BACKGROUND: Small airway dysfunction (SAD) is common in pediatric asthma and is associated with poor disease control and frequent exacerbations. Current therapies show limited efficacy in improving small airway function. Dupilumab suppresses type 2 inflammation and may provide benefit; however, real-world evidence in Chinese children is limited. This study evaluated the association between dupilumab and improvements in SAD in children with uncontrolled moderate-to-severe asthma.
METHODS: This retrospective cohort study without a concurrent comparator group included 106 children (aged 6-14 yea

OBJECTIVE: Tobacco use is a leading cause of morbidity and mortality worldwide. Complementary therapies, such as auriculotherapy, are increasingly being explored for smoking cessation; however, inconsistent administration methods have limited definitive evidence. This study evaluated whether a single auriculotherapy session promotes smoking cessation.
METHODS: In this multicenter, randomized, single-blind, superiority clinical trial conducted at a tertiary care university hospital and two private practices in France, we enrolled cigarette smokers aged ≥18 years who were motivated to quit. Par

BACKGROUND: The diagnosis of cough-variant asthma (CVA) traditionally relies on clinical manifestations and bronchial provocation test (BPT). However, the BPT can be costly and complex, and poses risks of severe bronchospasm. Most CVA patients exhibit type 2 airway inflammation and small airway dysfunction. Identifying practical biomarkers such as total immunoglobulin E (tIgE), percentage of eosinophil cell count in induced sputum (sEOS%), peripheral blood eosinophil count (PBEC), and fractional exhaled nitric oxide (FeNO) may enhance the diagnostic process for cough-variant asthma.

Respiratory medicine is transitioning from symptom-driven, standardized care to a more precise, patient-specific approach guided by molecular profiling. This evolution is being enabled by advances in liquid biopsy, multiomics, and artificial intelligence (AI) analytics. Fractional exhaled nitric oxide (FeNO) and blood eosinophils, the two commonly used markers in asthma, are now being joined by more precise airway markers such as galectin-10, which could aid clinicians in making more informed decisions for biological treatments. In chronic obstructive pulmonary disease (COPD) similar progress

Malaria remains a major global health challenge. Prompt, accurate diagnosis is crucial for effective case management. Current diagnostic approaches rely on invasive or minimally invasive sampling via venous or fingerpick blood draw, posing a risk to healthcare workers via the handling of potentially infectious body fluids. They may also be a barrier to recipients, particularly in malaria-endemic areas where there is routine testing of non-symptomatic individuals. Non-invasive tests based on saliva, exhaled volatile organic compounds (VOCs), and transdermal detection have the potential to incre

Asthma experienced by both adults and children is a phenotypically heterogeneous condition. Severe asthma, characterized by ongoing symptoms and airway inflammation despite high doses of inhaled and/or systemic corticosteroids, is the focus of research efforts to understand this underlying heterogeneity. Clinical phenotypes in both adult and pediatric asthma have been determined using supervised definition-driven classification and unsupervised data-driven clustering methods. Efforts to understand the underlying inflammatory patterns of severe asthma have led to the seminal discovery of type 2

House dust mites (HDM) are the world's most important cause of allergic asthma. It is unclear why some patients with HDM allergy develop an early asthmatic reaction (EAR) only, whereas others react with a dual asthmatic reaction-EAR plus late asthmatic reaction (LAR). In patients with LAR, the symptoms and bronchial inflammation are more severe, and the current knowledge suggests that the EAR always precedes the LAR. The aim of the present study was to investigate whether a LAR can occur separately even without a significant EAR. In a pilot study of 20 patients with asthma and HDM allergy, a b

OBJECTIVES: The Breathing for Life Trial (BLT) was a multicentre randomised controlled trial testing the hypothesis that a fractional exhaled nitric oxide-based intervention to guide asthma therapy in pregnancy improves perinatal outcomes. While BLT was negative based on selected outcomes, the conduct of the trial over 7 years showed potential for assessing the broader research impacts and returns on investment in BLT. The aim of this study was to retrospectively assess and report on the impact and value of BLT to show accountability for the research investment in what was deemed a 'negative'

Background: Lung volume recruitment (LVR) is a stacked-breath assisted inflation technique in which consecutive insufflations are delivered, without exhaling in between, until the maximum tolerable inflation capacity is reached. Although LVR is recommended in some neuromuscular disease guidelines, there is little information detailing when and how allied health professionals (AHPs) prescribe LVR. Objective: This study is aimed at describing the use of LVR in practice across Brazil. Methods: A cross-sectional e-survey (Sep-Nov 2023) explored LVR practices among qualified clinical or home care A
