Surgical management of the late complications of proliferative diabetic retinopathy.

The late complications of proliferative diabetic retinopathy (PDR) comprise vitreous haemorrhage, tractional retinal detachment, combined tractional-rhegmatogenous retinal detachment, and severe fibrovascular proliferation (including macular distortion or dragging, tractional macular oedema, and media opacity due to fibrovascular tissue). This article will review the indications, techniques, and outcomes of vitrectomy surgery to treat these conditions. A careful assessment of the surgical anatomy, with particular attention to the configuration of vitreoretinal attachments, is important when de

Vanishing retinal arterial aneurysms: a case report.

An 18-year-old woman with retinal vasculitis developed multiple retinal arterial aneurysms over a period of 3 years. Subsequently she developed macular oedema, peripheral neovascularisation, and vitreous haemorrhage and was treated with systemic steroids, laser photocoagulation, and vitreoretinal surgery. No systemic cause for vasculitis was found. Serial fluorescein angiography demonstrated gradual resolution of the arterial aneurysms over the subsequent 7 years.

Small contact surface minified wide-angle contact lens for pediatric vitreous surgery.

The wide-angle contact lens for pediatric vitreous surgery is a modification of the existing wide-angle contact lens with a minified contact surface of 8.5 mm. The magnification is 0.387 and the field of view is 98 degrees/127 degrees. The lens facilitates performing complex pediatric vitreoretinal procedures due to its small contact surface and wide field of view of the fundus.

Intraocular Pressure Control and Long-Term Outcomes With the Reservoir Technique: The Wisconsin Silicone Oil Study (Report 2).

Purpose: To evaluate the long-term outcomes of silicone oil (SO) tamponade using the reservoir technique vs standard oil fill technique for complex vitreoretinal surgery. Methods: This retrospective comparative case series evaluated 313 SO tamponade surgeries (230 eyes). In the reservoir technique, the posterior segment is filled with SO. The infusion line is temporarily opened to atmosphere, allowing SO to egress into the line, creating the reservoir. The pressurized air infusion is then reset to 15 mm Hg to maintain a complete SO fill during sclerotomy closure. In the palpation method, SO is…

Endophthalmitis after cataract surgery: an update on recent advances.

PURPOSE OF REVIEW: Prevention and management of postcataract endophthalmitis remain quite relevant for anterior segment and vitreoretinal surgeons. Although the Endophthalmitis Vitrectomy Study, published in 1996, remains the only level 1 evidence for the management of postcataract endophthalmitis, recent advances have resulted in an evolution of practice patterns. The aim of this review is to summarize the literature regarding postcataract endophthalmitis with a focus on the last 18 months. RECENT FINDINGS: The IRIS registry indicates the rates of endophthalmitis are decreasing in the United

[Paediatric retinal detachment and hereditary vitreoretinal disorders].

The number of retinal detachments in children is very low in comparison to the number in adults. One predisposing factor for development of paediatric retinal detachment is suffering from hereditary vitreoretinal degeneration (e.g., Stickler syndrome, Wagner syndrome, Kniest dysplasia, familial exudative vitreoretinopathy, congenital X-linked retinoschisis, Knobloch syndrome, incontinentia pigmenti, Norrie disease). Hereditary vitreoretinopathies are characterised by an abnormal-appearing vitreous gel with associated retinal changes. In most of these eyes further ocular abnormalities can be di

Sutureless vitrectomy: evolution and current practices.

This review presents the evolution of sutureless vitrectomy and describes the minimisation of surgical techniques in the posterior segment, as well as their advantages and disadvantages. Shorter operating time, improvement of patient comfort and faster visual recovery are feasible when using small-diameter sutureless vitrectomy technique and equipment. However, a number of associated problems, such as postsurgical hypotony and endophthalmitis, were noticed initially in higher rates compared to 20-gauge vitreoretinal surgery. The purpose of this review is to summarise the available information …

Problems and timing in the removal of silicone oil.

The risk associated with silicone oil removal after complex vitreoretinal surgery is unclear. Therefore, a cohort of 87 consecutive cases of silicone oil removal were analyzed. Eyes with attached retina before silicone oil removal with a follow-up of at least 5 months were included into the study. Forty-eight eyes had severe proliferative diabetic retinopathy; 39 eyes had complex proliferative vitreoretinopathy or giant retinal tears after trauma. Additional clinical features included the presence of a secondary cataract or secondary glaucoma in some eyes. The rate of postoperative complicatio…

Glaucoma associated with the management of rhegmatogenous retinal detachment.

Transient or permanent elevation of intraocular pressure (IOP) is a common complication following vitreoretinal surgery. Usually secondary glaucoma, which develops after scleral buckling procedures, or pars plana vitrectomy for repair of rhegmatogenous retinal detachment, is of multifactorial origin. It is essential, for appropriate management, to detect the cause of outflow obstruction. An exacerbation of preexisting open-angle glaucoma or a steroid-induced elevation of IOP should also be considered. Scleral buckling may be complicated by congestion and anterior rotation of the ciliary body r…

Ophthalmic survey of a diabetic clinic: II. Requirements for treatment of retinopathy.

The eyes of 658 patients attending a district general hospital diabetic clinic were examined for retinopathy requiring treatment by photocoagulation or vitreoretinal surgery. Although the majority of patients with serious retinopathy were already under ophthalmic supervision, 39 cases (52 eyes) were found which required treatment. Eighty eyes had had previous photocoagulation, and treatment was recommended for maculopathy in 40 eyes, proliferative retinopathy in seven eyes, and combined maculopathy and proliferative retinopathy in five eyes. This suggested that existing referral criteria for p…
