Epidemiology and frequency of resistance among pathogens causing urinary tract infections in 1,510 hospitalized patients: a report from the SENTRY Antimicrobial Surveillance Program (North America).

Bacterial urinary tract infections (UTIs) are an important cause of septicemia resulting in high mortality rates, prolonged hospital stays and increased healthcare costs. Periodic reviews of pathogen frequency and susceptibility patterns impact on appropriate antimicrobial usage, leading to more effective prescribing practices. As part of the SENTRY Antimicrobial Surveillance Program (SENTRY, 1998), participants collected 50 consecutive UTI pathogens from patients hospitalized in 31 medical centers (26 in the United States and five in Canada) and forwarded subcultures to the coordinating cente

Antimicrobial resistance patterns and difficult-to-treat resistance in uropathogens among diabetic and non-diabetic patients: a cross-sectional study.

BACKGROUND: While antimicrobial resistance (AMR) is a global challenge, data comparing Difficult-to-Treat Resistance (DTR) in diabetic versus non-diabetic urinary tract infection (UTI) patients is scarce. This study uniquely evaluates DTR and first-line resistance patterns to inform targeted empirical treatment strategies for these distinct patient cohorts. METHODS: A prospective observational cross-sectional study was conducted from August 2021 to January 2024, at a tertiary care teaching hospital in Vadodara, Gujarat, India. During the period, 2408 patients with symptoms of UTI were screene

Antibiotic Resistance Profiling and Identification of Risk Factors Associated With Prevalence of Urinary Tract Infections: A Cross-Sectional Study.

Urinary tract infections (UTIs) are among the most common and serious global health concerns. The alarming rise of antibiotic resistance levels in uropathogens has further complicated the situation. This study aimed to determine the epidemiology, antibiotic resistance pattern, and associated risk factors of clinical isolates from patients suspected to have UTIs. A cross-sectional study was conducted at a tertiary-care hospital in North Kashmir, India, from June to December 2024. The samples were collected from the clinically suspected UTI patients and processed through standard procedures. Out

Efficacy and in vitro activity of gepotidacin against bacterial uropathogens, including subsets with molecularly characterized resistance mechanisms and genotypes/epidemiological clones, in females with uncomplicated urinary tract infections: results from two global, pivotal, phase 3 trials (EAGLE-2 and EAGLE-3).

Gepotidacin, a novel, bactericidal, first-in-class triazaacenaphthylene antibacterial, was noninferior to nitrofurantoin in two pivotal trials (EAGLE-2 and EAGLE-3) in females with uncomplicated urinary tract infections (uUTIs). Using pooled data, gepotidacin in vitro activity and clinical efficacy were evaluated for subsets of molecularly characterized isolates in the microbiological Intent-to-Treat population. The subsets of isolates were characterized based on phenotypic/MIC criteria; all microbiological failure isolates were also characterized. Of 1,159 Escherichia coli isolates, 30% harbo

Intersecting social and environmental determinants of multidrug-resistant urinary tract infections in East Africa beyond antibiotic use.

The global health crisis of antibacterial resistance (ABR) poses a particular threat in low-resource settings like East Africa. Interventions for ABR typically target antibiotic use, overlooking the wider set of factors which drive vulnerability and behaviours. In this cross-sectional study, we investigated the joint contribution of behavioural, environmental, socioeconomic, and demographic factors associated with higher risk of multi-drug resistant urinary tract infections (MDR UTIs) in Kenya, Tanzania, and Uganda. We sampled outpatients with UTI symptoms in healthcare facilities and linked t

Comparison of intravenous and oral definitive antibiotic regimens in hospitalised patients with Gram-negative bacteraemia from a urinary tract infection.

OBJECTIVES: Transitioning patients from intravenous (IV) to oral antibiotic therapy has been shown to be a successful approach for several infections. However, minimal data exist evaluating outcomes following transition from to oral antibiotics for patients with bacteraemia secondary to a urinary tract infection (UTI). This study compared treatment failures between patients treated exclusively with IV antibiotics and those transitioned from IV to oral antibiotics for bacteraemia secondary to UTI. METHODS: This single-centre, retrospective cohort study included hospitalised, non-critically ill

A case of toxic epidermal necrolysis caused by trimethoprim-sulfamethoxazole.

Toxic epidermal necrolysis (TEN) is a rare but serious dermatological emergency characterised by diffuse exfoliation of the skin and mucous membranes due to immune mediated destruction of the epidermis which can lead to sepsis and respiratory distress. Trimethoprim-sulfamethoxazole is a widely used antibiotic which can rarely lead to TEN. Early diagnosis and aggressive medical care is essential for the reduction of high morbidity and mortality associated with this disease. We present a case of successfully recovered TEN due to trimethoprim-sulfamethoxazole in a 62-year-old woman.

Clinical and bacteriological effects of therapy of urinary tract infection in primary health care: relation to in vitro sensitivity testing.

17 primary health care (PHC) centres participated in a 1-month evaluation of a county urinary tract infection (UTI) management program. A total of 302 patients contributing 355 episodes, dominated by female (93%), lower symptomatic (75%) and Escherichia coli (74%) episodes were studied. In therapeutic failure gram-negative bacteria other than E. coli showed an increased prevalence whereas Staphylococcus saprophyticus was not found. The general pattern of drug resistance was little influenced by UTI history and the mean pretherapy prevalence of resistance to the 7 antibacterial agents studied w

Antibiogram and molecular characterization of multi-drug resistant microorganisms isolated from urinary tract infections.

Bacteria are the commonest etiological factor among the microbes that cause UTIs. The most prevalent bacteria identified in the lab are Escherichia coli, Klebsiella pneumonia and Pseudomonas aeruginosa. Antibiotics are the empiric therapy for such infections but the reoccurrence rate is becoming high owing to the development of resistance due to their irrational and indiscriminate use across the globe. This study was designed on UTI cases of OPD, Medical, Nephrology, Surgical, Main OT, Urology and ICU wards of Allied hospital Faisalabad. 11 antibiotics were used which showed that E. coli is se

Rising Burden of ESBL-Producing Escherichia coli and Klebsiella pneumoniae in Burkina Faso: A Five-Year Retrospective Analysis.

Extended-spectrum β-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae are major contributors to antimicrobial resistance worldwide. Data from Burkina Faso remain limited, despite growing concerns about multidrug resistance in urinary pathogens. We conducted a retrospective cross-sectional study using microbiological records from three medical laboratories in Ouagadougou between January 2018 and December 2022. All urine culture isolates of E. coli and K. pneumoniae were included. Patient demographics, hospitalization status, and antimicrobial susceptibility profiles were ext
