Osteoarthritis pain masterclass: Moving beyond structure to mechanism-based clinical reasoning.

INTRODUCTION: Osteoarthritis (OA) is a leading cause of pain, disability, and reduced quality of life worldwide. Although OA is commonly interpreted through a structural lens, pain severity is often only weakly related to radiographic severity, suggesting that structure-based models are insufficient to explain many clinical presentations. PURPOSE: This masterclass aims to challenge persistent structural assumptions surrounding OA pain, summarizes the peripheral, central, behavioral, and systemic mechanisms that may shape pain presentation, and provide a mechanism-based clinical reasoning fram

Can we maximize both value and quality in gynecologic cancer care? A work in progress.

Value is defined as desirable health outcomes achieved per monetary unit spent. Comparative effectiveness research and cost-effectiveness research are methods that have been developed to quantify effectiveness and value to inform management decisions. In this article we review the comparative and cost-effectiveness literature in the field of ovarian cancer treatment. Studies have shown that improved ovarian cancer survival is associated with complete primary surgical cytoreduction, with treatment at high volume facilities by subspecialist providers (gynecologic oncologists) and with National C

Value analysis of digital breast tomosynthesis for breast cancer screening in a commercially-insured US population.

PURPOSE: The objective of this study was to conduct a value analysis of digital breast tomosynthesis (DBT) for breast cancer screening among women enrolled in US commercial health insurance plans to assess the potential budget impact associated with the clinical benefits of DBT. METHODS: An economic model was developed to estimate the system-wide financial impact of DBT as a breast cancer screening modality within a hypothetical US managed care plan with one million members. Two scenarios were considered for women in the health plan who undergo annual screening mammography, ie, full field dig

Detecting Patients with Chronic Kidney Disease Using General Practitioner Electronic Health Records and Electronic Phenotypes.

Chronic kidney disease (CKD) is a major public health issue. Estimating CKD prevalence using Electronic Health Records (EHR) of General Practitioners (GPs) is of value to guide patient care and healthcare system policies. Using clinical guidelines, we developed two different CKD electronic phenotypes (e-phenotypes) fit for EHR data from GPs: a laboratory-derived e-phenotype based on the estimated glomerular filtration rate and albumin-to-creatinine ratio, and a diagnosis code-derived e-phenotype. We assessed them based on the CKD prevalence retrieved. Each e-phenotype identified a different nu

The Disposition Decision: A Qualitative Study of Swedish Emergency Physicians' Objectives Through a Value-Focused Thinking Analysis.

OBJECTIVES: Disposition decisions in emergency departments involve multiple, often conflicting objectives. This study aimed to develop avalue assessment framework, expressed as a hierarchy of objectives, based on emergency physicians' values. METHODS: We used value-focused thinking to analyze the disposition decision through semi-structured interviews with thirteen emergency physicians in Sweden. A hierarchy of objectives was developed through content analysis and refined through iterative group interviews with the participants. RESULTS: Three fundamental objectives were identified: maximizi

Rethinking Secondary Cardiovascular Prevention in the Digital Era: Patient Empowerment, Adherence and Optimisation of Guideline-based Care.

Cardiovascular disease is the leading cause of morbidity and mortality globally. Despite effective treatments, poor adherence limits their long-term benefits. Digital health solutions can enhance clinicians' ability to optimise guideline-based therapies, improving patient outcomes. Digital tools for remote consultations, monitoring, cardiac device interrogation and clinical decision support systems are now widely available. Digital health monitoring improves care quality, providing value to patients, healthcare professionals, hospitals and governments. This transformation is fostering a future

Beyond viral suppression: Quality of life among stable ART clients in a differentiated service delivery intervention in Tanzania.

BACKGROUND: With antiretroviral therapy, more people living with HIV (PLHIV) in resource-limited settings are virally suppressed and living longer. WHO recommends differentiated service delivery (DSD) as an alternative, less resource-demanding way of expanding HIV services access. Monitoring client's health-related quality of life (HRQoL) is necessary to understand patients' perceptions of treatment and services but is understudied in sub-Saharan Africa. We assessed HRQoL among ART clients in Tanzania accessing two service models. METHODS: Cross-sectional survey from May-August 2019 among sta

Addressing gender bias in sexual and reproductive health care: A scoping review of training tools and interventions.

Gender norms and bias influence how health workers interpret and deliver sexual and reproductive health and rights (SRHR) services, contributing to stigma, discrimination, and inequities in care. This scoping review aimed to map training tools and educational interventions designed to strengthen health workers' competencies and address gender bias in SRHR care. This scoping review followed the Arksey and O'Malley framework and PRISMA-ScR guidelines. Searches were conducted across multiple databases and organizational websites for literature published between 2000 and November, 2025. Eligible r

Measuring the Value of a Clinical Practice Guideline for Children With Perforated Appendicitis.

