Ocular side effects possibly associated with isotretinoin usage.

PURPOSE: To evaluate reported ocular side effects associated with isotretinoin usage. METHODS: One thousand seven hundred forty-one case reports received from spontaneous reporting systems, data from the Drug Safety Section of Roche Pharmaceuticals, and the world's literature were evaluated using the World Health Organization Causality Assessment Guide of Suspected Adverse Reactions. RESULTS: Thirty-eight different signs or symptoms of ocular abnormalities associated with isotretinoin usage have been classified as "certain," "probable/likely," "possible," "unlikely," or "conditional/unclassi

Impact of contact lens correction on wavefront aberrations and vision quality in keratoconus.

PURPOSE: To review the optical principles of contact lens correction in keratoconus, with a focus on residual ocular wavefront error and its impact on vision performance. FINDINGS: Corneal rigid gas permeable (RGP) contact lenses, commonly used in keratoconus management, often fail to improve vision quality to levels observed in non-diseased eyes due to residual higher-order aberrations (HOAs), primarily originating from posterior corneal irregularity. Dependent on the nature of the residual wavefront error, various lens parameter modifications, including optic zone asphericity, base-down pri

Bilateral Iridochorioretinal Coloboma Managed with Low Vision Rehabilitation: A Case Report.

UNLABELLED: Ocular coloboma is a rare congenital disability. If involving the macula, it affects the patient's vision and subsequently affects childhood development and quality of life in the future. Appropriate low vision aid and timely rehabilitation can provide the best possible quality of life for visually impaired children. We report a 9-year-old boy who presented with a diminution of vision in both eyes, and who was just enrolled in pre-school. He was diagnosed with bilateral iridochorioretinal coloboma associated with nystagmus and unilateral cataract. After all the necessary evaluation

Ocular complications of cryptococcal meningitis in patients with HIV: report of two cases and review of the literature.

AIDS-related cryptococcal meningitis can result in significant vision loss, but the risk factors, prognostic features, and optimal management of patients with ocular complications is unknown. We present 2 cases of blindness associated with cryptococcal meningitis and review the literature for associated cases. Twenty-six additional cases of vision deterioration or loss as a result of HIV-associated cryptococcal meningitis were reviewed. Irreversible and complete loss of vision occurred in 14 patients (50%). Permanent vision loss was more likely to occur in patients with ocular symptoms and neu

Linezolid-induced optic neuropathy.

Many systemic antimicrobials have been implicated to cause ocular adverse effects. This is especially relevant in multidrug therapy where more than one drug can cause a similar ocular adverse effect. We describe a case of progressive loss of vision associated with linezolid therapy. A 45-year-old male patient who was on treatment with multiple second-line anti-tuberculous drugs including linezolid and ethambutol for extensively drug-resistant tuberculosis (XDR-TB) presented to us with painless progressive loss of vision in both eyes. Color vision was defective and fundus examination revealed o

Impact of Blur and Disparity Cues on Accommodation, Vergence, and Pupil Size in Response to Static Stimuli in Adolescents With and Without Concussion.

PURPOSE: To objectively investigate the effects of blur and disparity cues on accommodative response and microfluctuations (root mean square and low-frequency components), vergence, and pupillary responses to static targets in adolescents with persistent post-concussion symptoms. METHODS: Thirty-one adolescents with concussions and 32 controls completed the study. Refractive states, eye position (vergence), and pupil size were measured simultaneously using photorefraction (50 Hz) while participants viewed near stimuli for 15 seconds at nine distances ranging from 100 to 25 cm under three view

Individual differences in binocular coordination are uncovered by directly comparing monocular and binocular reading conditions.

PURPOSE: We evaluated systematically binocular coordination during a reading task by comparing binocular and monocular reading, and considering the potential effects of individual heterophoria and eye dominance. METHODS: A total of 13 participants (aged 19-29 years, refractive errors -0.5 to 0.125 diopters [D]) read single sentences in a haploscope while eye movements were measured with an EyeLinkII eyetracker. RESULTS: When reading monocularly, saccade amplitudes increased by 0.04 degrees and first fixation durations became longer by approximately 10 ms. Furthermore, saccade disconjugacies

Effect of myopia-control lenses on central and peripheral visual performance in myopic children.

