Cortical activity during the first 4 months after anterior cruciate ligament reconstruction while performing an active knee joint position sense test: A pilot study.

BACKGROUND: Anterior cruciate ligament (ACL) rupture is thought to alter the way in which the brain receives and processes information, affecting body movements. Although alterations in brain activity after ACL rupture have been described, these are limited to time points more than 6 months after rupture. Therefore, this pilot study aims to investigate cortical activity during an active knee joint position sense (JPS) test within the first 4 months after ACL reconstruction. METHODS: Twelve participants with ACL reconstruction (nine males; age 25.3 ± 6.4 years; height 173.6 ± 8.0 cm; mass 71.1

[The rubber hand illusion in patients with complex regional pain syndrome. Successful illusion induction shows multisensory integration].

Next to neurogenic inflammation and pathological sympathetic-afferent coupling, functional imaging studies have shown the crucial role of maladaptive cortical reorgansation in the pathophysiology of CRPS. Bilateral neuroplastic alterations in the somatosensory cortex seem to play a substantial role in the dysfunctional sensory processing of stimuli. The aim was to investigate the multimodal integration of sensory and visual stimuli into the body scheme and the influence of higher cognitive body representation in the integration of multimodal schema, body relevant stimuli in patients with CRPS.

Bilateral deficits in fine motor control and pinch grip force are not associated with electrodiagnostic findings in women with carpal tunnel syndrome.

OBJECTIVE: : The aim of this study was to analyze the differences in deficits in fine motor control and pinch grip force between patients with minimal, moderate/mild, or severe carpal tunnel syndrome (CTS) and healthy age- and hand dominance-matched controls. DESIGN: : A case-control study was conducted. The subtests of the Purdue Pegboard Test (one-hand and bilateral pin placements and assemblies) and pinch grip force between the thumb and the remaining four fingers of the hand were bilaterally evaluated in 66 women with minimal (n = 16), moderate (n = 16), or severe (n = 34) CTS and in 20 a

Stance performance under different sensorimotor conditions in patients with post-traumatic otolith disorders.

Dynamic posturography with the Equitest system is helpful to screen patients for balance deficits, but diagnostic specificity, for otolith disorders is unknown until now. It was therefore the aim of our present paper to examine patients with a well-defined otolith disorder on the Equitest at different test conditions while simultaneously recording trunk sway by means of the Sway Star system. A total of 22 patients with different types of otolith disorders were included in this study. All test results of the patients were matched with respect to age and gender to controls without history of ENT

Functional fatigue and upper extremity sensorimotor system acuity in baseball athletes.

CONTEXT: The sensorimotor system controls the balance between upper extremity stability and mobility during athletic performance. Research indicates that fatigue hampers sensorimotor system function; however, few investigators have studied functional fatigue or multijoint, multiplanar measures. OBJECTIVE: To examine the effect of functional fatigue on upper extremity position reproduction in overhead throwing athletes. DESIGN: Single-session, repeated-measures design. SETTING: University musculoskeletal laboratory. PATIENTS OR OTHER PARTICIPANTS: Sixteen healthy collegiate baseball players

Modulation of flexion reflex induced by hip angle changes in human spinal cord injury.

The flexion reflex can be elicited via stimulation of skin, muscle, and high-threshold afferents inducing a generalized flexion of the limb. In spinalized animal models this reflex is quite prominent and is strongly modulated by actions of hip proprioceptors. However, analogous actions on the flexion reflex in spinal cord injured (SCI) humans have not yet been examined. In this study, we investigated the effects of imposed static hip angle changes on the flexion reflex in ten motor incomplete SCI subjects when input from plantar cutaneous mechanoreceptors was also present. Flexion reflexes wer

Deficits and recovery of body stabilization during acrobatic locomotion after focal lesion to the somatosensory cortex: a kinematic analysis combined with cortical mapping.

We used a kinematic analysis for assessing locomotor impairments and evaluating the time course of recovery after focal injury to the forepaw area of the primary somatosensory cortex (SI) in rats. The animals were trained to traverse a beam that was rotated at various speeds. Changes in orientation of the body and independent movement of the anterior and posterior parts of the body were reconstructed using a 3D motion analysis. In addition, we used electrophysiological cortical mapping to search for neurophysiological changes within the spared cortical zones surrounding the lesion. Neuronal re

Podiatric manifestations in patients with acute and chronic neurologic disorders.

It is impossible to delineate, in the short space provided, the entire spectra of possible and potential podiatric manifestations of acute and chronic neurologic disease. An attempt has been made to delineate the neuroanatomic, neurophysiologic, etiologic, and diagnostic procedural knowledge required to fully comprehend, evaluate, and manage neurologic complaints in presenting patients. By following a specific outline for evaluation, a functional and meaningful differential diagnosis can be formulated. By properly employing the ancillary procedures discussed, a correct diagnosis will be attain

Heterologous contralateral repeated bout effect: eccentric knee extensor conditioning enhances neuromuscular fatigue resistance and attenuates muscle stiffness in contralateral elbow flexors in young women.

BACKGROUND: The repeated bout effect (RBE) protects skeletal muscles from exercise-induced muscle damage (EIMD). While homologous contralateral RBE is well-documented, the existence of heterologous RBE-specifically whether lower limb conditioning can protect upper limb muscles-remains debated. PURPOSE: This study investigated whether high-intensity eccentric exercise of the knee extensors attenuates eccentric exercise-induced neuromuscular fatigue and muscle stiffness and preserves neuromuscular control in the contralateral elbow flexors. METHODS: Twenty-four sedentary young women were rando

Wearable Multi-Sensor Fusion for Field Assessment of Dynamic Postural Stability During the Curling Delivery in Young Adult Curlers: Integrating Trunk Kinematics, Plantar Pressure and Core-Muscle Activation.

