The central nervous system and infection by Candida species.

In this paper we have reviewed the main clinico-pathologic disease groups of neurocandidiasis: the microabscesses, the macroabscesses, and the meningitis. Special attention has been paid to the predisposing conditions for the appearance of neurocandidiasis, the neuroimaging techniques, and the study of the cerebrospinal fluid, needed for diagnosis. We have also discussed the differential diagnosis with other illnesses. Treatment should be given with amphotericin-B and 5-fluorocytosine. The use of other antifungal drugs for neurocandidiasis is also discussed.

[Acute rhinosinusitis and sinusitis].

During the common cold in the adult, involvement of the sinusal mucosa, confirmed by CT scan, is virtually constant and justifies the term "rhinosinusitis" that is now applied. The presence of bacterial superinfection can only be demonstrated by positive laboratory results on a sample, which is never performed in usual practice; thus the practitioner must rely on presumptive factors. Among them, an essential element is the unilateral nature of painful symptoms and purulent rhinorrhoea. When symptoms are bilateral, antibiotics should not be prescribed except when the condition has not been reso

Actinomycotic splenitis and intestinal volvulus in an alpaca (Lama pacos).

Morphologic, microbiologic, and polymerase chain reaction amplification techniques were used to evaluate an alpaca (Lama pacos) with splenitis and intestinal volvulus. The intestinal volvulus produced a severe necrosuppurative typhlocolitis associated with vascular thrombosis and was most likely the cause of death of this animal. In addition, this animal had multiple coalescing abscesses affecting most of the splenic tissue. The isolation of Actinomyces spp. from the spleen and the morphology of the colonies when stained with Gram and Steiner stains support a diagnosis of splenic actinomycosis

[Pulmonary nocardiosis: presentation of 3 clinical cases].

INTRODUCTION: Nocardiosis is an infectious disease that frequently involves to the respiratory tract. Nocardia asteroides is the predominant human pathogen. Nocardiosis is difficult to diagnose and that's why its incidence is not well established. CASES: Three cases diagnosed in a Pneumology Section are reported. None of them was affected by AIDS and all of them were initially diagnosed from bronchopneumonia. Empirical antibiotherapy was initiated and because of the unsatisfactory progress, bronchoscopy was performed in all cases and in one case a transthoracic puncture was made. Nocardia spp

Cerebral Salmonella typhimurium abscess in a patient with a stroke.

A 59-year-old woman with renal failure was admitted in a coma with seizures. Computerized tomography (CT) revealed a frontal mass in the right cerebral hemisphere and on day 12 new low density areas had appeared in both the right and left hemisphere. Blood cultures on day 13 grew Salmonella typhimurium, and the patient died the next day. An autopsy showed cerebral infarction with abscess formation. The elusive features of non-typhoid Salmonella infections make it advisable to obtain blood and faecal cultures from susceptible patients with fever or focal disorders of unknown aetiology.

Image-guided surgical techniques for infections and trauma of the central nervous system.

The value of image-guided stereotactic systems is directly dependent on the ease and speed of their use. In the past, most stereotactic techniques were complicated and timely to set up; thus, they were used exclusively for either resecting neoplasms or for neurologic function. However, current systems equipped with advanced registration techniques are much simpler and faster to employ, and indications for their use are rapidly increasing. We describe an advanced image-guided navigation system and provide examples of its successful use in neurosurgical treatment of central nervous system infect

Bilateral coronal craniosynostosis with novel TWIST1 mutation.

INTRODUCTION: Syndromic craniosynostosis is characterized by premature fusion of one or more cranial sutures, often in association with multisystem anomalies affecting the airway, cardiovascular, musculoskeletal, and neurodevelopmental systems. Variants in genes such as TWIST1 contribute to phenotypic heterogeneity and may influence surgical timing, risk stratification, and long-term craniofacial planning. We present a severe syndromic craniosynostosis phenotype associated with a previously undescribed TWIST1 variant and discuss perioperative considerations of staged cranial vault reconstructi

Fungal brain infection-no longer a death sentence.

The aim of this case series was to provide a modern cohort of patients with cerebral aspergillosis and show the effectiveness of modern treatment concepts. In a 10-year period from January 2009 to January 2019, we identified 10 patients (6 male, 4 female) who received surgery or frameless stereotactic drainage of a cerebral aspergilloma at our center. Patients' and disease characteristics were recorded. The median age was 65 (range 45 to 83). We conducted 133 cranial surgeries in 100 patients due to cerebral brain abscess (BA) during that time, which leads to a percentage of 10% of aspergillom…

The Role of the Visor Flap in Scalp Reconstruction: A Case Series of 21 Patients.

BACKGROUND: Full-thickness scalp defects pose a reconstructive problem, especially in the setting of infection, chemotherapy/radiation, and underlying cranial defects. Current options include dermal matrices, skin grafts, and local flaps. Local flaps often fail, requiring subsequent microvascular free flap reconstruction. OBJECTIVE: To describe the visor flap, a novel bipedicled advancement flap design, and its role in reconstruction of scalp defects. METHODS: A retrospective review of 21 adult patients who developed scalp defects reconstructed using the visor flap from 2013 to 2017. The vis

Bone flap resorption: risk factors for the development of a long-term complication following cranioplasty after decompressive craniectomy.

Aseptic bone flap resorption (BFR) is a known long-term complication after cranioplasty (CP). We analyzed our institutional data in order to identify risk factors for BFR. From October 1999 to April 2012, 254 patients underwent CP after decompressive craniectomy (DC) at our institution, and had a long-term follow-up period of >1 year after CP (range 12-146 months). Overall, BFR occurred in 10 of 254 patients as a long-term complication after CP (4%). BFR developed more often in patients aged ≤18 years (p=0.008), in patients who previously underwent DC for traumatic brain injury (p=0.04), and i

Salvage endoscopic nasoseptal flap repair of persistent cerebrospinal fluid leak after open skull base surgery.

PURPOSE: Persistent cerebrospinal fluid (CSF) rhinorrhea after open skull base surgery can be challenging to manage due to the risk of meningitis, brain abscess, surgical morbidity associated with revision craniotomy, and the lack of available healthy autologous tissue after failure of a pericranial flap. Given the recent success of the vascularized pedicled nasoseptal flap (PNSF) for reconstruction after endoscopic skull base surgery, we have adopted this technique as a salvage method to treat recalcitrant CSF rhinorrhea after previous open skull base surgery in order to avoid revision cranio…
