CASE: We herein report a rare case of a Japanese man diagnosed with cystic fibrosis (CF) following a workup for male infertility and recurrent pancreatitis and discuss the clinical and diagnostic implications in a population wherein CF is exceptionally rare. A 29-year-old man with primary infertility for 2 years presented for further evaluation. Repeated spermiograms showed azoospermia and an acidic pH. Sonographically, both testes showed normal size and parenchyma, but the vas deferens was bilaterally not palpable. Magnetic resonance imaging showed a cystically dilated left vas deferens and a dilated left seminal vesicle. The right vas deferens could not be visualized. A case of urogenital nonunion is presented to illustrate the importance of laparoscopy for the impalpable testis. A 4-year-old boy with an impalpable left testis underwent laparoscopy. This revealed not only the vas deferens entering the deep inguinal ring but also a small intraabdominal testis supplied by the testicular vessels.
