Patients undergoing challenging medical procedures often suffer from high levels of anxiety and stress. Their most important need is a feeling of control and safety. Hypnotic communication provides a means of addressing these feelings without medication. The core technique is to use the perceived stimuli, like beeping sounds or the pressure of the ventilation mask and re-evaluate them as helpful signs. For example, the beeping sounds can be an indicator of the optimal treatment with the highest level of medical equipment in the intensive care unit and the pressure of the ventilation mask might

Responses of Bilevel Ventilators to Unintentional Leak: A Bench Study.

BACKGROUND: The impact of leaks has mainly been assessed in bench models using continuous leak patterns which did not reflect real-life leakage. We aimed to assess the impact of the pattern and intensity of unintentional leakage (UL) using several respiratory models. METHODS: An active artificial lung (ASL 5000) was connected to three bilevel-ventilators set in pressure mode; the experiments were carried out with three lung mechanics (COPD, OHS and NMD) with and without upper airway obstruction. Triggering delay, work of breathing, pressure rise time, inspiratory pressure, tidal volume, cycli

Computerized energy balance and complications in critically ill patients: an observational study.

BACKGROUND AND AIMS: An accurate energy balance is difficult to achieve in hospitalized patients. The aim of the study was to measure the daily cumulative energy balance in critically ill patients receiving mechanical ventilation using a bedside computerized information system (CIS), and to assess its impact on outcome. METHODS: Fifty intensive care unit (ICU) patients (33 male, 17 female, mean age 59 +/- 18 years) were prospectively followed. Mean body mass index was 26.85 +/- 5.23 kg/m2 and mean APACHE II score, 23.1 +/- 7.7. Resting energy expenditure was measured daily with indirect calor

Pre-emptive active drainage of reflux (PARD) in Ivor-Lewis oesophagectomy with negative pressure and simultaneous enteral nutrition using a double-lumen open-pore film drain (dOFD).

BACKGROUND: Postoperative reflux can compromise anastomotic healing after Ivor-Lewis oesophagectomy (ILE). We report on Pre-emptive Active Reflux Drainage (PARD) using a new double-lumen open-pore film drain (dOFD) with negative pressure to protect the anastomosis. METHODS: To prepare a dOFD, the gastric channel of a triluminal tube (Freka®Trelumina, Fresenius) is coated with a double-layered open-pore film (Suprasorb®CNP drainage film, Lohmann & Rauscher) over 25 cm. The ventilation channel is blocked. The filmcoated segment is placed in the stomach and the intestinal feeding tube in the duo

[Negative Pressure Wound Therapy as a Treatment Option for Extensive Post-Trauma Subcutaneous Emphysema].

Subcutaneous emphysema can be a secondary complication of chest trauma or one of the complications of ruptured bullae in advanced chronic obstructive pulmonary disease. Massive subcutaneous emphysema impairs the respiratory mechanics and affects the venous returns of the head and neck. It can lead to respiratory insufficiency with the need for mechanical ventilation. The treatment should focus on the primary pathology. Nonetheless, in patients with subcutaneous emphysema as the only but serious symptom, the treatment can zero in solely on this complication. The standard procedure consists in t

Positive end-expiratory pressure in the pediatric intensive care unit.

Application of positive end-expiratory pressure (PEEP) targeted towards improving oxygenation is one of the components of the ventilatory management of pediatric acute respiratory distress syndrome (PARDS). Low end-expiratory airway pressures cause repetitive opening and closure of unstable alveoli, leading to surfactant dysfunction and parenchymal shear injury. Consequently, there is less lung volume available for tidal ventilation when there are atelectatic lung regions. This will increase lung strain in aerated lung areas to which the tidal volume is preferentially distributed. Pediatric cr

Continuous positive airway pressure versus continuous negative pressure around the chest for patients with acute exacerbation of chronic obstructive pulmonary disease in the intensive care unit: a pilot study.

