Report from the National Transplantation Pregnancy Registry (NTPR): outcomes of pregnancy after transplantation.

Female solid organ recipients with good graft function generally tolerate pregnancy well. However, the combination of mother, fetus, transplanted organ, and immunosuppressive and other medications increases the complexity of management and raises the specter of adverse outcomes. For the mother, considerations include the nature of the original disease (i.e. genetic risk of transmission), co-morbid conditions which increase pregnancy risk (i.e. hypertension, diabetes, renal insufficiency), and long-term maternal prognosis. For the fetus, questions include the adequacy of maternal physiology (ca

The Collision of Mental Health, Substance Use Disorder, and Suicide.

Suicide is the 10th leading cause of death, and, in women, nearly half of all suicide deaths occur during their reproductive years. Suicide is associated with psychiatric illness, especially mood and anxiety disorders. Childhood adversities, such as physical, emotional, and sexual abuse, and intimate partner violence increase the risk of suicide. Having more than one psychiatric disorder or comorbid substance use disorder also increases the risk of suicide. Substance use disorders can be the triggering factor for a suicide attempt among those who have a psychiatric condition and suicidal thoug

Influence of past trauma and health interactions on homeless women's views of perinatal care: a qualitative study.

BACKGROUND: Homeless women are twice as likely to become pregnant and are less likely to receive antenatal care than women who are not homeless. Prevalent biopsychosocial complexity and comorbidities, including substance use and mental illness, increase the risk of obstetric complications, postnatal depression, and child loss to social services. AIM: To explore the perspectives of women who have experienced pregnancy and homelessness to ascertain how to improve perinatal care. DESIGN AND SETTING: A qualitative study with a purposive sample of women who had experienced pregnancy and homelessn

Accuracy of immediate antepartum ultrasound estimated fetal weight and its impact on mode of delivery and outcome - a cohort analysis.

BACKGROUND: The aim of the study was to investigate the accuracy of ultrasound-derived estimated fetal weight (EFW) and to determine its impact on management and outcome of delivery. METHODS: In this single-center cohort analysis, women with a singleton term pregnancy in the beginning stages of labor were included. Women with immediately antepartum EFW (N = 492) were compared to women without ultrasound (N = 515). RESULTS: EFW was correct (deviation from birth weight ≤ 10%) in 72.2% (355/492) of patients with fetal biometry; 19.7% (97/492) were underestimated, and 8.1% (40/492) were overesti

Penn State Worry Questionnaire - 10: A new tool for measurement-based care.

The Penn State Worry Questionnaire - Past Week (PSWQ-PW) is an adaptation of the widely used Penn State Worry Questionnaire, measuring pathological worry weekly. However, it contains problematic negatively worded items and has not been validated in a large sample yet. To meet the needs of measurement-based care (MBC), we developed a shortened version (PSWQ-10) based on the PSWQ-PW, retaining only positively worded items, and examined its psychometric properties and clinical utility. Patients with Generalized Anxiety Disorder (GAD), Major Depressive Disorder (MDD), and other anxiety disorders c

Long-term outcome of severe puerperal psychiatric illness: a 23 year follow-up study.

BACKGROUND: Although there have been many follow-up studies of severe puerperal psychiatric illness, few have been very long-term. METHODS: Sixty-four subjects from 85 (75.3%) in an unselected sample of women admitted to a psychiatric hospital within 6 months of childbirth were successfully followed up a mean of 23 years (range 17-28) later. Most subjects were interviewed in detail, with further information obtained from general practice and hospital records. Data included subsequent illnesses and diagnoses, subsequent childbirth, longitudinal social function, current symptoms and social func

Correlates of postnatal depression in mothers and fathers.

BACKGROUND: We compare and contrast some correlates of paternal and maternal depression after the live birth of a first child, as part of a longitudinal study. METHOD: Fifty-four first-time mothers attending obstetric services in Oporto, Portugal, and 42 of their husbands or partners participated in a longitudinal study of their mental health. All subjects were given a semi-structured clinical interview (SADS) at six months antenatally and at 12 months postnatally and sub-samples were interviewed at three months postnatally. At all these times all the mothers and fathers also completed a tran

Examining dynamic changes in prefrontal connectivity and internalizing symptoms during the first and third trimesters of pregnancy.

Pregnancy is characterized by pronounced neuroplasticity and heightened risk for psychopathology. While previous research has primarily examined postpartum depression, less work has investigated changes in internalizing symptoms measured dimensionally across the peripartum period. Furthermore, limited research has identified functional changes in the prefrontal cortex (PFC) across pregnancy, a brain area implicated in psychopathology. Understanding the dynamic changes in internalizing symptoms and PFC neuroconnectivity throughout pregnancy and their interrelations is crucial for early identifi

Associations Between Mental Health Conditions and Adverse Pregnancy Outcomes Among Active-Duty Servicewomen.

