[Parotid gland masses: diagnostic value of MR imaging with histopathologic correlations].

BACKGROUND AND PURPOSE: The purpose of our study has been to describe the pre-operatively MR Imaging appearance of masses in the parotid gland which may be sources of clinical and radiological confusion. METHODS: Sixty-eight adults patients with a parotid mass (39 female, 29 male) has been prospectively investigated. T1-weighted, T2-weighted, diffusion-weighted and contrast-enhanced dynamic MR images were performed by using a 1.5 or 1 T MR Imaging unit (Philips Gyroscan Intera 1.5 T. Siemens Expert 1 T). All patients underwent a parotidectomy with histopathologic analysis. RESULTS-CONCLUSION

Potential clinical role of fluorodeoxyglucose-positron emission tomography in assessing primary or secondary lymphomas of the parotid gland.

BACKGROUND: We have reviewed the utility of fluorodeoxyglucose-positron emission tomography (FDG-PET) imaging for clinical management of primary parotid lymphoma (by applying strict criteria of primary extranodal involvement by this disease) as well as of cases in which spread of the disease to the parotid gland had occurred as a consequence of primary nodal disease. PATIENTS AND METHODS: A total of 9 cases of parotid lymphoma (5 primary parotid lymphoma and 4 cases of combined nodal/extranodal lymphoma with parotid involvement) were identified and analyzed for this study. A retrospective rev

Parotid-sparing intensity-modulated radiotherapy (IMRT) for nasopharyngeal carcinoma: preserved parotid function after IMRT on quantitative salivary scintigraphy, and comparison with historical data after conventional radiotherapy.

PURPOSE: To evaluate the parotid function after parotid-sparing intensity-modulated radiotherapy (IMRT) in patients with nasopharyngeal carcinoma (NPC). METHODS AND MATERIALS: From March 2003 to May 2004, 16 patients with nonmetastatic NPC underwent parotid-sparing IMRT. Eight of these patients had Stage III or IV NPC based on the 1997 American Joint Committee on Cancer staging system. The post-IMRT parotid function was evaluated by quantitative salivary scintigraphy and represented by the maximal excretion ratio (MER) of the parotid gland after sialogogue stimulation. The parotid function of

Combined saphenous vein with free flap reconstruction in the treatment of carotid stenosis.

Revascularization for arterial stenosis in varied anatomic sites exposed to therapeutic radiation has been well described. In most circumstances, symptomatic end organ ischemia has been the indication for surgical intervention. We report the case of a patient who had symptomatic carotid stenosis 40 years after having ipsilateral neck dissection with radiation therapy. We did a saphenous vein graft interposition and free flap reconstruction of the overlying damaged skin.

[Facial motor lesion after surgery of the parotid gland].

The authors report a retrospective study of 351 parotidectomies observed during a 28 years period. The average follow-up is 28 months. Preoperative and post-operative facial nerve function, type of parotidectomy, surgical management of facial nerve and histologic diagnosis according to the revised WHO classification (1990) are presented. Type of parotidectomy and degree of tumor malignancy are statistically analyzed. It seems that occurrence of post-operative facial nerve dysfunction depends on radical parotidectomy, whereas long term dysfunction is determined by tumor malignancy. Malignant ep

[Aspects to the malignoma of the parotid gland and extratemporal facial nerve surgery (author's transl)].

Malignomas of the great salivary glands are--after own examinations--localized to ca. 90% in the parotid gland and ca. 10% in the sublingual gland. The frequency of different malignomas of the parotid gland, their age and sex incidence, pareses of the facial nerve before therapy, methods of therapy, relapses, letality and survival time are discussed. On own cases the method of extra-temporal reconstructive plastic surgery of the facial nerve is demonstrated.

Neck management in cutaneous squamous cell carcinoma with parotid metastasis.

OBJECTIVE: Our objective is to assess the oncologic outcomes of observation, elective neck dissection (END), and elective neck irradiation (ENI) in the neck management of head and neck cutaneous squamous cell carcinoma (HNcSCC) with parotid metastasis (P+) and to evaluate the quality of life (QoL) of patients who received END or ENI. METHODS: Patients with P+ HNcSCC were retrospectively enrolled. The impact of observation, END, and ENI on regional control (RC) and overall survival (OS) was analyzed using Cox proportional hazards model with presentation via hazard ratio (HR) with a 95% confide

Recurrent pleomorphic adenoma: results of surgical treatment.

BACKGROUND: Recurrent parotid pleomorphic adenoma surgery increases the risk of facial nerve injury, and there is also a risk of ulterior recurrence. METHODS: Postoperative results from 62 consecutive patients operated for recurrent pleomorphic adenoma were analyzed. It was the first recurrence for 49 patients (79%), the second or more for 13 patients (21%). RESULTS: Total parotidectomy was performed in 69.4% of cases. Skin resection was performed in 47 patients (75.8%). Resection of a facial nerve branch was performed in seven patients (11.3%). Pathologic examination findings revealed carci

Tumour-simulating squamous cell metaplasia (SCM) in necrotic areas of salivary gland tumours.

