Microscopic antrostomies in children: a review of the literature in chronic sinusitis and a plan of medical and surgical treatment.

Potential complications, morbidity, treatment failures and "nasal cripples" make one cautious about advising radical sinus surgery. Though antibiotics have greatly reduced the need for radical surgery, some patients do not respond to conservative treatment. Reviewed is pertinent literature concerning treatment of chronic, refractory sinusitis in children, particularly intranasal antrostomy. Medical treatment for chronic sinusitis precedes surgical intervention. Allergic history, nasal cytology and radiographic examination are essential. Since principles of adequate surgical drainage and ventil

Small ethmoid mucocele with intracranial extension associated with fibrous dysplasia: A rare case report.

Ethmoid mucoceles are expansile, benign cystic lesions characterized by slow growth, predominantly affecting the frontal sinus, followed by the ethmoid sinus, and infrequently the sphenoid and maxillary sinuses. Intracranial involvement is rare and is typically associated with large or longstanding lesions. We present a case involving a 34-year-old immunocompetent woman who experienced non-specific facial pain subsequent to an influenza infection without exhibiting typical sinonasal or neurological symptoms. Computed tomography and magnetic resonance imaging revealed a small ethmoid sinus lesi

Maxillary Sinus Floor Augmentation with Two Different Inorganic Bovine Bone Grafts: an Experimental Study in Rabbits.

OBJECTIVE: To compare the sequential healing of maxillary sinuses grafted with two different xenogeneic bone substitutes processed at either a low (300°C) or high (1200°C) temperature. METHODS: A sinus augmentation procedure was performed bilaterally in 20 rabbits and two different xenogeneic bone grafts were randomly used to fill the elevated spaces. Healing was studied after 2 and 10 weeks, in 10 rabbits during each period. RESULTS: After 2 weeks of healing, very small amounts of new bone were observed in both groups, and were mainly confined to close to the sinus bone walls and osteotomy

Leonardo da Vinci and dental anatomy.

Leonardo da Vinci, the Renaissance polymath, is still recognized today-above all for his oil paintings and mechanical inventions. His anatomical studies have attracted less attention, even though he devoted over 30 years of his life to them. This paper outlines Leonardo's career and research methods and focuses on the importance of his medical images for anatomical research and teaching. Following a short presentation of the state of (dental) medicine in the early Renaissance period, it offers a description of five of his cranial drawings that show the anatomy of the teeth, the nervous and vas

Parathyroid Hormone (1-34) Might Not Improve Early Bone Healing after Sinus Augmentation in Healthy Rabbits.

Purpose. This study evaluated the effect of administering intermittent parathyroid hormone [PTH (1-34), henceforth PTH] on the early-stage bone healing of maxillary sinus augmentation in healthy rabbits. Materials and Methods. Bovine bone mineral was grafted on the sinuses of 20 female New Zealand white rabbits. The animals were randomly divided into two groups, PTH (n = 10) or saline (n = 10), in which either PTH or saline was injected subcutaneously 5 days a week for 2 weeks. Half of the animals in each group were killed at 2 weeks postoperatively and the other half were killed at 4 weeks po

Primary Ewing's Sarcoma of the Sinonasal Tract: A Case Report.

A 23-year-old male presented with a 3-month history of left purulent rhinorrhea, progressive nasal obstruction, and intermittent epistaxis. A fiberoptic examination revealed a large vascular polypoid mass completely filling the left nasal cavity. CT and MRI scans showed a large hypervascular mass involving the left nasal airway, maxillary antrum, and the anterior ethmoid cells. There was no bony erosion or contiguous spread, and the remaining sinuses, orbit, and cranial fossa were uninvolved. The patient underwent complete removal of the mass via an external lateral rhinotomy approach. The sof

Computed tomography of Paget disease of the skull versus fibrous dysplasia.

OBJECTIVE: Radiologists are often challenged to review CT examinations of the skull without pertinent clinical information or plain radiographs. Skull lesions of fibrous dysplasia (FD) may often be confused with Paget disease (PD). The purpose of this article is to evaluate radiographic similarities and to find the signs that can differentiate PD from FD of the skull on head CT and to describe the CT imaging features of PD and FD. DESIGN AND PATIENTS: CT scans of the skull in eight cases of PD, 18 cases of FD (13 cases of skull and facial bones, five cases of only facial bones) and 10 normals

Diagnosis of malignant lymphomas of the nasal cavity, paranasal sinuses and nasopharynx.

The diagnosis of non-Hodgkin's malignant lymphoma of the nasal cavity, paranasal sinuses, and nasopharynx was made in 15 patients over a 7 year period (nasopharynx, 9; nasal cavity, 3; maxillary sinus, 2; frontal sinus, 1). A wide variety of head and neck symptoms, often characteristic of benign disease, was reported ranging from 2 weeks to 4 months prior to presentation. Of the 15 cases, the original diagnosis was inconclusive in 6; 4 of the 6 required rebiopsy, while the diagnosis in the other 2 was confirmed on further pathologic consultation. Tissue marker studies, which have recently beco

Comparison of swabs versus suction traps for endoscopically guided sinus cultures.

BACKGROUND: Knowledge of the causative organism(s) in bacterial rhinosinusitis has become the cornerstone of adequate medical and surgical management. Little uniformity and data exist for the best method of obtaining sinus cultures. Most otolaryngologists tend to use the nasal swab for obtaining transnasal middle meatal cultures. A prospective study was carried out to compare the effectiveness of standard nasal swabs versus suction traps in obtaining bacterial isolates under endoscopic guidance. METHODS: Fifty-two patients with purulence in the middle meatus or frontal recess were included in