PURPOSE: To evaluate the short-term effects of three myopia-control lenses, which impose peripheral myopic defocus while providing clear central vision, on central and peripheral visual performance in myopic children. METHODS: Twenty-one myopic children were enrolled in the study. Central visual performance was assessed using the quick contrast sensitivity function. Peripheral visual performance was evaluated by measuring peripheral contrast threshold and global motion perception, while subjects maintained fixation through the central portion of the lens. Single-vision spectacle lenses (SVL),

Changes in accommodation and convergence function after refractive surgery in myopic patients.

In recent years, with the gradual maturation of myopia correction surgery, the changes in binocular visual function after surgery have attracted widespread attention. The accommodation of facility, the amplitude of accommodation, relative accommodation, and accommodation convergence/accommodation assembly are essential parameters for assessing binocular visual function. The changes in these parameters are significant for guiding patients in the design of the preoperative surgical approach and alleviating postoperative visual fatigue. This paper aims to guide the preoperative assessment, design

Visual signs and symptoms of multiple system atrophy.

Multiple system atrophy (MSA) is a rare movement disorder and a member of the 'parkinsonian syndromes', which also include Parkinson's disease (PD), progressive supranuclear palsy (PSP), dementia with Lewy bodies (DLB) and corticobasal degeneration (CBD). Multiple system atrophy is a complex syndrome, in which patients exhibit a variety of signs and symptoms, including parkinsonism, ataxia and autonomic dysfunction. It can be difficult to separate MSA from the other parkinsonian syndromes but if ocular signs and symptoms are present, they may aid differential diagnosis. Typical ocular features

Visual axes in eyes that may be astigmatic and have decentred elements.

The visual axis of the eye has been defined in terms of nodal points. However, astigmatic systems usually do not have nodal points. The purpose of this note is to offer a modified definition of visual axes that is in terms of nodal rays instead of nodal points and to show how to locate them from knowledge of the structure of the eye. A pair of visual axes (internal and external) is defined for each eye. The visual axes then become well defined in linear optics for eyes whether or not they are astigmatic or have decentred elements. The vectorial angle between the visual axes and the optical axi

Ocular prevalence: difference between crossed and uncrossed disparities of stereo objects.

BACKGROUND: Many observers perceive a horizontal offset between a stereoscopically viewed object and a reference object in the fixation plane, although both objects are presented along a single line that projects through the midpoint between both eyes. These observers weigh one eye more than the other when determining the visual directions of stereo objects. This unequal weighting has been called ocular prevalence. We considered that ocular prevalence might be asymmetric, that is, different for crossed and uncrossed stereo disparities. METHODS: We examined 11 subjects with normal eyes and a s

Convergence accommodation to convergence (CA/C) ratio in patients with intermittent exotropia and decompensated exophoria.

PURPOSE: To evaluate the convergence accommodation to convergence (CA/C) ratio in strabismic patients and to clarify its clinical implications. METHODS: Seventy-eight consecutive patients (mean age: 12.9 +/- 6.0 years) with intermittent exotropia and decompensated exophoria who showed binocular fusion at least at near viewing were recruited. The CA/C ratio was estimated by measuring accommodative responses induced by horizontal prisms with different magnitudes under accommodation feedback open-loop conditions. The CA/C ratios were compared with accommodative convergence to accommodation (AC/A

Vertical vergence adaptation for normal and hyperphoric patients.

This paper discusses the use of vertical fixation disparity (VFD) measurements to determine a patient's ability to adapt to vertical prism. Vertical vergence adaptation for orthophoric patients, hyperphoric patients, and patients with vertical deviations associated with superior oblique paresis is reported. The effect of orthoptic training on the ability to adapt to vertical prism is also reported.

The efficacy of optometric vision therapy. The 1986/87 Future of Visual Development/Performance Task Force.

Vision is not simply the ability to read a certain size letter at a distance of 20 feet. Vision is a complex and adaptable information gathering and processing system which collects, groups, analyzes, accumulates, equates, and remembers information. In this review, some of the essential components of the visual system and their disorders which can be physiologically and clinically identified, i.e., the oculomotor, the accommodative, and the fusional vergence systems have been discussed. Any dysfunctions in these systems, can lessen the quality and quantity of the initial input of information i
