Tricuspid valve straddling: An uncommon cause of left ventricular outflow tract obstruction in transposition of great artery with ventricular septal defect.

Transposition of great arteries (TGA) can be associated with left ventricle outflow tract (LVOT) obstruction. In the presence of ventricular septal defect (VSD), septal leaflet of tricuspid valve may prolapse through perimembranous VSD or rarely tricuspid valve tissue may override to produce LVOT obstruction. Occasionally, this may be mistaken for vegetation due to associated pulmonary valve endocarditis. We report a case of d-TGA with presumptive pulmonary valve endocarditis and LVOT obstruction that was found to be due to tricuspid valve straddling on transesophageal echocardiography, result

Usefulness of preoperative echocardiography in predicting left ventricular outflow obstruction after primary repair of interrupted aortic arch with ventricular septal defect.

Residual left ventricular outflow tract (LVOT) obstruction is a significant problem after repair of interrupted aortic arch (IAA) and ventricular septal defect. Resection of subaortic tissue at the time of primary repair, however, is associated with increased morbidity and mortality. We reviewed the preoperative echocardiograms and the postoperative clinical course and echocardiograms of 23 consecutive patients who underwent primary repair of IAA without widening of the subaortic region. Nine patients (39%) developed significant LVOT obstruction (pressure gradient >40 mm Hg). LVOT obstruction

Left ventricular outflow tract obstruction in complete transposition of the great arteries with intact ventricular septum. A cross sectional echocardiography study.

The roles of posterior bulging of the interventricular septum (septal bulge) and of systolic septal mitral apposition in patients with simple transposition of the great arteries are not known. Cross sectional echocardiograms of 40 such patients were reviewed (after exclusion of those with fixed left ventricular outflow tract obstruction) and haemodynamic findings were compared with long and short axis measurements within the left ventricle. There was no significant correlation between the degree of septal bulge and systolic gradient across the left ventricular outflow tract, but septal bulge c

Unruptured congenital sinus of Valsalva aneurysm causing right ventricular outflow tract obstruction associated with hidden ventricular septal defect: a case report.

BACKGROUND: Sinus of Valsalva aneurysms are rare congenital lesions. Unruptured aneurysms are often silent, but when they are large, they can cause symptoms by compressing nearby structures. This case is notable because an unruptured aneurysm from the right sinus produced severe right ventricular outflow tract obstruction and concealed a ventricular septal defect that was not detected on preoperative imaging. CASE PRESENTATION: A 45-year-old man with a remote history of congenital heart disease and no follow-up presented with three weeks of progressive shortness of breath, which was consisten

Effect of right ventricular outflow tract obstruction on right ventricular volumes and exercise capacity in patients with repaired tetralogy of fallot.

Patients with tetralogy of Fallot and combined right ventricular outflow tract obstruction (RVOTO) and pulmonary regurgitation (PR) have a less dilated right ventricular (RV) and better RV function compared with patients without RVOTO. It is not known whether RVOTO is associated with improved exercise capacity. We compared cardiac magnetic resonance imaging, echocardiography, and exercise tests in 12 patients with RVOTO (Doppler peak RVOT gradient ≥30 mm Hg) and 30 patients without RVOTO. RV end-systolic and end-diastolic volumes were smaller in patients with RVOTO compared with patients witho

Negative ionotropic agents for the treatment of left ventricular outflow tract obstruction due to sigmoid septum and concentric left ventricular hypertrophy.

BACKGROUND: Negative ionotropic agents are established treatments for left ventricular outflow tract obstruction (LVOTO) in hypertrophic cardiomyopathy (HCM). However, their benefit in those with LVOTO without typical HCM is unclear. We evaluated if negative ionotropic agents are beneficial in treatment of LVOTO in patients without typical HCM. METHOD: In this retrospective cohort study, we evaluated echocardiograms and reviewed records of 15 patients with resting LVOT obstruction due to sigmoid septum (SS, n=9) and mild concentric left ventricular hypertrophy (LVH, n=6). Patients received tr

Complete transposition of the great arteries with intact ventricular septum and left ventricular outflow tract obstruction. Surgical management and anatomic considerations.

Surgical management of patients with complete transposition and intact ventricular septum may become difficult in the presence of left ventricular outflow tract obstruction. A Mustard operation and direct resection of the obstruction through the pulmonary artery has been the treatment of choice for this combination. Our study of the structure of the left ventricular outflow tract in four specimens with the anatomic findings of complete transposition, intact ventricular septum, and subpulmonary stenosis suggests that direct resection of the stenosis through the pulmonary artery can seldom be ad

Left ventricular outflow tract obstruction in pregnancy: changes in echocardiogram gradients and association with maternal-fetal outcomes.

BACKGROUND: Women with left ventricular outflow tract obstruction (LVOTO) are at risk of adverse pregnancy outcomes, which have been related to the severity of the LVOTO. However, the physiological adaptations of pregnancy increase the gradient across an LVOTO, often altering the clinical perception of risk. Here, we examined how the LVOTO gradient changes during pregnancy, investigating whether a greater increase in the gradient is associated with worse pregnancy outcome and whether any increase in the gradient is transient, reversing after the end of the pregnancy. METHODS: The study was co

Cardiovascular magnetic resonance-derived metrics as diagnostic markers for left ventricular outflow tract obstruction in hypertrophic cardiomyopathy.

