Complications of steroid use on Mt. Everest.

Steroids are used for the prevention and treatment of high-altitude illnesses. However, these agents can cause significant side effects. We report a case of altered mental status, gastrointestinal bleeding, skin rash, and avascular necrosis in a climber taking prophylactic dexamethasone prior to an attempt to climb Mt Everest. High-altitude cerebral edema (HACE), steroid toxicity, and acute adrenal crisis can have similar clinical presentations. Differentiating between these life-threatening conditions at high altitude is essential for successful treatment.

Coati (Nasua nasua) attacks on humans: case report.

Coatis [including Nasua nasua, the ring-tailed coati], are medium-sized mammals widely distributed in the Americas. They are social animals, whose normal diet includes insects, fruits, and small vertebrates, and rarely prey on larger sized animals. There are, to our knowledge, no reports in the medical literature of attacks on humans. This report describes a coati attack on 2 children in their home. The children sustained deep scratches and bites. The animal may have injured the humans in a defensive strike, but motivation for attack was uncertain. Coati attacks may occur in places where there

Sensationalistic journalism and tales of snakebite: are rattlesnakes rapidly evolving more toxic venom?

Recent reports in the lay press have suggested that bites by rattlesnakes in the last several years have been more severe than those in the past. The explanation, often citing physicians, is that rattlesnakes are evolving more toxic venom, perhaps in response to anthropogenic causes. We suggest that other explanations are more parsimonious, including factors dependent on the snake and factors associated with the bite victim's response to envenomation. Although bites could become more severe from an increased proportion of bites from larger or more provoked snakes (ie, more venom injected), the

Adverse encounters with alligators in the United States: an update.

OBJECTIVE: Severe injuries and fatalities can occur from an alligator attack. Encounters with alligators appear to be increasing in the United States. This review provides information from alligator attacks reported in the United States as well as infections that may occur after an alligator bite. METHODS: Telephone interviews were conducted with state wildlife offices in all Southern states in order to collect information on the number of alligator bites, nuisance calls, and the estimated alligator population of each state. Detailed information from alligator attacks in Florida is presented,

Sailing injury and illness: results of an online survey.

OBJECTIVE: The purpose of this study was to describe the relative frequency, patterns, and mechanisms of sailing-related injuries in dinghies and keelboats. Data were also collected on risky and risk-averse behaviors of sailors, as well as on sailing-related illnesses. METHODS: A web-based, logic-driven, multiple-choice survey was developed and links were posted on sailing-related websites. Data were collected from March through November 2006 on any injuries or illnesses sailors sustained over the prior 12 months. RESULTS: From 1188 respondents, a total of 1715 injuries and 559 illnesses was

Extensor hood syndrome--osteophytic irritation of digital extensor tendons in rock climbers.

OBJECTIVE: Injuries to the flexor tendons and flexor tendon pulleys are frequently reported in rock climbers. Osteoarthritic changes with bone spurs are also well known. We report on the less commonly described extensor tendon irritation caused by such osteophytes. METHODS: Thirteen high-level rock climbers (12 men, 1 woman; average age 33.8 years [range 17-55]; average years of climbing experience 19 [range 5-30]; average climbing level 10.2) with extensor hood irritation caused by dorsally located osteophytes of proximal interphalangeal (n = 10) or distal interphalangeal joints (n = 3) were

Coracoid impingement syndrome due to intensive rock climbing training.

Overuse and acute injuries to the upper body are common in rock climbing. Such injuries primarily affect the fingers; but shoulder problems are increasingly common, especially among more experienced and older climbers who climb at a high ability level. Such shoulder problems are often due to subacromial impingement, shoulder dislocations with bankart lesions, hyperlaxity, SLAP lesions or irritations of the long biceps tendon. In contrast to these known conditions, we describe a case of an ambitious female rock climber who trained intensively and developed a coracoid impingement caused by hyper

The global snakebite crisis--a public health issue misunderstood, not neglected.

The global problem of venomous snakebite continues to attract attention despite it being described as a "neglected" issue. The current focus of the World Health Organization (WHO) remains anti-snake venom quality, although "availability and sustainability" of supply are consistently described as the key issues. Sustainability of antivenom supply has been elusive, with cost and pricing in developing countries being cited as the major reasons. The current WHO approach fails to explore the cost issue, but rather focuses on quality improvements, which may well adversely affect the costs of a produ

Scorpion sting envenomation in children in southeast Turkey.