INTRODUCTION: With chronic obstructive pulmonary disease (COPD) exacerbations, continuous positive airway pressure (CPAP) has been used to overcome the threshold load provided by intrinsic positive end expiratory pressure and decrease the inspiratory work of breathing. In this pilot study, we observed whether a continuous negative pressure (CNP) around the thorax and upper abdomen with a shell and wrap would provide a similar level of relief in dyspnoea. METHODS: In eight patients with COPD in the intensive care unit receiving CPAP, CNP was alternated twice with CPAP (30 minutes each time). W

Recent advances in intubation and mechanical ventilation.

The ideal care of the critically ill infant or child should demonstrate a seamless service starting with recognition of potential critical illness and initiation of early resuscitative interventions, escalating to advanced life support and skilled transfer to an appropriate location where intensive care can be continued. This article discusses advances in the package of respiratory care commencing with the decision to provide respiratory support, which includes the decision whether or not to intubate, leading to the choice of mechanical ventilatory support mode in the critically ill child. Cranial Electrode Belt Position Improves Diagnostic Possibilities of Electrical Impedance Tomography during Laparoscopic Surgery with Capnoperitoneum.

Laparoscopic surgery with capnoperitoneum brings many advantages to patients, but also emphasizes the negative impact of anesthesia and mechanical ventilation on the lungs. Even though many studies use electrical impedance tomography (EIT) for lung monitoring during these surgeries, it is not clear what the best position of the electrode belt on the patient's thorax is, considering the cranial shift of the diaphragm. We monitored 16 patients undergoing a laparoscopic surgery with capnoperitoneum using EIT with two independent electrode belts at different tomographic levels: in the standard pos

Successful management of drug-induced hypercapnic acidosis with naloxone and noninvasive positive pressure ventilation.

A 74-year-old man was referred to our hospital due to deteriorating level of consciousness and desaturation. His Glasgow Coma Scale was 6, and his pupils were constricted but responded to light. Chest radiograph was negative for significant findings. Arterial blood gas evaluation on supplemental oxygen revealed severe acute on chronic respiratory acidosis: pH 7.15; PCO2, 133 mm Hg; PO2,64 mm Hg; and HCO3, 31 mmol/L. He regained full consciousness (Glasgow Coma Scale, 15) after receiving a 0.4 mg dose of naloxone, but because of persistent severe respiratory acidosis (pH 7.21; PCO2, 105 mm Hg),

End-tidal CO2 changes as predictors of significant blood pressure variations during general anesthesia.

PURPOSE: This study aimed to establish thresholds for end-tidal CO2 (EtCO2) changes that indicate hemodynamic alterations occurring between intermittent blood pressure measurements. Given that EtCO2 reflects cardiac output under conditions of stable systemic vascular resistance, it may serve as a continuous hemodynamic surrogate when noninvasive blood pressure monitoring is intermittent. METHODS: This prospective observational study included participants (ASA I-IV) undergoing elective surgery with general anesthesia and controlled ventilation. EtCO2 and blood pressure were recorded every 3 mi

Characterization of feeding skills and clinical markers of preterm newborns with gastroschisis in a neonatal therapy unit.

OBJECTIVE: To describe the clinical and feeding findings of premature infants with gastroschisis (GTQ) in a neonatal intensive care unit and compare them to preterm infants (NBs) without congenital anomalies. METHODS: A retrospective case-control study was conducted with 50 premature NBs (25 with GTQ and 25 without comorbidities - control group). The NBs were compared regarding demographic and clinical data: risk of mortality and speech-language assessment (nonnutritive and nutritive sucking). Subsequently, a multiple logistic regression model was applied to determine the variables associated

[Postoperative laryngospasm may induce pulmonary edema. An unusual or overlooked complication?].