BACKGROUND: This study aims to assess the utility of cardiovascular magnetic resonance (CMR)-derived measurements as diagnostic indicators for left ventricular outflow tract obstruction (LVOTO) in hypertrophic cardiomyopathy (HCM) patients, using Doppler echocardiography as the reference method. METHODS: In this single-center retrospective cross-sectional study, adult HCM patients with transthoracic echocardiography and CMR within a 6-month time window were enrolled. Doppler LVOT gradient was measured both at rest and under provocative maneuvers. LVOTO was defined as a gradient ≥30 mmHg. The

Transposition of great arteries with left ventricular outflow tract obstruction undergoing definitive repair beyond the third decade.

A 30-year-old male presented with progressive dyspnea, cyanosis, and chest pain due to unrepaired d-transposition of the great arteries with ventricular septal defect (VSD) and severe left ventricular outflow tract obstruction, underwent successful late biventricular repair including VSD closure, neo-aortic mechanical valve implantation, coronary transfer, and LeCompte maneuver. Despite postoperative stroke and complete heart block requiring pacemaker implantation, he was discharged in stable condition with preserved biventricular function. This case supports the feasibility of anatomic repair

Incidence of Reoperation After Surgical Procedure for Left Ventricular Outflow Tract Obstruction in Children and Young Adults.

BACKGROUND: The common causes of subaortic left ventricular outflow tract obstruction (LVOTO) are hypertrophic cardiomyopathy (HCM) and membranous/tunnel subaortic stenosis (SAS). Reoperation after corrective surgery may be due to recurrent disease, associated congenital defects, or complications of the initial procedure. This study compares the late outcomes of young patients with HCM and SAS. METHODS: We studied clinical, echocardiographic, and operative data of patients ≤21 years of age at the time of surgery for LVOTO between August 1963 and August 2018. We stratified patients into HCM (n

Left Ventricular Outflow Tract Gradient Is Associated With Coronary Artery Obstruction in Children With Williams-Beuren Syndrome.

OBJECTIVES: Patients with Williams-Beuren syndrome are associated with a high risk of hemodynamic collapse during sedation and/or anesthesia, presumably due to occult coronary obstruction. The objective of this study was to determine the association between transthoracic echocardiogram findings and the presence of coronary obstruction to examine if coronary obstruction can be predicted by transthoracic echocardiogram before anesthesia. DESIGN: Retrospective data analysis of patients with Williams-Beuren syndrome who underwent transthoracic echocardiogram, cardiac catheterization, and/or surgi

Dynamic thromboembolic left ventricular outflow tract obstruction after aggressive procoagulant treatment in hemorrhagic shock: a case report.

BACKGROUND: In cases of hypertrophic obstructive cardiomyopathy (HOCM), the systolic anterior motion of the mitral valve apparatus results in an obstruction of the left ventricular outflow tract (LVOT), which is known as the SAM [systolic anterior motion] phenomenon. Hypothetically, a pathological obstruction of the LVOT of a different etiology would result in a comparable hemodynamic instability, which would be refractory to inotrope therapy, and may be detectable through echocardiography. CASE PRESENTATION: We observed a severely impaired left ventricular function due to a combination of a

Outcomes following the surgical management of left ventricular outflow tract obstruction; A systematic review and meta-analysis.

BACKGROUND: Left ventricular outflow tract obstruction (LVOTO) causes exertional symptoms in two thirds of patients with hypertrophic cardiomyopathy (HCM). Consensus guidelines recommend surgical intervention in patients with drug refractory symptoms. The primary aim of this study was to perform a systematic review and meta-analysis to determine morbidity and mortality after surgery. METHODS: Study Selection: Studies reporting outcomes following surgical intervention for symptomatic LVOTO in HCM. Rapidly progressing giant aneurysm of right ventricular outflow tract with severe conduit obstruction: Report of two cases.

Pseudoaneurysm of the right ventricular outflow tract (RVOT) has been reported as a rare complication of RVOT reconstruction performed using conduit replacement or patch repair. The RVOT needs to be followed up on echocardiography and if aneurysm a large aneurysm forms, it needs to be operated. Usually progression of aneurysm is slow. Rarely, it may present with symptoms secondary to the compression of adjoining mediastinal structures or thrombus formation inside the aneurysm. We report two cases of rapidly progressing massive RVOT aneurysm needing early surgical intervention. Both patients ha

Transient left ventricular outflow tract obstruction with systolic anterior motion of the mitral valve: A stunning cause.

Left ventricular outflow tract obstruction (LVOTO) and systolic anterior motion (SAM) of the mitral valve may have various etiologies, of which hypertrophic cardiomyopathy is the most common. More rarely, an acute coronary syndrome, myocardial stunning, and takotsubo cardiomyopathy may give rise to LVOTO and SAM. Here, we present a 70-year-old female patient with a non-ST-elevation acute coronary syndrome treated with percutaneous coronary intervention. Echocardiography the day after, because of dyspnea and hypotension, revealed apical akinesia, LVOTO, and SAM, which proved completely reversib

Dynamic left ventricular outflow tract obstruction secondary to hypovolemia in a German Shepard dog with splenic hemangiosarcoma.

Dynamic left ventricular outflow tract obstruction (DLVOTO) is a common condition in cats and humans. In this case report, a dog is described with DLVOTO secondary to severe intra-abdominal hemorrhage caused by a hemangiosarcoma. The dog was a 9-year-old, 35.7-kg, spayed female German Shepard dog that presented with a history of tachypnea and collapse. A Levine II/VI systolic murmur was present at the heart base. Abdominal ultrasonography revealed a splenic mass and a large amount of ascites. Echocardiography showed a reduced left ventricular diameter and an increased aortic velocity caused by