BACKGROUND: Scorpion sting envenomation is a life-threatening emergency and a common public health problem in many regions of the world, particularly in children. Children are at greater risk of developing severe cardiac, respiratory, and neurological complications. OBJECTIVES: The aims of this study were to evaluate demographic, clinical, laboratory, treatment, and outcome characteristics for pediatric patients with scorpion sting envenomation in southeast Turkey and to describe features that may be predictive of the need for pediatric intensive care unit (PICU) care. METHODS: A total of 52

Fatal fall into a volcanic fumarole.

Fatalities secondary to inhalation of volcanic gases in the United States have rarely been reported. We report the deaths of 3 ski patrol members at a popular California ski resort. After a snowstorm, ski patrol members were fencing off a well-known volcanic fumarole when the snow around the vent collapsed. Two members slid into the deep hole and rapidly lost consciousness. A third member carrying oxygen descended into the hole and also lost consciousness. A fourth member affixed an oxygen mask, but still lost consciousness upon descent. The 3 initial victims expired at the scene, while the fo

Recreational surfing injuries in Cornwall, United Kingdom.

OBJECTIVE: To analyze the nature of surfing injuries in Cornwall in order to identify trends and inform clinical management. METHODS: The details of patients presenting (with injuries sustained while surfboard/bodyboard riding) to the Emergency Department (ED) of the Royal Cornwall Hospital, Truro (UK), from September 2004 until August 2006 were recorded prospectively. The notes were then retrospectively reviewed by a senior ED physician. The records of each visit were scrutinized for date, age, sex, injury type, and injury severity and outcome; in addition, the patient's residential status (

Ocular toxicity associated with indirect exposure to African spitting cobra venom.

Direct ocular inoculation with African spitting cobra (Naja nigricollis) venom in the United States is uncommon, especially in an urban setting, but can lead to serious acute and chronic ocular injury depending on the extent of exposure. We report 2 cases of indirect ocular inoculation with venom from an African spitting cobra, manifesting as periocular soft tissue swelling, extensive conjunctivitis, and corneal epithelial erosion. Both of the reported cases involve young male patients who received prompt emergency evaluation and treatment including copious irrigation of the ocular surface, fo

Time for an alternative perspective: the eternal problem of supply and quality of anti snake venom in the developing world--"it's the economy, stupid".

The "crisis in anti snake venom supply" has been an enduring problem. Despite the frequency with which it appears in the literature, it remains unquantified and an enigma. If there is a serious shortage of anti snake venom (ASV), why has this not been resolved? Anti snake venoms are produced, and yet many suppliers are described as leaving the market. There appears to be a problem in the call for highly effective, high-quality, and cheap anti venoms that contributes to this result of suppliers leaving the market. Private companies are tasked with achieving adequate shareholder returns and by d

A case of elevated liver function tests after crown-of-thorns (Acanthaster planci) envenomation.

The crown-of-thorns starfish (Acanthaster planci) inhabits coral reefs, largely throughout the Indo-Pacific region. Its dorsal surface is covered with stout thorn-like spines. When handled or stepped on by humans, the spines can puncture the skin, causing an immediate painful reaction, followed by inflammation and possible infection. Initial pain and swelling may last for days. Effects of envenomation on the liver have been demonstrated previously in animal models, but hepatic toxicity has not previously been described in humans. We describe elevated liver enzymes in a 19-year-old female assoc

Tick attachment sites.

OBJECTIVE: The duration of tick attachment is related to the likelihood of disease transmission. To be able to locate and remove an attached tick is important. METHODS: Sixty-seven patients presenting with tick bites to the Karadeniz Technical University emergency department and 3 different first-stage health institutions between April and July 2006 were investigated to determine the distribution of the bites. RESULTS: Tick attachment site distribution was determined as follows: 9% head-neck, 5% arm, 24% stomach-groin, 7% back, 18% chest-shoulder, 25% leg-foot, and 12% hip. CONCLUSIONS: Acc

Mountain sickness knowledge among foreign travelers in Cuzco, Peru.

OBJECTIVE: Cuzco, Peru, is host to a dangerous mix of high altitude and rapid access from low elevation, which results in a high prevalence of acute mountain sickness (AMS). Thus, it is important that travelers entering Cuzco understand the basics of AMS. To this end, we assessed travelers' awareness of AMS, especially the resources used to obtain this knowledge. With this information we make recommendations with regard to better avenues for distribution of AMS information. METHODS: One hundred foreign travelers, representing an international population, completed a 45-item written questionna