BACKGROUND: Active-duty servicewomen (ADSW) are at increased risk for mental health conditions compared to civilian women. Research suggests women with mental health conditions experience serious pregnancy complications at rates 50% higher than those without mental health conditions; yet research in military populations is limited. OBJECTIVE: To explore associations between mental health conditions and adverse pregnancy outcomes among ADSW. METHODS: Department of Defence data were used to identify a cohort of live birth pregnancies among ADSW that started and ended between October 2016 and D

Comorbidities of Adolescent Pregnancy at a Tertiary Teaching Hospital in Southwestern Uganda.

Background Adolescent pregnancy presents significant health challenges worldwide. Each year, millions of teenage girls in developing regions become pregnant. In Uganda, teenage pregnancy remains common, with marked regional variations. Adolescents in low socioeconomic situations, with limited education, or who face barriers to contraceptive access are more prone to unintended pregnancies. These pregnancies may result in multiple health complications, including premature births, small for gestational age babies, postpartum depression, anaemia, and urinary tract infections (UTIs). Methods A cros

Prevalence of psychiatric disorders during pregnancy-a feasibility study at second trimester ultrasound in the general population (GROUP study): study protocol.

INTRODUCTION: During the perinatal period, women have an increased risk for psychiatric disorders, which are highly prevalent in this context. In addition, there are significant delays in diagnosing these conditions, worsening their prognosis and increasing their societal burden. Studies describing psychiatric disorders in the perinatal period often focus on specific disorders; only postpartum depression and, to a lesser extent, anxiety disorders are studied. There are also very few evaluations conducted by clinicians based on a semistructured interview, relying on the diagnostic criteria of i

Infant Distress From the Still-Face and its Association With Increased Problem Solving: Implications for Early Resilience.

OBJECTIVE: This study aimed to investigate the relationship between infants' negative affect and problem-solving skills, along with the moderating role of mothers' perceived social support. METHODS: The sample consisted of 110 infants (49% females) and their mothers, who participated in the Perinatal Experiences and COVID-19 Effects (PEACE) Study at 2 time points: T1 (peripartum period, from late second trimester to 6 months postpartum) and T2 (when the infants were 8-10 months old). We used the Face-to-Face Still-Face (FFSF) procedure as an observational task to assess infant negative affect

Maternal Sleep Disorders and Maternal and Birth Outcomes: A Retrospective US Claims-Based Study.

INTRODUCTION: Sleep disturbances are commonly reported during pregnancy and have been associated with adverse maternal, fetal, and neonatal outcomes. However, self-reported sleep disturbances may not be accurately reflected in the prevalence of clinically diagnosed sleep disorders. METHODS: Using Optum's de-identified Clinformatics Data Mart Database, we conducted a cross-sectional study of 93,767 American women who were pregnant with singletons between January 1, 2015 and June 30, 2021, to (1) determine the prevalence of clinically diagnosed sleep disorders and breathing abnormalities; (2) e

A core outcome set for maternal and neonatal health research and surveillance of emerging and ongoing epidemic threats (MNH-EPI-COS): a modified Delphi-based international consensus.

BACKGROUND: Disease outbreaks significantly affect maternal and neonatal health. Variability in reporting health outcomes hinder evidence generation. We aimed to develop a core outcome set (COS) for maternal and neonatal health research and surveillance during emerging and ongoing epidemic threats and to agree on outcomes' definitions. METHODS: We conducted a systematic review of observational and experimental studies related to epidemics to identify outcomes, and a four-stage modified-Delphi consensus. 150 international stakeholders participated in online surveys, and 24 representatives in c

Maternal health outcomes associated with ambient air pollution: An umbrella review of systematic reviews and meta-analyses.

A growing body of literature demonstrated an association between exposure to ambient air pollution and maternal health outcomes with mixed findings. The objective of this umbrella review was to systematically summarize the global evidence on the effects of air pollutants on maternal health outcomes. We adopted the Joanna Briggs Institute (JBI) methodology and Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) reporting standards for this umbrella review. We conducted a comprehensive search across six major electronic databases and other sources to identify relevant sys

Fibromyalgia pathogenesis explained by a neuroendocrine multistable model.

Fibromyalgia (FM) is a central disorder characterized by chronic pain, fatigue, insomnia, depression, and other minor symptoms. Knowledge about pathogenesis is lacking, diagnosis difficult, clinical approach puzzling, and patient management disappointing. We conducted a theoretical study based on literature data and computational analysis, aimed at developing a comprehensive model of FM pathogenesis and addressing suitable therapeutic targets. We started from the evidence that FM must involve a dysregulation of central pain processing, is female prevalent, suggesting a role for the hypothalamu