OBJECTIVE: To determine the incremental cost-effectiveness of a clinical practice guideline (CPG) compared with "usual care" for treatment of perforated appendicitis in children. Secondary objective was to compare cost analyses using hospital accounting system data versus data in the Pediatric Health Information System (PHIS). BACKGROUND: Value-based surgical care (outcomes relative to costs) is frequently touted, but outcomes and costs are rarely measured together. METHODS: During an 18-month period, 122 children with perforated appendicitis at a tertiary referral children's hospital were t

Simple clinical criteria to identify sepsis or pneumonia in neonates in the community needing treatment or referral.

BACKGROUND: Sepsis, meningitis and pneumonia annually kill 1.1 million neonates in developing countries; most deaths occur at home. OBJECTIVES: To develop simple clinical criteria, enabling health workers in communities to identify neonates with potentially fatal sepsis; and to identify the danger signs alerting mothers to seek care. METHODS: In a field trial in 39 villages in Gadchiroli, India, trained health workers visited all neonates at home 8 times during the first 28 days of life, recording signs and outcome without interventions during 1995-1996 and with home-based management of sick

Integrating Community Linkages into Care Management Workflows to Address Contextual Drivers of Health: Lessons from a Pilot Intervention.

Adverse contextual drivers (eg, food, housing, transportation insecurity) influence health outcomes, particularly among individuals with chronic conditions. Electronic health record (EHR)-based care management tools can support screening and referrals for contextual drivers, but under-resourced health centers often lack support to integrate these tools into routine workflows. This pilot study assessed the feasibility of a multicomponent implementation support model to improve care managers' use of EHR tools to address contextual drivers. Three health centers within a national network using a s

Integrating Patient-Reported Outcomes into Atrial Fibrillation Care Pathways: Implementation Challenges, Health System Implications, and Future Directions.

Background/Objectives: Atrial fibrillation (AF) imposes a substantial long-term clinical and healthcare system burden, recurrent hospitalizations, impaired quality of life, and increasing long-term healthcare costs. Although patient-reported outcome measures (PROMs) are increasingly used in AF research and clinical practice, their broader role in healthcare delivery, implementation, and system-level decision-making remains insufficiently defined. Existing assessment strategies frequently prioritize symptom burden while underrepresenting cognitive, emotional, social, and functional dimensions o

Applications of Big Data in Perioperative Outcomes Research and Evidence-Based Clinical Practice.

Regional anesthesia (RA), including neuraxial anesthesia and peripheral nerve blocks, has shown benefits in improving perioperative outcomes. The increase of big data enables large-scale analysis of RA's impact on patient morbidity and mortality, supporting value-based health care models focused on quality and cost. Initiatives like the Perioperative Surgical Home and Enhanced Recovery After Surgery protocols highlight anesthesiologists' roles in optimizing care. Studies using clinical registries and administrative databases suggest that RA can lower postoperative mortality and complications i…

Is it time to introduce health coaching in inflammatory bowel disease management?

Inflammatory bowel disease is a chronic, relapsing-remitting condition with profound physical, psychological, and social consequences. Conventional therapies are essential for controlling intestinal inflammation but often fail to address behavioral and motivational dimensions critical for adherence, stress management, and quality of life. Health Coaching has emerged as a patient-centered intervention that fosters self-efficacy, resilience, and sustainable behavior change. This narrative review synthesizes current evidence on Health Coaching in chronic disease management and explores its potent

Reducing alcohol-associated liver disease burden in the general population.

The prevalence of alcohol use disorder (AUD) and alcohol-associated liver disease (ALD) is rising. The National Institute on Alcohol Abuse and Alcoholism organised a multistakeholder workshop focused on reducing the burden of ALD. Decreasing ALD morbidity and mortality requires a multipronged approach, including increased population-based screening for AUD, early recognition of ALD, and multidisciplinary treatment. Recommended screening tools for alcohol use include the alcohol use disorders identification test for consumption (AUDIT-C). In patients with elevated AUDIT-C scores (AUDIT-C score

Comparison of scenarios in the trajectory of body mass index among adolescents monitored in primary health care: the VigiNUTRI Brazil study.

We compared the BMI-for-age (BMI/A) trajectory of Brazilian adolescents monitored in the primary health care (PHC) setting based on a simulated scenario. We used a real-life cohort of adolescents monitored by the Food and Nutrition Surveillance System (Sisvan) between 2008 and 2018. The LMS method was employed to estimate the simulated BMI/A evolution curve, assuming that the adolescents maintained the conditions observed during their first assessment (simulation curve). This curve was compared with that of the real-world cohort (Sisvan curve). Both curves were compared to the World Health Org
