The aim of this retrospective study was to assess the success rates of limb salvage, and the primary and secondary patency rates of reconstructions of critical limb ischemia (CLI) patients undergoing combined infrainguinal reconstruction and intraluminal angioplasty of crural arteries. In 2000-2005, infrainguinal reconstruction with concomitant intraluminal angioplasty of crural arteries was performed in 30 patients with CLI, mean age was 63 years (S.D.=10); according to the Fontaine classification, 28 (93%) patients had stage IV and 2 (7%) stage III ischemia. During mean follow-up of 12.9 (S.

Decellularized artificial blood vessels prepared using physical and chemical methods often exhibit limitations, including poor mechanical performance, susceptibility to inflammation and calcification, and reduced patency. Cross-linking techniques can enhance the stiffness, as well as anti-inflammatory and anti-calcification properties of decellularized vessels. However, conventional cross-linking methods fail to effectively alleviate residual stress post-decellularization, which significantly impacts the patency and vascular remodeling following the implantation of artificial vessels. BACKGROUND/AIM: The aim of this paper was to present our 3-year multicenter experience in creating a vascular access using the basilic vein. The third choice in creating vascular access is the brachiobasilic arteriovenous fistula (AVFs) with transposition of the basilic vein.
MATERIALS AND METHODS: During 2010-2012, out of a total of 874 AVFs that were performed in our two centers, 54 were brachiobasilic fistulas (6.71%), performed on 54 patients. All surgical procedures were performed by two surgical teams, one for each center. There were no significant differences regarding the patients' ag

OBJECTIVES: Autologous saphenous vein (ASV) for arterial reconstruction, in vascular limb injuries is the graft material of choice. Denatured saphenous vein homograft (DSVH), thanks to its characteristics of readily available autologous biological prosthesis, has been proposed as alternative. We report our prospective experience with DSVH employed for arterial reconstruction in civilian limb vascular injuries.
MATERIALS: From January 1994 to June 1996, DSVH was implanted in 16 male patients (pts.) treated for arterial civilian injuries of eight upper limbs and eight lower limbs.
METHODS: In

One hundred and one surgical procedures performed in children for construction and maintenance of vascular accesses for haemodialysis were retrospectively analyzed. There were 86 operations performed to create a new fistula in patients without vascular access or with nonrecoverable failed angio-access. Fifteen surgical procedures were performed to treat fistula complications. The new fistulas were radiocephalic n = 60 (70%), ulnar-basilic n = 5 (5.8%), antecubital n = 9 (10.3%), and PTFE grafts n = 12 (14%). Microsurgical techniques were used in all cases, including PTFE graft fistulas. A micr

AIM: Survival of patients with end-stage renal disease naturally depends on the preservation of functional vascular access. This study aims to compare 1-year patency rates, procedural success, and reintervention requirements of surgical revision with endovascular interventions in the treatment of arteriovenous fistula dysfunction.
MATERIAL AND METHODS: Data of 1008 patients who underwent intervention due to arteriovenous fistula dysfunction between 2015 and 2022 were analyzed retrospectively. Treatment allocation to the surgical group (n = 896) or endovascular group (n = 112) was strictly dic

INTRODUCTION: Vascular access-related aneurysms (VARA) are a complication of arteriovenous fistulas. Repair techniques have been described in the literature with varied outcomes.
MATERIALS AND METHODS: We conducted a prospective cohort study on patients who had VARA repair over 41 months. The indication for repair was an aneurysmal arteriovenous fistula (AVF) at risk of haemorrhage or difficulty in cannulation. Pseudoaneurysms, infected AVF and bleeding VARA were excluded. All patients underwent outflow stenosis treatment when present, followed by aneurysmorrhaphy. They were monitored periodi

BACKGROUND: Radiocephalic arteriovenous fistula (RCAVF) is the best access modality to be considered initially when planning arteriovenous fistula (AVF) for maintenance hemodialysis. Considering the higher incidence of RCAVF failed maturation (M), it is important to perform proper preoperative evaluation and identification of high-risk patients. There has been no study on the influence of preoperative cardiac function on the M and patency of AVFs. The purpose of this investigation is to determine whether preoperative cardiac index (CI) is a predictor of M and primary patency of RCAVF.

