or 6 weeks (long term) after partial bladder outlet obstruction and the bladders were te

Effects of outlet obstruction and its reversal on mitochondrial enzyme activity in rabbit urinary bladders.

PURPOSE: This study investigated the effects of outlet obstruction and its reversal on mitochondrial enzyme activity in the rabbit urinary bladder. MATERIALS AND METHODS: We induced mild bladder outlet obstruction in male New Zealand White rabbits. Following two weeks of obstruction, one group of animals (n = 6) was sacrificed, while outlet obstruction was relieved in three additional groups of animals, which were sacrificed one (n = 5), two (n = 5) and four (n = 5) weeks after relieving the obstruction. Seven sham operated rabbits served as controls. We extracted mitochondria from fresh detr
