Fetal breathing: induction in utero and effects of vagotomy and barbiturates.

Breathing movements were induced in the mature fetal lamb in utero by (1) both electrical stimulation and manipulation of the sciatic nerve, … nsity, sustained regular breathing. The breathing responses were not affected by bilateral cervical vagotomy, but were inhibited completely by maternal administration of pentobarbital and of quantities of thiopental and methohexital equal to or less than the doses used clinically.

Late effects of proximal gastric vagotomy compared with antrectomy and selective vagotomy for chronic duodenal ulcer. A randomized study with 5-year follow-up.

Twenty-three patients who had been given a proximal gastric vagotomy and 29 patients who had had an antrectomy were examined periodically for 5 years after their operations for duodenal ulcers. Five years after surgery, 83% of the proximal gastric vagotomy patients and 86% of the antrectomy and selective vagotomy patients were included in Visick grades I-II. We found 4/24 recurrent ulcers in the vagotomy group and 1/29 in the antrectomy group; in addition 3 of the antrectomy patients had to be reoperated. Acid secretion was reduced by 54% in the vagotomy patients and by over 90% in the antrect…

Clinical results of parietal cell vagotomy and selective vagotomy with pyloroplasty in the treatment of duodenal ulcer. Two-year follow-up of a prospective randomized study.

Fifty patients were randomized in a consecutive series of parietal cell vagotomy (PCV) and selective vagotomy with pyloroplasty (SV +P) in the treatment of duodenal ulcer. There were no operative deaths, and the length of the hospital stay and time off from work were the same in both groups. The clinical results were evaluated at 1 and 2 years after operation. Within the first 3 years there were two recurrences after PCV and three after SV +P. The overall clinical result 2 years after surgery did not significantly differ between the two groups; 16/24 after PCV and 17/23 after SV +P classified …

Lymphocyte Antigen 6G Mediates Vagotomy-Associated Reduction in Body Weight.

Dysregulation of adipocyte function is an important component of metabolic and cardiovascular diseases. It is increasingly evident that inte … with controls. Vagotomized animals showed increased Ly6G+ cell infiltration in eWAT. Interestingly, vagotomy-associated weight loss was significantly attenuated in neutrophil-deficient Ly6GcreMcl1fl/fl mice and in mice treated with repeated injections of anti-Ly6G antibodies. Together, these observations indicate that there is a Ly6G-mediated vagotomy-asso …

Pantothenic acid alleviates endometrial injury induced by vagotomy in mice.

Endometrial injury triggers inflammation responses, and persistent inflammation is a recognized cause of infertility. Emerging evidence underscores the critical role of the vagus nerve in modulating immunity. Although vagotomy is known to induce systemic inflammation, the specific mechanism by which the vagus nerve regulates uterine health is still unclear. In this study, we employed a vagotomy model to investigate the therapeutic potential of pantothenic acid in alleviating endometrial injury. Our results showed that left cervical vagotomy reduces the integrity of the endometrium and the expr…

Laparoscopic-assisted truncal vagotomy and gastro-jejunostomy: trial of simplification.

BACKGROUND: Truncal vagotomy and gastro-jejunostomy is an effective procedure for Gastric Outlet Obstruction. The authors' experience with the procedure is presented and an alternative technique for creating the gastro-jejunostomy is described. METHODS: From January 2005 through June 2007, 18 patients with gastric outlet obstruction secondary to a cicatrizing peptic duodenal ulcer underwent laparoscopic truncal vagotomy and an ante-colic laparoscopic-assisted stapled gastro-jejunostomy. RESULTS: All procedures were completed laparoscopically with a mean operative time of 81.2 +/- 11.1 minute…

[Duodenal ulcer: truncal vagotomy with celioscopy. Report of 53 cases].

The recent introduction of laparoscopic surgery in the therapeutic arsenal of duodenal ulcer enabled us to carry out these interventions by abdominal route in order to appreciate the early results. It concerned a prospective study from March 1999 to August 2000 of 53 cases of which were 47 men and 6 women, aged from 16-75 years average age 52 years. Twenty six patients had pyloro-duodenal stenosis, the rest, chronic duodenal ulcer. Vagotomy was carried out on a patient on dorsal decubitus under general anesthesia with oro-tracheal intubation operator within the legs of the patient and the moni…

Thoracoscopic vagotomy for peptic ulcer disease--report of two cases.

Thoracoscopic vagotomy is a well-established technique in the management of recurrent peptic ulceration after previous abdominal vagotomy. The procedure allows an easy approach to the vagus nerves without having to dissect through dense adhesions from previous surgery. With thoracic vagotomy, the main morbidity is postoperative pain from the thoracotomy incision and rib spreading. Advances in endoscopic camera systems and instrumentation allow thoracic vagotomy to be safely and effectively accomplished by thoracoscopic surgery techniques, with reduced postoperative pain. We describe our experi…

Recurrent peptic ulceration after highly selective vagotomy: long-term outcome.

A prospective assessment was made of the outcome 4 years after diagnosis of recurrence in a group of 27 patients with documented ulceration after highly selective vagotomy (16 symptomatic recurrence and 11 asymptomatic). In the 16 patients with a previous symptomatic recurrence, eight of the 11 patients with duodenal ulcer underwent a further endoscopy at 4 years and one active ulcer was found. Five patients with previous symptomatic gastric ulcer recurrence have all undergone further surgery. In the 11 patients who originally had an asymptomatic ulcer recurrence (five gastric, six duodenal) n…

[Reflux esophagitis at remote periods after vagotomy for duodenal ulcer].

Results of a comparative clinico-instrumental and morphological examination of 163 patients are described. Vagotomy was shown not to exert great influence on the function of lower esophagus sphincter. In patients with duodenal ulcer and reflux-esophagitis it is expedient to perform anti-reflux interventions on the cardia to prevent further development of reflux-esophagitis and its complications.

Technique of proximal gastric vagotomy with adrenergic preservation (PGV-AP).

The designation proximal gastric vagotomy with adrenergic preservation (PGV-AP) is proposed for the technique of PGV which preserves the blood vessels of the lesser curvature of the stomach, thus preserving the sympathetic innervation to that organ. The author presents a detailed and fully illustrated description of the technique of PGV-AP performed during the last 14 years on 335 patients. An ulcer recurrence rate of 1.8 per cent was noted. The excellent results are attributed to the preservation of the sympathetic innervation to the stomach. These clinical findings confirm recent experimenta…

[Results of studies in 165 patients following selective proximal vagotomy].

Selective proximal vagotomy was performed on 165 patients at the I. Surgical Clinic of Innsbruck University, between 1970 and 1984. Results of 90 patients were retrospectively evaluated. Lethality was 0.6 per cent, while the rate of recurrence amounted to 14.4 per cent and average acid reduction to 50 per cent. 65 per cent of the above 90 patients were almost without any postoperative complaint, when rated by the I. and II. degree according to Visick. Results were particularly favourable in cases of perforated ulcer, combined ulcer (Type II according to Johnson), hyperacid gastroduodenitis, an…

[Critical position on indications for selective proximal vagotomy without drainage].

In 128 patients with chronic duodenal ulcer the selective proximal vagotomy without drainage was carried out in a prospective study. Intraoperatively the test for completeness of the vagotomy was performed by means of the dye test in 50%. Postoperatively no lethal complications were to be seen, only three bronchopneumonias and one wound rupture. The late results after one year showed 77.5% Visick II, 10% Visick III and 12.5% Visick IV. After a successless conservative treatment of the 16 patients with recidivation in 10 patients the second operation had to be performed. In 6 of 10 patients pyl…