BACKGROUND: Due to the long waiting time for kidney transplantation, most End-Stage renal disease patients are commenced on either hemodialysis or peritoneal dialysis. Reusable fistulas have the lowest risk for death, cardiovascular events, and infections among all vascular accesses. This study aims to report the outcomes of the arteriovenous fistulas and PTFE grafts and the related predictive clinical and demographic variables.
METHODS: This retrospective study reviewed the charts of all hemodialysis patients between January 2017 and January 2021 at the Dialysis Center of An-Najah National U

BACKGROUND: Long-term hemodialysis (HD) treatment requires the establishment of a cannulatable vascular access (VA) point. While the arteriovenous fistula (AVF) is considered the gold standard, the arteriovenous graft (AVG) is a viable alternative especially in patients with poor superficial venous anatomy. Few studies have assessed the efficacy of the brachial-brachial arteriovenous graft (BB-AVG) for long-term HD access. By analyzing one surgeon's experience in creating, surveilling and maintaining BB-AVGs, this retrospective study aims to add to the body of literature in assessing patency o

OBJECTIVE: Haemodialysis is an important tool for end-stage renal disease (ESRD) patients to correct electrolyte disturbance and improve life quality. This requires a method of long-term vascular access. For many ESRD patients, arteriovenous fistula (AVF) formation is the optimal method of access. AVF success and patency are critical to allow access for haemodialysis. Failure to mature and thrombosis are key factors involved in AVF failure. Our aim is to report outcomes from a single-centre, retrospective study investigating the association between artery and vein measurements as well as intra

INTRODUCTION: As nearly half of patients with end-stage kidney disease (ESKD) who initiate hemodialysis (HD) are over 65 years old (commonly defined as elderly), the fistula first strategy is controversial even in HD patients ≥65 years.
METHODS: In Korea's National Health Insurance Service database from 2008 to 2019, 41,989 elderly (≥ 65 years) HD patients were retrospectively reviewed to identify their clinical characteristics and outcomes. Vascular access (VA) patencies, risk factors associated with patencies and patient survival between arteriovenous fistula (AVF) and arteriovenous graft (

Fresh saphenous vein homografts are gaining popularity as conduits for femoral-popliteal or distal bypass grafts. Aside from major blood group compatibility, there is little clinical evidence that rejection is a significant factor in long-term patency. Some have suggested that blood vessels are weakly antigenic and others indicate that rejection does not endanger long-term patency. Transplantation and toxic damage were produced in experimental animals in order to compare healing processes. Both types of injuries produced early significant endothelial cell proliferation detected by H3-thymidine

BACKGROUND: There are only a few options to replace infected infrainguinal synthetic vascular grafts in the absence of suitable autologous veins. To use a biosynthetic vascular graft (Omniflow®II) might be a valuable alternative.
METHODS: We retrospectively analyzed the clinical course of 29 patients who underwent replacement of an infected infrainguinal vascular prosthesis (Szilagyi 3) by an Omniflow®II graft.
RESULTS: Because of the lack of suitable autologous veins, 15 above-knee femoro-popliteal, 5 below-knee femoro-popliteal, and 9 femoro-tibial bypasses were replaced with biosynthetic

PURPOSE: To describe a novel bailout technique to approach below-the-ankle (BTA) chronic total occlusions or plantar-arch severe disease where the balloon/catheter is unable to follow the crossing guidewire and no other described recanalization approach is feasible.
TECHNIQUE: When facing a complex BTA revascularization, if the guidewire crosses but the balloon cannot progress due to a lack of pushability, an antegrade puncture of the infrapopliteal vessel where the tip of the guidewire lays is performed. The guidewire is then carefully navigated through this distal BTA vessel into the needle

When performed properly, a transposition is the most efficient and most durable procedure for reconstructing the subclavian artery for proximal occlusive disease or to extend the landing zone in the aortic arch prior to endovascular therapy for thoracic aortic pathology. The proximal subclavian artery, on the right or on the left, is approached quite differently than it is for a bypass procedure. The technique is described in detail here. There are very few contraindications to transposition, the advantages over the alternate options are many, and it can be the approach of choice in the vast m

Acute deep vein thrombosis (DVT) is associated with substantial long-term morbidity, primarily through the development of postthrombotic syndrome (PTS). While endovascular strategies aim to clear thrombus and preserve venous patency, randomized evidence has not demonstrated consistent reductions in overall PTS across a broad DVT patient population. New clinical and basic research into individual variability in vein wall inflammation and fibrosis, ability to mount endogenous fibrinolysis, response to antithrombotic therapy, and impact of timing of treatment initiation continue to refine our und

BACKGROUND: There has been a dramatic increase in both the availability of hemodialysis and long-term survival of patients with chronic renal failure. Patients who require long-term hemodialysis need long-term vascular access. The transposed autologous basilic vein to brachial artery arteriovenous fistula provides vascular access in the absence of adequate superficial vein.
PATIENTS AND METHODS: We follow an aggressive "all-autogenous" policy with regard to dialysis access and recommend prosthetic graft when autogenous options are exhausted. A retrospective analysis was performed of consecuti

Directional atherectomy (DA) was developed as a new therapeutic modality for coronary artery disease. For the past 3 years, we have applied DA intervention to vascular access failure with either poor blood flow rate or high venous pressure. DA intervention was performed on 27 lesions of 16 hemodialyzed patients. A reduction of stenosis to less than 20% of that before treatment was judged a technical success, and the rate of technical success was 84%. All patients showed adequate blood flow rates after DA intervention, indicating initial success. Although restenotic events occurred frequently,
