[Studies on the largest Lyapunov exponents of the standing posture in patients with unilateral vestibular dysfunction].

UNLABELLED: In the present study, we investigated the body sway in patients with unilateral vestibular dysfunction by the largest Lyapunov exponents using a chaotic time series analysis. The largest Lyapunov exponent is regarded as a parameter indexing an orbital instability. MATERIALS AND METHODS: Subjects consisted of 55 normal healthy subjects, 11 patients diagnosed as having vestibular neuritis (VN), 6 patients diagnosed as having sudden deafness (SD) with vertigo, 23 patients diagnosed as having Meniere disease (MD), 11 patients diagnosed as having benign paroxysmal positional vertigo (B

Acrophobia and pathological height vertigo: indications for vestibular physical therapy?

BACKGROUND AND PURPOSE: Acrophobia (fear of heights) may be related to a high degree of height vertigo caused by visual dependence in the maintenance of standing balance. The purpose of this case report is to describe the use of vestibular physical therapy intervention following behavioral therapy to reduce a patient's visual dependence and height vertigo. CASE DESCRIPTION: Mr N was a 37-year-old man with agoraphobia (fear of open spaces) that included symptoms of height phobia. Exposure to heights triggered symptoms of dizziness. Intervention. Mr N underwent 8 sessions of behavioral therapy

Neurotology.

As in most areas of clinical medicine, the history often is the key to determining the diagnosis in patients presenting with neurotologic symptoms. The most common cause of vertigo--benign positional vertigo--can be diagnosed at the bedside based on the characteristic history and the finding of fatigable positional nystagmus on the Dix-Hallpike positional test. Furthermore, it can be cured with a simple positioning maneuver. Quantitative auditory and vestibular function tests are important for documenting the site and severity of deficit in patients with chronic neurotologic symptoms. Only a f…

Stabilometry in balance assessment of dizzy and normal subjects.

Normal adults and patients referred to the Dizzy Clinic at the Medical College of Ohio had their standing balance assessed during combinations of normal and altered visual and somatosensory orientation conditions using a fixed-force platform to measure center-of-pressure translations. Significant differences were identified between normal subjects and dizzy patients, depending on the particular diagnostic category, the sensory condition tested, and the particular sway component being measured. Patients with central and peripheral vestibular dysfunctions had significantly greater sway than all

Vestibular signs and symptoms of volumetric abnormalities of the vestibular aqueduct.

OBJECTIVE: The objective of this study was to identify a pattern of signs, symptoms and neuroradiological findings which would assist investigation of vestibular function (especially otolith function) in a group of adult patients with anatomical alterations of the endolymphatic aqueduct and sac. METHODS: Fifteen subjects affected by volumetric abnormalities of the vestibular aqueduct were selected from a cohort of patients referred to a tertiary referral neurotological centre between 1 January 2004 and 30 June 2006. All patients underwent accurate clinical history-taking and were evaluated us

Vestibular Screening Before Cochlear Implantation: Clinical Implications and Challenges in 409 Cochlear Implant Recipients.

OBJECTIVE: Cochlear implantation (CI) carries a risk of loss of vestibular function following surgery. Thus, vestibular assessment presurgery is used to identify vestibulopathy that may contraindicate implantation and guide in selecting the candidate ear. The aim of this study was to evaluate the clinical implications of preoperative vestibular assessment, and to identify challenges in performing vestibular testing in patients with profound hearing loss, i.e., CI candidates. STUDY DESIGN: Retrospective study of all CI recipients implanted since the introduction of a vestibular screening progr

A Novel Stimulation Paradigm for Obtaining Interfrequency Amplitude Ratio of Ocular Vestibular Evoked Myogenic Potentials.

Purpose Interfrequency amplitude ratio (IFAR), the ratio of amplitude between ocular vestibular evoked myogenic potentials (oVEMPs) for tone bursts of 500 and 1000 Hz, aids in identification of Ménière's disease with high sensitivity and specificity. However, it requires recording of oVEMP for 2 frequencies, which doubles the time of recording. Use of simultaneous recording of responses for both frequencies could potentially bring down testing time. Simultaneous recording might be possible by modifying the oddball paradigm and using a ratio of 1:1 for the 2 frequencies. This might also reduce

Vestibular dysfunction in Turner syndrome: a case report.

OBJECTIVE: Turner syndrome is a well-known cause of sensorineural hearing loss, and the lack of estrogen has been implicated in cochlear dysfunction. It has never been associated with vestibular dysfunction. We report a case of a patient with Turner syndrome who was found to have bilateral vestibular dysfunction based on video-oculography (VOG) testing. PATIENT: A single patient with a history of Turner syndrome who was found to have significant bilateral vestibular dysfunction. INTERVENTION: After noticing a deficit in the vestibulo-ocular reflexes on qualitative horizontal head impulse exa

Changes in blood serotonin in patients with tinnitus and other vestibular disturbances.

The role of serotonin as a mediator and stress hormone and its function in vessel tonus regulation is proved. The aim of the work is to study the concentration of serotonin in blood in patients with vestibular disturbances before and after vestibular provocation and in patients with tinnitus. The study was performed on 134 persons distributed among three groups: group I, 35 patients with vestibular disturbances; group II, 75 healthy persons; group III, 24 patients with tinnitus. Serotonin was examined twice in patients with vestibular disturbances (group I) and in healthy persons (group II), b

Vertigo.

Vertigo is one of the most frequent reasons for consultation in daily medical practice. Recent studies show that vertigo involves considerable social costs before being managed efficiently, as it is often incorrectly diagnosed. Sometimes a psychiatric symptom, the original causes are vestibular in Ménière's disease, vestibular neuritis, and benign positional vertigo. Ménière's disease to some extent resembles that of progressive autoimmune deafness. Soon, perhaps, the target of the long-suspected immune reactions will be identified and specific treatments will be developed for the rapidly prog

[Strabismus induced by peripheral vestibular lesions].

Little attention has been paid in the ophthalmological literature to strabismus resulting from lesions located in the peripheral vestibular system. However, this phenomenon is commonly encountered in clinical practice. As a rule, this kind of strabismus shows a prominent vertical component. It is generally combined with a change in perception of verticality, conjugate cyclotorsion of the eyes, and head tilt. This association is known as "ocular tilt reaction". It occurs in a number of clinical settings which are believed to be related with alteration in the otolithical and/or vertical semicirc

Closed-loop caloric, harmonic acceleration and active head rotation tests: norms and reliability.

Norms and test-retest data from 20 persons with normal hearing and balance function were obtained for bithermal caloric (BC) using a closed-loop irrigator, sinusoidal harmonic acceleration (SHA) and active head rotation (AHR) tests. Each subject was tested twice with each test, with at least 24 hours between retest. Results of BC testing indicate that a unilateral weakness of 30% should be present to confidently identify a significant inter-ear difference when using a closed-loop irrigator for stimulus. Test-retest correlations of unilateral weakness were extremely poor, reflecting the potenti

Effectiveness of non-pharmacological treatments for vestibular and oculomotor dysfunction in patients with persistent post-concussive symptoms: protocol for a systematic review and meta-analysis.

INTRODUCTION: Concussion is a form of mild traumatic brain injury that disrupts brain function. Although symptoms are mostly transient, recovery can be delayed and result in persistent postconcussive symptoms (PPCS). Vestibular and oculomotor dysfunction are among the most debilitating impairments associated with PPCS. However, pharmacological interventions for these impairments are associated with deleterious side effects. Accordingly, increasing research has examined the utility of non-pharmacological interventions for PPCS. The aim of this review is to synthesise and evaluate the effectiven
