A 54-year-old woman had been resuscitated after ventricular fibrillation and her electrocardiogram showed a QT prolongation (QTc=510 ms), and genetic screening revealed a missense variant, R1644C, in the SCN5A gene. A 48-year-old man was admitted after an episode of aborted sudden death with external defibrillation. An implantable cardioverter defibrillator implanted 2 years before for secondary prevention failed to sense properly an episode of ventricular fibrillation. Interrogation of the device showed large oscillatory changes of the amplitude of the local electrogram during ventricular fibrillation, causing undersensing and inappropriate refraining from shock therapy.

We cared for a victim of the Wenchuan earthquake who was rescued after being buried in rubble for 52 hours. On admission to the hospital, the patient had severe hyperkalemic acidosis and necrosis of both legs, requiring bilateral amputation. In the operating room, the patient developed ventricular fibrillation. The patient was resuscitated and recovered without significant systemic injury. We review the diagnosis and treatment of crush syndrome, which occurs in many victims of natural disasters.

BACKGROUND: Derived variables of ventricular fibrillation, such as the frequency distribution by fast Fourier transformation and its evolution over time, have been used to determine the optimum timing for defibrillation. The description of the case of 35 years old patient with paroxysmal ventricular fibrillation in a course of Prinzmetal angina pectoris treated unsuccessfully with antiarrhythmic drugs who required implantation of cardioverter/defibrillator. The via-venous cardioverter/defibrillator was implanted (ICD). This study investigated whether the irregularity of ventricular cycle length during atrial fibrillation (AF) is affected by digoxin. Patients (n = 41) with paroxysmal AF enrolled in a randomized crossover comparison of digoxin and placebo underwent 24-hour ambulatory monitoring during each treatment.
