A randomized trial of one versus three hyperbaric oxygen sessions for acute carbon monoxide poisoning.

INTRODUCTION: Hyperbaric oxygen (HBO₂) improves outcome in patients with acute carbon monoxide (CO) poisoning, but optimal dose/timing are unknown. In this double-blind, sham-controlled randomized trial, we compared neuropsychological sequelae at six weeks and six months in patients receiving three HBO₂ sessions or one HBO₂ session and two sham chamber sessions after acute CO poisoning. METHODS: After completing one HBO₂ session (3.0 ATA, 60 minutes, 2.0 ATA, 65 minutes), CO-poisoned patients were randomized (1:1): two sham chamber sessions (1 ATA air, 120 minutes) or two additional HBO₂sessi

Early complications in patients with multiple injuries and polytraumatism with special regard to traumatic fat embolism.

In the introductory part contemporary data concerning fat embolism occurrence and mortality, as taken from the world literature, are evaluated. Thus, the author opens the whole complicated problem of post-mortem examination findings with a different extent of fat embolization (most often in the lungs, rarely in organs belonging to the area supplied by the systemic blood circulation) on the one hand and the fat embolism clinical syndrome on the other hand. The clinical syndrome is a rare phenomenon in comparison with the relatively frequent morphological abduction findings of a more serious cha

Patterns of Trauma Presentation in Ilorin, Nigeria: A 15-Year Review.

BACKGROUND: Injuries make up a significant portion of the emergency surgical patient load in most hospitals, representing a major public health threat The burden of injury as a public health issue is especially pronounced in low- and middle-income countries, where injuries are responsible for up to 90% of mortality. Identifying common mechanisms of trauma and patterns of presentation can aid in identifying at-risk populations and in the development of targeted preventive protocols. METHODS: From February 2000 to January 2015, a prospective observational study was conducted to examine the patt

A retrospective epidemiological review of maxillofacial injuries in a tertiary care centre in Goa, India.

PURPOSE: Trauma accounts for the leading cause of morbidity and mortality worldwide in the present day and may rightly be called the new pandemic. The prominent nature of the face exposes it to various traumatic injuries. A timely, prompt diagnosis along with employment of correct and quick treatment greatly improves the outcome for these patients. The aim of this retrospective study was to analyse the characteristics of maxillofacial injuries over a decade. METHODS: The data were collected manually from the medical records of patients who reported to the tertiary centre from 1 January 2011 t

The epidemiology of wrist fractures in older men: the Osteoporotic Fractures in Men (MrOS) study.

UNLABELLED: There is limited wrist fracture information on men. Our goal was to calculate frequency and identify risk factors for wrist fracture in the Osteoporotic Fractures in Men (MrOS) study. We confirmed that fracture history and certain medications are predictors, and identified novel predictors including markers of kidney function and physical performance. INTRODUCTION: To calculate the incidence of wrist fractures and their risk factors in older community-dwelling men from the US Osteoporotic Fractures in Men (MrOS) study. METHODS: Using triannual postcards, we identified incident wr

ORIF Patella Fracture With a Tension Band Construct.

Patellar fractures can occur as a result of direct or indirect injuries. Indirect injury is more common and occurs when the patella fails in tension when the force exerted by the extensor mechanism exceeds that of the bone. Common mechanism of indirect injury is rapid knee flexion with fully contracted quadriceps muscle. Treatment with a tension band construct is indicated for transverse patella fractures without significant comminution. This can be performed with 18-gauge wire and either 2 parallel, longitudinally oriented cannulated screws or 2 Kirschner wires. Biomechanical studies have sho

Change in child mortality patterns after injuries in Sweden: a nationwide 14-year study.

INTRODUCTION: Sweden has one of the world's lowest child injury mortality rates, but injuries are still the leading cause of death among children. Child injury mortality in the country has been declining, but this decline seems to decrease recently. Our objective was therefore to further examine changes in the mortality of children's death from injury over time and to assess the contribution of various effects on mortality. The underlying hypothesis for this investigation is that the incidence of lethal injuries in children, still is decreasing and that this may be sex specific. PATIENTS AND

The Low Fall as a Surrogate Marker of Frailty Predicts Long-Term Mortality in Older Trauma Patients.

BACKGROUND: Frailty is associated with adverse outcomes including disability, mortality and risk of falls. Trauma registries capture a broad range of injuries. However, frail patients who fall comprise a large proportion of the injuries occurring in ageing populations and are likely to have different outcomes compared to non-frail injured patients. The effect of frail fallers on mortality is under-explored but potentially significant. Currently, many trauma registries define low falls as less than three metres, a height that is likely to include non-frailty falls. We hypothesized that the low

Implementation of an all-ages mandatory helmet policy for ice skating.

Ice skaters sustain a significant number of head injuries each winter. We are the first to implement an all-ages helmet policy at a university-based Canadian arena. We report our experience from a cross-sectional observational study as well as the policy's consequences on helmet use and skating participation. Educational programming was provided prior to policy implementation. Observations of helmet use, falls and skater demographics were conducted prior to education/implementation and after policy implementation. The number of skaters observed was essentially unchanged by the policy; 361 skat

Role of cervical spine radiography in the initial evaluation of stable high-energy blunt trauma patients.

BACKGROUND: Detecting cervical spine injuries in trauma patients is essential because undetected injuries in the this area may result in severe neurological disability and probably quadriplegia. Thus, radiography of the cervical spine is considered mandatory in the initial evaluation of trauma patients according to Advanced Trauma Life Support. This approach results in many unnecessary normal radiographs. Therefore, we performed this study to determine the role of routine cervical radiography in the initial evaluation of stable high-energy blunt trauma patients. METHODS: This was a prospectiv

Individual and area socioeconomic inequalities in cause-specific unintentional injury mortality: 11-year follow-up study of 2.7 million Canadians.

This study investigated the association between individual and area socioeconomic status (SES) and leading causes of unintentional injury mortality in Canadian adults. Using the 1991-2001 Canadian Census Mortality Follow-up Study cohort (N=2,735,152), Cox proportional hazard regression was used to calculate hazard ratios and 95% confidence intervals for all-cause unintentional injury, motor vehicle collision (MVC), fall, poisoning, suffocation, fire/burn, and drowning deaths. Results indicated that associations with SES differed by cause of injury, and were generally more pronounced for males.
