BACKGROUND: Vascular calcification (VC) is a common finding in chronic kidney disease (CKD) patients and predicts subsequent cardiovascular morbidity and mortality in this population. Vascular calcification is linked to disordered mineral metabolism and has been associated with bone histomorphometry changes in CKD. However, data on predialysis patients is scarce.
METHODS: A cross-sectional study was conducted on a cohort of 56 CKD patients not yet on dialysis, who underwent a transiliac bone biopsy for histomorphometric evaluation after double tetracycline labeling. Patients had no previous e…

Calcification is a prominent feature of late-stage atherosclerosis, but the mechanisms driving this process are unclear. Using a biobank of carotid endarterectomies, we recently showed that Proteoglycan 4 (PRG4) is a key molecular signature of calcified plaques, expressed in smooth muscle cell (SMC) rich regions. Here, we aimed to unravel the PRG4 role in vascular remodeling and intimal calcification. PRG4 expression in human carotid endarterectomies correlated with calcification assessed by preoperative computed tomographies. PRG4 localized to SMCs in early intimal thickening, while in advanc

Vascular calcification refers to the pathological deposition of calcium and phosphate minerals into the vasculature. It is prevalent in atherosclerosis, ageing, type 2 diabetes mellitus and chronic kidney disease, thus, increasing morbidity and mortality from these conditions. Vascular calcification shares similar mechanisms with bone mineralization, with smooth muscle cells playing a critical role in both processes. In the last decade, a variety of microRNAs have been identified as key regulators for the differentiation, phenotypic switch, proliferation, apoptosis, cytokine production and mat…

Aortic aneurysm is a vascular disease whereby the ECM (extracellular matrix) of a blood vessel degenerates, leading to dilation and eventually vessel wall rupture. Recently, it was shown that calcification of the vessel wall is involved in both the initiation and progression of aneurysms. Changes in aortic wall structure that lead to aneurysm formation and vascular calcification are actively mediated by vascular smooth muscle cells. Vascular smooth muscle cells in a healthy vessel wall are termed contractile as they maintain vascular tone and remain quiescent. However, in pathological conditio…

BACKGROUND AND AIMS: Vascular calcification is a common health problem related to oxidative stress, inflammation, and circulating calciprotein particles (CPP). Hydrogen sulfide is an endogenous signaling molecule with antioxidant properties and potential for drug development targeting redox signaling. Yet, its molecular mechanisms of action in vascular smooth muscle cell (VSMC) calcification have not been delineated. We therefore sought to identify key pathways involved in the calcification-inhibitory properties of sulfide employing our recently developed CPP-induced VSMC calcification model.

CONTEXT: The relationship between sex steroids and atherosclerosis is poorly understood.
OBJECTIVE: To describe the association of serum total T (TT), calculated free T (cFT), estrone (E1), estradiol (E2), and SHBG to vascular calcification in adult men.
DESIGN: Observational study (Framingham Heart Study). Analyses are cross-sectional. TT, E1, and E2 were measured by liquid chromatography-tandem mass spectrometry, and SHBG by immunofluorometric assay. Estimates of association were obtained by Tobit regression, which acknowledges the influence of floor effects on outcomes.

BACKGROUND: Our pilot data showed an increased intima media thickness in the patients with sporadic idiopathic hypoparathyroidism (SIH). Alteration in homeostasis of calcium, phosphate, and parathyroid hormone (PTH) may predispose to increase the risk of cardiovascular morbidity and mortality. The data on objective assessment of this increased risk is however lacking.
OBJECTIVE: To assess the effect of altered calcium, phosphate, and PTH homeostasis in the patients with SIH on coronary calcium score (a marker of increase vascular risk) by multidetector computed tomography scan (MDCT).

