Urinary tract infection caused by extended-spectrum β-lactamase-producing bacteria: Risk factors and antibiotic resistance.

BACKGROUND: The aim of this study was to investigate the risk factors for acquisition of extended spectrum β-lactamase (ESBL)-producing bacteria in community-acquired urinary tract infection (UTI) and to evaluate their antimicrobial resistance. METHODS: The medical records of hospitalized children were retrospectively evaluated. Children with ESBL-producing bacteria UTI were matched with controls with non-ESBL-producing bacteria UTI of the same age and gender. RESULTS: A total of 243 patients with community-acquired UTI in a 5 year period were evaluated, of whom 46% had UTI caused by ESBL ba

Occurrence of multidrug resistance to oral antibiotics among Escherichia coli urine isolates from outpatient departments in Germany: extended-spectrum β-lactamases and the role of fosfomycin.

The in vitro activities of fosfomycin and seven other antibiotics commonly used for oral treatment of urinary tract infections (UTIs) were evaluated for 499 Escherichia coli isolated from urine samples during a nationwide laboratory-based surveillance study in 2010. Overall, the highest resistance rates were found for amoxicillin (42.9%), followed by amoxicillin/clavulanic acid (32.7%), trimethoprim/sulfamethoxazole (SXT) (30.9%), ciprofloxacin (19.8%), cefuroxime (10.0%), cefpodoxime (8.6%) and cefixime (8.2%). One-half of the isolates (n=252; 50.5%) were fully susceptible to the eight drugs,

Bacteria Associated With Urinary Tract Infections, Antimicrobial Resistance Profiles, and Associated Factors Among Diabetes Mellitus Patients at Debre Tabor Comprehensive Specialized Hospital, Northwest Ethiopia.

BACKGROUND: Patients with diabetes mellitus are at increased risk of urinary tract infections (UTIs) due to impaired immune function and metabolic changes, which may lead to serious complications and increased healthcare burdens. In the catchment area of Debre Tabor, northwest Ethiopia, data on UTI-causing bacteria and their antimicrobial resistance profiles among diabetic patients remain limited. Given the rising threat of antimicrobial resistance, continuous local surveillance is essential for effective management and antibiotic stewardship. This study is aimed at determining the bacterial e

Bacterial profile and antimicrobial susceptibility pattern of urinary tract infection among diabetic patients at Bule Hora University Teaching Hospital, Southern Ethiopia.

Urinary tract infections (UTIs) are among the most common infectious diseases in both hospital and community settings, leading to increased morbidity and mortality, particularly in patients with DM due to impaired immune function and hyperglycemia. The rise of antimicrobial resistance (AMR) further complicates the treatment of bacterial uropathogens, presenting a significant public health challenge. Thus, this study aimed to assess the magnitude, antimicrobial resistance patterns, and risk factors of bacterial UTIs in DM patients at Bule Hora University Teaching Hospital, Southern Ethiopia, 20

Development and implementation of a clinical decision support tool for treatment of uncomplicated urinary tract infections in a family medicine resident clinic.

OBJECTIVES: Outpatient antimicrobial stewardship has become increasingly important. While clinical decision support (CDS) tools have been effective in improving guideline-directed antibiotic prescribing, most notably for upper respiratory tract infections, their use for uncomplicated urinary tract infections (UTIs) has been less extensively studied. The objective of this study was to develop and implement a CDS tool to optimize antimicrobial prescribing for uncomplicated UTIs. SETTING: University-affiliated family medicine resident clinic. PRACTICE DESCRIPTION: This outpatient clinic is the

Increased adherence to treatment guidelines in patients with urinary tract infection in primary care: A retrospective study.

BACKGROUND: Urinary tract infection (UTI) is common in primary care and leads to a high number of antibiotic prescriptions. Antimicrobial resistance is a global health problem; better antimicrobial prescribing is one way to limit antimicrobial resistance. We aimed to describe the number of consultations for patients diagnosed with lower urinary tract infection (LUTI) and pyelonephritis and changes in prescribing of antibiotics to men and women with LUTI and pyelonephritis in Swedish PHC between the years 2008 and 2013. METHODS: We performed a descriptive study of changes in UTI diagnosis and

[THE RISK FACTORS AND CHEMOPREVENTION OF FEBRILE URINARY TRACT INFECTION AFTER TRANSURETHRAL URETEROLITHOTRIPSY].

(Objectives) Transurethral Ureterolithotripsy (TUL) has become an increasingly more common treatment for ureteric stones since the reduction in ureteroscope diameter and other device improvements. At the same time, TUL sometimes shows postoperative febrile urinary tract infection (fUTI) with severe complications. Therefore we investigated the occurrence and risk factors of fUTI in our hospital, and assessed the effect of antibiotic prophylaxis prior to TUL. (Materials and methods) The subjects were 260 patients who underwent TUL in our department during the period from January 2011 to October

Inadequate adherence to Swedish guidelines for uncomplicated lower urinary tract infections among adults in general practice.

In a primary care study of urinary tract infections (UTIs) performed 2008 in Uppsala County, Sweden, 43% of the patients were culture negative. In order to investigate the background to the observed overdiagnosis of UTI, study invitations were sent to the previously included patients. A total of 256 patients (88% women) approved to participate. Patient charts and recorded laboratory data were reviewed. Two or more of the cardinal symptoms were reported in 53% of the women and in 19% of the men. A dipstick test was performed in 93% of the consultations. The highest positive predicted values in

Exogenous alanine and/or glucose plus kanamycin kills antibiotic-resistant bacteria.

Multidrug-resistant bacteria are an increasingly serious threat to human and animal health. However, novel drugs that can manage infections by multidrug-resistant bacteria have proved elusive. Here we show that glucose and alanine abundances are greatly suppressed in kanamycin-resistant Edwardsiella tarda by GC-MS-based metabolomics. Exogenous alanine or glucose restores susceptibility of multidrug-resistant E. tarda to killing by kanamycin, demonstrating an approach to killing multidrug-resistant bacteria. The mechanism underlying this approach is that exogenous glucose or alanine promotes th

Susceptibility to acute pyelonephritis or asymptomatic bacteriuria: host-pathogen interaction in urinary tract infections.

Our knowledge of the molecular mechanisms of urinary tract infection (UTI) pathogenesis has advanced greatly in recent years. In this review, we provide a general background of UTI pathogenesis, followed by an update on the mechanisms of UTI susceptibility, with a particular focus on genetic variation affecting innate immunity. The innate immune response of the host is critically important in the antibacterial defence mechanisms of the urinary tract, and bacterial clearance normally proceeds without sequelae. However, slight dysfunctions in these mechanisms may result in acute disease and tiss

Management of complicated urinary tract infections in the era of antimicrobial resistance.

Complicated urinary tract infections (cUTIs) are a major cause of hospital admissions and are associated with significant morbidity and health care costs. Patients presenting with a suspected UTI should be screened for the presence of complicating factors, such as anatomic and functional abnormalities of the genitourinary tract. In the setting of cUTIs, the etiology and susceptibility of the causative organism is not predictable; therefore, when infection is suspected, patients should undergo a urinalysis in addition to culture and sensitivity testing. Although not warranted in all cases of co
