Experience in the treatment of synchronous and metachronous carcinoma of the oesophagus and the head and neck.

BACKGROUND AND OBJECTIVES: Treatment of multiple primary squamous cell carcinomas of the head and neck and oesophagus is controversial. The poor prognosis of these 2 types of carcinoma taken individually and their anatomic proximity complicate the therapeutic strategy and limit the treatment choices for each location. METHODS: From 1986 to 1998, 43 patients received curative treatment for multiple synchronous (n = 30) or metachronous (n = 13) primary neoplasms of the oesophagus and head and neck. For synchronous cancers, the therapeutic strategy consisted of first curing the head and neck can

Should malrotation in children be treated differently according to age?

PURPOSE: The aim of this study was to better define the mode of presentation, rate of volvulus, and surgical findings in children younger than 2 versus older than 2 years of age with malrotation. METHODS: The authors reviewed the charts of all patients with malrotation admitted to their hospital between January 1980 and December 1998, excluding patients having malrotation as a secondary finding. RESULTS: An upper gastrointestinal series was done in 90 patients (6% falsely negative) and a barium enema in 20 patients (40% read as normal). Fifty-eight patients had 114 associated congenital anom

Peripheral blood lymphocyte changes after benign gastrointestinal surgery. Role of thymostimulin in reversing the postoperative immunodepression seen in humans.

A generalized state of immunosuppression during surgery has been implicated in the development of septic complications postoperatively. We studied 18 patients operated upon for benign diseases, to examine the influence of surgical trauma on circulating lymphocyte subpopulations in man. Additionally, we evaluated the effect of thymostimulin on these changes, in another group of 10 patients. Our results suggest that the total number of lymphocytes, as well as lymphocyte subpopulations CD3+ and CD4+, fell significantly following surgery. This reduction in cell number is more pronounced on the hel

Life Support in the ICU: Outcomes among Patients Undergoing Gastrointestinal Cancer Surgery.

BACKGROUND: Postoperative intensive care use after gastrointestinal cancer surgery may reflect evolving perioperative triage, yet national patterns of ICU admission and life-support intervention use remain poorly defined. STUDY DESIGN: Adults undergoing gastrointestinal cancer surgery identified in Epic Cosmos between 2016 and 2025 were identified. ICU admission and postoperative life-support intervention use, defined as invasive mechanical ventilation for greater than or equal to 24 consecutive hours or continuous vasopressor infusion during the index hospitalization, were assessed over time

Early postoperative pain and 30-day complications following major abdominal surgery: a retrospective cohort study.

BACKGROUND: Increasing evidence supports a positive relationship between the intensity of early postoperative pain, and the risk of 30-day postoperative complications. Higher pain levels may hamper recovery and contribute to immunosuppression after surgery. This leaves patients at risk of postoperative complications. METHODS: One thousand patients who underwent major abdominal surgery (cytoreductive surgery and hyperthermic intraperitoneal chemotherapy, esophageal, liver, or pancreas surgery) at the Radboud university medical center were randomly selected from eligible patients between 2014 a

Opioid prescriptions: are they really necessary after major intra-abdominal surgery? A single-centered prospective study.

BACKGROUND: In an effort to curb opioid use, we instituted a no routine opioids protocol for major colorectal and general surgical procedures requiring inpatient admission. METHODS: This was a prospective cohort study with preintervention (control) and postintervention groups that included abdominal and pelvic surgery requiring inpatient admission. The control groups received opioid prescriptions at discharge in keeping with the surgeon's standard practice whereas the intervention group received counseling on the use of nonopioid analgesics and avoiding opioid use. The goal of the interventio

Evaluating the therapeutic quality of prehabilitation programmes in patients scheduled for colorectal surgery: A systematic review and meta-analysis.

AIM: The aim of this work was to evaluate whether the therapeutic quality of exercise prehabilitation programmes is associated with their effectiveness to preoperatively improve aerobic fitness and reduce postoperative complications and length of hospital stay in patients scheduled for colorectal surgery. METHOD: Three electronic databases (PubMed, Embase and CINAHL) were systematically searched (up to October 2023) for randomized controlled trials that investigated the effects of prehabilitation before colorectal resection. Methodological quality and therapeutic quality were assessed using,

Association of hospital ownership status with outcomes after major gastrointestinal surgery.

BACKGROUND: For-profit (FP) hospital ownership in the United States has increased. Although hospital financial structures may influence quality of care, the impact of FP ownership on outcomes after gastrointestinal (GI) surgery remains unclear. We sought to assess the association among FP hospital ownership, postoperative outcomes, and expenditures among patients undergoing 5 major GI surgical procedures. METHODS: Using the 2016-2019 National Inpatient Sample, adults undergoing esophagectomy, hepatectomy, pancreatectomy, colectomy, and proctectomy were identified. Multivariable regression was

Risk factors for postoperative swallowing dysfunction in elderly patients undergoing gastrointestinal cancer surgery: A retrospective cohort study.