Young and healthy patients in whom laryngospasm secondary to postoperative extubation is encountered run a risk of developing pulmonary edema. The mechanism behind the edema is thought to be a strongly negative intrathoracic pressure generated by the patient's forced inspiration against a closed glottis. A net flow of fluid occurs to the interstitial space and further to the alveoli. Unusually the pulmonary edema can be noticed within a few minutes after relief of the obstruction, but occasionally it is delayed for several hours. The condition is potentially life-threatening, but usually respo

Pulmonary oedema associated with airway obstruction.

The purpose of this review is to describe the pathogenesis of pulmonary oedema associated with upper airway obstruction, summarize what is known of its clinical presentation, and reflect upon its implications for the clinical management of airway obstruction. The pathogenesis of pulmonary oedema associated with upper airway obstruction is multifactorial. However, as the phrase "negative pressure pulmonary oedema" suggests, markedly negative intrapleural pressure is the dominant pathophysiological mechanism involved in the genesis of pulmonary oedema associated with upper airway obstruction. Sciatic Nerve Neuropathy Secondary to Sacral Pressure Ulcer Following COVID-19: A Case Report.

We report the case of a previously healthy 25-year-old man who developed severe SARS-CoV-2 infection requiring prolonged mechanical ventilation. Following extubation, he presented with peripheral sensory deficits and a sacral pressure ulcer. He was later admitted to our service with fever and a Grade IV ulcer, which required surgical debridement, negative pressure wound therapy, and definitive reconstruction using a V-Y gluteal flap. Examination of the right leg at admission demonstrated a neuropathic pain pattern, with neurological evaluation and imaging confirming distal sensorimotor polyneu

The interaction between respiratory and autonomic function during sleep-related changes in pharyngeal airway patency.

Sleep-related changes in pharyngeal function result in an increased resistance to airflow and in some people complete pharyngeal occlusion. Clinically, pharyngeal occlusion causes obstructive sleep apnoea syndrome (OSA). This is a prevalent disorder, which is an independent risk factor for the development of systemic hypertension. Several mechanisms contribute to the sleep-related changes in pharyngeal function in both health and disease, including a reduction in respiratory-related muscle activation, and an increase in latency of the pharyngeal reflex to negative intralumenal pressure. Hemodynamic changes in preterm neonates with respiratory distress syndrome: high-flow nasal cannula versus nasal continuous positive airway pressure-a randomized controlled trial.

BACKGROUND: Non-invasive ventilation (NIV) in the form of heated humidified, high-flow nasal cannula (HHHFNC) or nasal continuous positive airway pressure (nCPAP) is widely used in preterm neonates with respiratory distress syndrome (RDS). We aimed to evaluate the impact of HHHFNC and nCPAP on cardiac, cerebral, and mesenteric hemodynamics. METHODS: One hundred twenty-three hemodynamically stable, low birth weight preterm neonates <34 weeks with RDS who needed NIV were randomized to receive either HHHFNC or NCPAP. Intraoperative prophylactic and therapeutic non-invasive ventilation: a systematic review.

Non-invasive ventilation (NIV) has been used to prevent or to treat perioperative acute respiratory failure (ARF). Intraoperative prophylactic and therapeutic use of NIV could be of interest to patients with anticipated difficulty in postoperative weaning from mechanical ventilation or to patients refusing tracheal intubation. Intraoperative NIV might also be useful when deep sedation is required, as this can cause respiratory depression. PubMed, Embase, Google Scholar, and Cochrane Library were searched for pertinent studies. Inclusion criteria were NIV use during surgery and adult patients;

Effects of nasal positive-pressure hyperventilation on the glottis in normal sleeping subjects.

We have previously observed that, in normal awake subjects passively hyperventilated with intermittent positive-pressure ventilation delivered through nasal access (nIPPV), the glottis could interfere with the ventilation. We report on data obtained in the same subjects during stable sleep. In all cases, the glottis was continuously observed through a fiber-optic bronchoscope, and other indexes were also continuously recorded. Mechanical ventilation was progressively increased up to 30 l/min. We have observed during passive nIPPV in stable sleep that increases in delivered minute ventilation (