Squamous cell metaplasia (SCM) adjacent to necrotic areas of salivary gland tumours must be distinguished from other types of SCM (focal SCM in the excretory ducts of salivary glands; necrotizing sialometaplasia; focal SCM within salivary gland tumours) in respect to the tissue structure. Based on the high cellular proliferation, arcade- or cord-like pseudoneoplastic SCM develops with stellate extension in the surrounding tissue and focal inclusion of goblet cell metaplasia. This proliferative SCM resembles the cellular demarcation of radicular dental cysts. In the Salivary Gland Register 8 ca

Administration of chemotherapy in patients on dialysis.

The prevalence of patients on dialysis has increased and these patients present a challenge for chemotherapy administration when diagnosed with cancer. A consensus on the dosage and timing of different chemotherapeutic agents in relation to dialysis has not been established. We describe the pattern of care and treatment outcome for cancer patients on dialysis in our institution. The dataset from the Australia and New Zealand Dialysis and Transplant Registry of patients on dialysis who had a diagnosis of cancer was obtained and matched to the pharmacy records in our institution to identify pati

[Long term results in adenoidcystic carcinoma].

BACKGROUND: Due to the discreet initial symptoms and the locally aggressive infiltration with perineural spread the adenoidcystic carcinoma (ACC) presents a special diagnostical and therapeutical challenge. PATIENTS: : In a retrospective study the forms of 54 patients were analysed, whose average age was 55.5 (24 - 77) years. RESULTS: The sex ratio showed a slight female preponderance with 57 % to 43 %. The major salivary glands were affected in 26 cases. The exact distribution of the ACC was: parotid gland (n = 18), submandibular gland (n = 8), oral cavity (n = 10), paranasal sinuses (n = 1

Immunohistochemical localization of D-β-aspartic acid and periostin in benign parotid gland tumors.

Ocular adnexal lymphomas (OALs) are a heterogeneous group of malignant lymphoproliferative tumors originating from clonal proliferations of lymphocytes, with a multifactorial etiopathogenesis. They can be distinguished in primary, involving ocular adnexa, and secondary, affecting also an additional site. Pathologically OALs encompass four histological subtypes with different biological behaviour and prognosis. The diagnosis relies on clinical manifestations, imaging and histopathological examinations. Clinical symptoms are classified in ophthalmologic, often nonspecific, and constitutional, in

Adding automated decision-tree models to multiparametric MRI for parotid tumours improves clinical performance.

PURPOSE: Therapeutic management of parotid gland tumours depends on their histological type. To aid its characterisation, we sought to develop automated decision-tree models based on multiparametric magnetic resonance imaging (MRI) parameters and to evaluate their added diagnostic value compared with morphological sequences. METHODS: 206 MRIs from 206 patients with histologically proven parotid gland tumours were included from January 2009 to January 2018. Multiparametric MRI findings (including parameters derived from diffusion-weighted imaging [DWI] and dynamic contrast-enhanced [DCE]) were

Primary Parotid Non-Hodgkin's B-Cell Lymphoma in an Elderly Woman on Pembrolizumab for Metastatic Invasive Papillary Urothelial Cell Carcinoma of the Bladder.

Primary lymphomas of the parotid are rare (4-5%) and seldom appear in patients with pre-existing metastatic cancer from other primary sources. We present a primary marginal zone B-cell lymphoma of the mucosaassociated lymphoid tissue (MALT) in an 84-year-old female with preexisting metastatic bladder cancer. A PET scan that identified positive pelvic/cervical lymphadenopathy and bilateral parotid masses. She underwent transurethral resection of a bladder tumor and started on pembrolizumab chemotherapy. After two years, the left parotid mass decreased in size but the right increased to 3.9cm. R

Ossified Carcinoma Ex Pleomorphic Adenoma in accessory lobe of parotid gland: Complexity in clinical, imaging and histologic diagnosis and minimally invasive surgery.

Carcinoma ex pleomorphic adenoma is not common among malignant salivary tumors and an accessory lobe of parotid gland tumor with a carcinoma ex pleomorphic adenoma histology is even rarer. Management of these tumors include a high index of suspicion, good understanding of the anatomy and a meticulous surgical access. The exceptional localization of an ossified Carcinoma ex Pleomorphic Adenoma in the accessory lobe of the parotid gland, never reported to date in the literature, has prompted us to report this case to stress the challenging difficulties related to clinical and histologic diagnosi

An Anatomic Variant of the Ansa Cervicalis Precluding Its Use as a Donor Nerve.

BACKGROUND: The ansa cervicalis is useful for cranial nerve repair, and may be harvested without apparent morbidity. Herein we report an unusual and surgically relevant anatomic variant of the ansa cervicalis. METHODS: An adult male with left parotid adenoid cystic carcinoma underwent parotidectomy with upper-division facial nerve resection and planned interposition repair using the ansa cervicalis. The ipsilateral hypoglossal nerve was identified, together with a descending branch producing strap muscle contraction when stimulated. This presumed descendens hypoglossi was unusually large in c
