In the Netherlands, more than 4000 patient cases of acute Q fever have been reported since 2007. Approximately 20% of these patients subsequently developed long-term fatigue. Such fatigue, as well as other physical symptoms and patient-perceived limitations in social functioning, have been compiled into what is known as the Q fever fatigue syndrome (QFS). The number of Dutch patients having QFS is expected to increase over the next years. The chance of a spontaneous recovery during the first six months following an acute Q fever infection is high. Prostate cancer is a common disease, affecting 1 in 8 men. The risk of mortality from untreated prostate cancer is limited in the first 5 to 10 years after treatment and as a result, the benefit of treatment is in men with greater than 10 years of life expectancy. When life expectancy is more limited, options for non-curative intervention include active surveillance, de-intensified surveillance, and watchful waiting. In this review, these strategies will be discussed.

We briefly describe the case of a 4-year-old girl, referred for imaging of a small, firm, round, skin-colored, subcutaneous nodule that suddenly appeared at her right sternoclavicular junction. A plain radiograph was non-contributory, but ultrasonography revealed a small cystic structure, leading to the diagnosis of a sternoclavicular ganglion cyst. Sternoclavicular ganglion cysts are a rare diagnosis, with only seven reported pediatric cases. A watchful waiting approach can be adopted so long as they remain asymptomatic.

Adolescent varicocele is associated with ipsilateral testicular hypotrophy and the concern for future infertility. A testicular size discrepancy greater than 15-20 % between left and right testicle is an indication for treatment to allow catch-up growth in the hope of preventing a future decline in fertility. Some authors advocate for a period of watchful waiting, as normal testicular growth may occur asymmetrically. We review the current literature to highlight some controversies and challenges in management.

Five cases of benign fetal arrhythmias presented with dramatic pictures of sustained irregular bradycardias. These cases were not associated with structural heart disease, the development of hydrops fetalis, or maternal autoantibodies. In all but one case, the arrhythmias resolved before delivery. Two fetuses had 2:1 atrioventricular block and the other three showed group beating due to blocked atrial trigeminy. With regular intermittent reevaluation, and if no signs of hydrops occur, watchful waiting is the suggested therapeutic modality.

Isolated vaginal bleeding before the onset of puberty is a rare presentation of isosexual precocity. In most cases, isolated vaginal bleeding without an abnormal genital examination is self-limited with resolution usually within 1 to 3 episodes. Watchful waiting is appropriate in most patients who do not have persistent bleeding, other signs of puberty, or signs/symptoms of an underlying etiology. Workup for patients with concerning features may include puberty hormone levels and/or transabdominal and transperineal ultrasound.

This guideline has been well thought out, and is a useful basis for the general practitioner to diagnose and manage hearing loss. However, it advises watchful waiting in children with otitis media, a questionable view. In evaluating simple diagnostic tools, the speech recognition is overvalued at the expense of audiometry and the tuning-fork test. Posttraumatic perforation of the eardrum, in combination with hearing loss is incorrectly presented as a self-limiting problem in the majority of cases.