The development of chronic kidney disease (CKD) is associated with poor outcomes largely due an increased likelihood of adverse cardiovascular events. Many factors are playing a role in cardiovascular disease in CKD including the development of vascular calcification (VC). Studies have indeed shown that the presence of VC is associated with decreased survival among patients with CKD. The pathogenesis of VC is itself multi-factorial. LikeLy playing a significant role is the altered mineral metabolism of CKD. Management of the altered mineral metabolism in CKD is quite difficult and may also pla…

BACKGROUND: Vascular calcification and low bone turnover with a relatively low parathyroid hormone (PTH) often coexist in diabetic patients undergoing haemodialysis. Since calcium salts (CaS) are used extensively as primary phosphate binders and have been associated with progressive vascular calcification, we studied the effects of CaS on coronary arteries and parathyroid activity in incident haemodialysis diabetic patients.
METHODS: We measured the change in coronary artery calcium scores (CACS) with sequential electron beam computed tomography (EBCT) in 64 diabetic and 45 non-diabetic patie…

BACKGROUND AND AIMS: Spotty calcification in the coronary arteries is considered to represent plaque vulnerability, whereas more advanced calcification is thought to be a feature of advanced stable plaque. However, data supporting this notion is limited. Inflammation plays a key role in atherogenesis, including the formation of early-stage calcification. We aimed to correlate spotty calcification assessed by optical coherence tomography (OCT) with vascular inflammation assessed by percutaneous coronary adipose tissue (PCAT) attenuation on coronary computed tomography angiography (CCTA) and OCT

BACKGROUND: Sarcopenia is widely recognized that affects multiple organs, including the cardiovascular system. Clinical studies have consistently shown associations between sarcopenia and vascular aging phenotypes such as vascular calcification, atherosclerosis, and abdominal aortic aneurysms (AAA). However, the molecular mechanisms by which muscle deterioration contributes to vascular degeneration remain poorly understood.
METHODS: To elucidate muscle-vascular crosstalk, we established a murine model combining inflammation-associated muscle atrophy induced by hindlimb immobilization with CaP…

Chronic kidney disease (CKD) is a major global public health challenge, and cardiovascular complications such as vascular calcification (VC) are crucial in CKD-related mortality. Emerging evidence shows that open reading frames (ORFs) within non-coding genomic regions can encode micropeptides, but their roles in VC progression remain largely unexplored. In this study, we identify a 40-amino acid micropeptide, termed PKM2 AP, encoded by an ORF2-containing intron-retaining transcript derived from the LINC01139 locus. PKM2 AP localizes to both mitochondrial and cytoplasmic compartments and intera…

BACKGROUND: Vascular calcification is a hallmark of chronic kidney disease (CKD)-mineral and bone disorder and a major determinant of cardiovascular mortality. Secondary calciprotein particles (CPP2) are key mediators of vascular calcification; however, the mechanisms regulating their maturation and pathogenic properties remain incompletely understood. CPT (Calprotectin, S100A8/A9), a myeloid-derived inflammatory mediator, is elevated in CKD and predicts cardiovascular events and mortality, particularly under high-phosphate conditions, but the underlying mechanisms remain unclear.
METHODS: We…

Small-diameter vascular grafts are inevitably subjected to oxidative stress and inflammatory responses, which delay graft endothelialization and ultimately result in intimal hyperplasia and vascular calcification. In this study, copper sulfide (CuS) nanozymes with catalase(CAT) and glutathione peroxidase(GPx) like activities were developed, which could simultaneously generate nitric oxide (NO) and hydrogen sulfide (H2S). CuS nanozymes were blended with polymer and electrospun to prepare grafts. The grafts could reduce reactive oxygen species levels, inhibit inflammatory responses, enhance endo…

BACKGROUND: Vascular calcification (VC) affects up to 90% of patients with end-stage renal disease and increases cardiovascular mortality 3- to 5-fold. Once considered passive mineral deposition, VC is now recognized as an active, toxin-driven process orchestrating vascular smooth muscle cell transdifferentiation, endothelial dysfunction, and matrix remodeling. However, current uremic toxin classifications remain biochemically oriented, providing limited clinical guidance for risk stratification and therapeutic selection.
METHODS: This comprehensive review reframes uremic toxin-driven VC thro…
