Vulval lichen sclerosus and lichen planus.

Lichen sclerosus (LS) and lichen planus (LP) are both immunologically mediated diseases with a preference for the genitalia. The basic principles of management of vulval LS and vulvovaginal LP are the same and involve explanation of the disease, emphasizing the chronic nature of the condition and outlining treatment options. The main difference between the two conditions is that LP has a propensity to involve the mucous membranes including the mouth and vagina which are rarely affected in LS. First-line treatment for LS is a super-potent topical corticosteroid ointment which has a high respons

Enterobious vermicularis (pinworm) infestation of the vulva: report of 2 cases of a pseudoneoplastic lesion mimicking squamous carcinoma.

Enterobious Vermicularis (pinworm) infestation outside the gastrointestinal tract is rare. We report 2 patients with vulval involvement, one of whom presented with a clinically suspicious, rapidly growing mass. The histology of both lesions showed similar features of epidermal proliferation in the form of hyperkeratosis, acanthosis, and papillomatosis; this pseudoepitheliomatous hyperplasia raised the possibility of a well-differentiated squamous carcinoma. There was associated inflammation in both cases, including large numbers of eosinophils in 1 case. On the surface or within the keratin la

Pigmented lesions of the vulva.

Pigmented vulvar lesions, including diffuse hyperpigmentation, are present in 10% to 12% of white women. About 2% of them are nevocellular nevi. In general, nevi on the vulva are identical by morphologic and histopathologic criteria to nevi elsewhere on the body, with the exception of a small subset of nevi in younger women. Nevi in this subset have the unusual features of enlarged junctional nests that are variable in size, shape, and position. The long-term biologic behavior of these nevi has not been determined. Other benign pigmented lesions include lentigines, melanosis, postinflammatory

The significance of the histologic alterations adjacent to invasive vulvar carcinoma.

It has been superficially recognized, but not appreciated, that carcinoma of the vulva differs in many respects from carcinoma of the uterine cervix. Past studies have suggested that "leukoplakia and leukoplakia-like lesions" are probably premalignant conditions since they are commonly associated with invasive cancer. Conversely, studies of the dystrophies during the past 15 years have indicated that the latter uncommonly progress to invasive cancer if treated adequately. In an attempt to review the histopathologic changes associated with invasive cancer, the last 98 cases of invasive disease

Extramammary Paget Disease: a Therapeutic Challenge, for a Rare Entity.

PURPOSE OF REVIEW: Extramammary Paget disease (EMPD) is a rare entity which is more frequently localized at the vulva, though it only accounts for 1-2% of vulvar neoplasms. It is a primary cutaneous adenocarcinoma whose cell of origin is still a matter of controversy: it can either arise from apocrine/eccrine glands or from stem cells. The diagnosis demands a biopsy and entails a histopathological analysis by which cells show similar characteristics as breast Paget disease. RECENT FINDINGS: Treatment approach can entail surgery, radiotherapy, photodynamic therapy, systemic chemotherapy, and t…

Large bilateral labial fibroepithelial polyps with myxoid features: A case report and review of literature.

Fibroepithelial polyps are benign mesenchymal tumours that usually occur in skin folds, such as the axillae or groin. They are usually unilateral lesions measuring less than 5cm in their greatest dimension. We present the case of a 27-year-old woman with bilateral fibroepithelial polyps of the labia majora. This case is unusual not only because of the bilateral nature of the lesions, but also due to their size and the presence of a striking myxoid stroma on microscopic examination. When diagnosing vulval masses, it is essential to consider both benign and malignant conditions that may occur in

Metachronous metastasis from the right colon adenocarcinoma to the vulva: an unusual report and literature review.

A 67-year-old woman who was presented to the gynecologist with a two-month history of heavy vaginal bleeding, after an excisional biopsy of vulvar mass, was diagnosed with the right colon adenocarcinoma metachronous metastasis to the vulva. Pathological examination preliminarily revealed by primary adenosquamous carcinoma of vulva. The original submitting pathologist and gynecologist were contacted to obtain the pathological and clinical information whenever possible, combining with a history of the right colon cancer, subsequently, which confirmed the diagnosis of vulvar metastatic adenocarci

A vulvar fibroepithelial stromal polyp appearing in infancy.

We present here a first case of vulvar fibroepithelial stromal polyp (vFSP) appearing in infancy, including previously undescribed papule as an early form of vFSP. A 7-month-old girl presented with a small (adzuki-sized), soft, erythematous papule in the right labium majus. The size of the lesion increased up to 2.3 cm in the following 7 months and exhibited cerebriform or frond-like configuration. The surgically removed polyp was characterized by the proliferation of bland spindle cells with tapered cytoplasm, and the diagnosis of vFSP was established. Twenty-one months after the first surger

Pedunculated angiomyxoma of the vulva - a rare clinical entity.

BACKGROUND: Aggressive angiomyxoma is an uncommon mesenchymal tumor that preferentially involves the pelvic and perineal regions of females. Since its initial description in 1983, approximately 65 cases have been reported in the English literature. CASE: A 33-year-old parous woman with a slow growing pedunculated soft tissue tumour of the vulva which later underwent surface ulceration. Pathological findings in the case were suggestive of an aggressive angiomyxoma, a distinctive soft tissue non-malignant tumour, which could, at times be locally aggressive.