Postural control in curling is expressed during a dynamic, sport-specific delivery on a single sliding leg, yet it is still assessed almost exclusively with static single-leg force-plate protocols that poorly reflect on-ice demands. This study developed and validated a wearable multi-sensor system that fuses trunk and pelvis inertial measurement units (IMUs), instrumented plantar-pressure insoles and core-muscle surface electromyography (sEMG) to quantify dynamic postural stability during the simulated curling delivery in 110 young adult curlers (age range 18-24 years). FaReWell Depression - a randomized controlled trial of a physiotherapeutic program for the facial rehabilitation of wellbeing in depression.

INTRODUCTION: Facial feedback mechanisms can modulate affective states. A series of clinical trials have shown that injection of botulinum toxin A can improve the symptoms of depression, probably via the disruption of proprioceptive afferences from the glabellar region to the brain. In extension of this research, we developed a physiotherapeutic program for the facial rehabilitation of wellbeing in depression (FaReWell Depression) with the objective to relax key facial muscles involved in the expression and experience of negative emotions and to tone and strengthen key facial muscles for the e

Reorganization of Human Gait and Foot Pressure Patterns After 1-Week Dry Immersion.

INTRODUCTION: Dry immersion (DI) is a ground-based model of simulated weightlessness that reduces support and proprioceptive input. This model produces reversible sensorimotor deconditioning and allows simulation of the early acute readaptation phase after spaceflight. We aimed to determine changes in spatiotemporal gait parameters and shin muscle work resulting from 7 d of DI, and whether plantar pressure distribution shifts accompany these changes. METHODS: A total of 28 healthy males (14 DI, 14 controls) completed overground walking tests twice pre-DI and 2-3 h post-DI at first standing. S

Neuromuscular training for ACL injury prevention in collegiate athletes: a four-week program improves biomechanical risk factors and functional performance.

BACKGROUND: Collegiate athletes face a high risk of non-contact ACL injuries during pivoting and landing maneuvers. Current evidence supports neuromuscular training for injury prevention, but optimal program duration and specific adaptations remain unclear. This study investigated whether a four-week neuromuscular training program could enhance functional performance and modifiable biomechanical risk factors in high-risk athletes. METHODS: This single-group pre-/post-test design recruited 30 competitive collegiate athletes (median age: 18.5 years; interquartile range [IQR]: 17-25) who complet

The Impact of Utilizing a Balancing Blindfold During Training on the Backward Running Technique in Experienced and Novice Male Handball Players.

Backward running is common in handball defense and relies heavily on proprioceptive control when visual information is limited. Twenty-eight male handball players were allocated to three groups: experimental novice group with blindfold training (n = 7), control novice group with the same training without blindfold (n = 7), and target professional group (n = 14). Both novice groups completed a 6-week balance program (3 × 20 min/week). Lower-limb kinematics during backward running were captured with a 6-camera motion analysis system, and inter-joint coordination was quantified by Mean Absolute R

The Role of Stabilization Exercise in Preventing Pain and Postural Defects in Young Football Players.

Background/Objectives: Maintaining proper posture and preventing musculoskeletal pain are essential for the healthy development of young football players. Contemporary concepts of postural control emphasize the importance of the lumbopelvic-hip complex and the activation of deep trunk muscles. This study aimed to evaluate the effects of a structured core stabilization training program on postural parameters and pain reduction in young football players. Methods: A total of 182 male football players, aged 9-15 years, were enrolled and allocated to either the intervention or control group. The 12

Optimizing head movement patterns to maximally modulate CSF flow by myodural bridge complex.

The myodural bridge complex (MDBC) is a tendon-like structure highly conserved during vertebrate evolution, suggesting it plays an important physiological role. Substantial evidence indicates that the MDBC may contribute to cerebrospinal fluid (CSF) circulation by generating mechanical force. However, the proprioceptive innervation within the MDBC remains poorly characterized. This study aimed to systematically identify the types of sensory nerve endings within the MDBC, thereby providing a neuroanatomical basis for optimizing mechanostimulation and developing new therapeutic strategies for ne

Anterior Cruciate Ligament (ACL) Repair Using Dynamic Intraligamentary Stabilization Combined with Lateral Extra-articular Augmentation: A Case Report and Technical Note.

The treatment options for an anterior cruciate ligament (ACL) rupture range from conservative therapy to ACL repair and reconstruction. ACL repair is particularly suitable for younger patients with an acute proximal tear, and moderate athletic demand. Preserving the ACL can restore its proprioceptive and stabilizing functions, avoid donor site morbidity, and shorten rehabilitation time. Repair techniques include the use of suture anchors, internal brace augmentation, and dynamic intraligamentary stabilization. Dynamic intraligamentary stabilization employs a coil spring mechanism for dynamic t

Modulations of stretch reflex by altering visuomotor contexts.

Various functional modulations of the stretch reflex help to stabilize actions, but the computational mechanism behind its context-dependent tuning remains unclear. While many studies have demonstrated that motor contexts associated with the task goal cause functional modulation of the stretch reflex of upper limbs, it is not well understood how visual contexts independent of the task requirements affect the stretch reflex. To explore this issue, we conducted two experiments testing 20 healthy human participants (age range 20-45, average 31.3 ± 9.0), in which visual contexts were manipulated i