PURPOSES: Postoperative swallowing dysfunction is a complication that adversely affects elderly patients undergoing gastrointestinal cancer surgery. However, the perioperative changes in swallowing function and their predictive factors remain unclear. METHODS: The subjects of this retrospective analysis were 170 patients aged ≥ 65 years who underwent gastrointestinal cancer surgery and screened positively for sarcopenia and/or cognitive or swallowing concerns. Swallowing function was assessed using the Fujishima Eating and Swallowing Ability Grade (Fujishima Grade), a 10-point scale commonly

Single-Incision Laparoscopic Endorectal Pull-Through for Hirschsprung's Disease in Neonates: A Prospective Study on Medium-Term Outcomes.

BACKGROUND: Single-incision laparoscopic endorectal pull-through (SILEP) has gained attention as a minimally invasive alternative to conventional laparoscopic surgery (CLS) for Hirschsprung's disease (HD). While SILEP offers superior cosmetic outcomes and reduced postoperative morbidity, its feasibility, safety, and functional outcomes in neonates remain underexplored. This study aimed to prospectively evaluate the outcomes of SILEP in neonates with HD. PATIENTS AND METHODS: Between January 2020 and December 2021, 23 neonates (≤28 days of age) underwent SILEP at our institution. The procedure

Diagnostic value of T-tube cholangiography and choledochoscopy in residual calculi after biliary surgery.

BACKGROUND: T-tube cholangiography and choledochoscopy are commonly used techniques for detecting residual bile duct stones after biliary surgery. However, the utility of routine cholangiography before T-tube removal needs further investigation. This study aims to evaluate the diagnostic efficacy of various methods for detecting residual calculi following biliary surgery. METHODS: We retrospectively analyzed the clinical data of 287 adult patients who underwent common bile duct exploration with T-tube drainage, followed by T-tube cholangiography and choledochoscopy, at the Department of Gener

[Comparison of early operative treatment and 48-hour conservative treatment in small bowel obstruction (COTACSO): intermediate results].

UNLABELLED: Optimal treatment for adhesive small bowel obstruction (SBO) is not defined. Surgery is the only method of treatment for obvious strangulating SBO. Non-operative management (NOM) is widely used among patients with low risk of strangulation, i.e. no clinical, laboratory and CT signs. Randomized controlled trials (RCTs) are recommended to determine the optimal method (early intervention or NOM), but their safety is unclear due to possible delay in surgery for patients needing early intervention. MATERIAL AND METHODS: A RCT is devoted to outcomes of early operative treatment and NOM

Serial Transverse Enteroplasty (STEP) for Short Bowel Syndrome (SBS) in Children: A Multicenter Study on Long-term Outcomes.

OBJECTIVES: Short Bowel Syndrome (SBS), secondary to various underlying diseases, is one of the main causes of intestinal failure in children. Surgical management by serial transverse enteroplasty (STEP), is feasible in selected cases, but results on long-term follow-up are scarce. The aim of this study was to report long-term outcomes of the STEP procedure in children with SBS. METHODS: We performed a multicenter national retrospective study reviewing medical charts of children who underwent a STEP technique between 2000 and 2022 in 6 university hospitals. Collected data included demographic

Impact of intraoperative adverse events in general and gastrointestinal surgery: A nationwide study.

BACKGROUND: Intraoperative adverse events (iAEs) during general surgery can lead to significant morbidity and healthcare burden, yet their impact remains underexplored. We aimed to estimate the nationwide incidence of iAEs in general surgery and explore their associations with mortality, complications, length of stay, and costs. METHODS: We conducted a retrospective cohort study using the Nationwide Readmissions Database 2019 and included adult patients (aged 18 years and older) who underwent general surgical procedures. Eligible patients were grouped based on the presence of an iAE, defined

Nutritional Outcomes of Bowel Lengthening Procedure in Patients with Short Bowel Syndrome.

BACKGROUND: Although parenteral nutrition (PN) significantly improves mortality rates in pediatric short bowel syndrome (SBS), long-term PN has many possible complications and impacts quality of life. Bowel lengthening procedures (BLPs) increase the contact surface of food and the intestinal mucosa and enable the better absorption of nutrients and liquids, possibly leading to a PN decrease. METHODS: We retrospectively reviewed the data of patients with short bowel syndrome who underwent BLPs in the period from January 2016 to January 2022. Overall, eight patients, four male, five born prematu

Anal fistulotomy with one-stage shaped skin grafting for intersphincter anal fistulas: study protocol on a multicentre randomised controlled trial.

BACKGROUND: Anal fistulas are mainly treated via surgery. They can be difficult to treat without surgical intervention. Numerous procedures, such as fistulectomy and fistulotomy, are performed to treat anal fistulas and achieve good effects. However, the wounds created through fistulectomy and fistulotomy take a long time to heal. Therefore, a multicentre randomised controlled trial (RCT) is proposed to study the efficacy of one-stage shaped skin grafting at the surgical wound to heal low simple intersphincter anal fistulas. METHODS: This study is a multicentre, hospital-based RCT. It will be
