[Abscess of the skull base--therapy and reconstruction after zygoma resection].

Abscesses of the base of skull, if not correctly managed, can lead to the intracranial spread of infection - a life-threatening complication. Infection can spread from the upper and lower molar teeth through the retromaxillary area or the parapharyngeal space to reach the temporal, infratemporal or pterygopalatine fossa. Our recommended treatment of abscesses in these fossae is wide abscess incision together with resection of the midsection of the zygomatic arch ("Partsch method"). Two case reports are included. The first report is of a patient with cerebral abscess caused by an oral infection

[ZNS complications in children with acute lymphatic leukemia. Computer tomographic studies].

78 children with acute lymphoblastic leukemia or non-Hodgkin-lymphoma were treated at the Children's Hospital of the University of Heidelberg from December 1971 to April 1979. Following cytostatic treatment and irradiation of the skull 11 children developed CNS-symptoms (mainly seizures and paresis) which were caused by intracerebral hemorrhage, infectious or degenerative CNS-diseases. Cranial axial tomography (CAT) was helpful in finding the cause of the CNS-complication. We recommend routine CAT in the beginning and during the course of treatment of leukemia to document CNS-changes as early

Community-acquired cerebral abscess and intracranial empyemas in children: a prospective cohort study.

OBJECTIVE: Community-acquired intracranial abscess and empyema can be associated with significant morbidity and mortality among children. Treatment strategies are made with local expert opinion supported by international guidelines, which are based on mostly low-quality evidence. Thus, the authors aimed to report their experience with the presentation, management, and outcomes of a pediatric cohort with intracranial abscess and empyema managed by their regional neuro-microbiological multidisciplinary team (MDT). METHODS: Data were collected from a prospectively collected database and suppleme

Silent otitis media with multiple intracranial complications.

A man in his early 40s, with no comorbidities, presented with a swelling in the right postauricular region for 5 days, which had progressively increased in size. He also gave a history of ear pain 2 months back. He denied history of ear discharge, difficulty hearing, giddiness or tinnitus. There was no history of fever, headache or seizures. On examination, the right pinna and external auditory canal were normal. The tympanic membrane was greyish white and intact. A swelling of size 4×3 cm was noted over the right mastoid region. High-resolution CT of the temporal bone showed right otomastoidi

A 32-Year-Old-Man With a Severe Headache, Visual Loss, and Nodular Pulmonary Opacities.

A 32-year-old man presented with severe headache, vomiting, and painless loss of vision of 5 days' duration. He had no seizures, other neurologic deficits, or fever. Two months prior to presentation, he had an episode of hemoptysis of 30 to 50 mL, which resolved spontaneously; there was no recurrence. He reported one to two episodes of epistaxis each month that resolved spontaneously, since childhood. He had no known comorbid illness, prior surgeries, dental procedures, or blood transfusions. He denied any history of substance abuse. He observed that his father and brother also had recurrent s

Complications of chronic suppurative otitis media: challenge for a developing country.

OBJECTIVES: This study aims to investigate the frequency and management of complications of chronic suppurative otitis media (CSOM). PATIENTS AND METHODS: Out of 376 patients with CSOM, 44 patients (25 males, 19 females; mean age 48.5±3.5 years; range 21 to 76 years) meeting study criteria were enrolled in this descriptive cross sectional study which was conducted at the Department of Ear Nose and Throat, Head and Neck Surgery, Postgraduate Medical Institute, Lady Reading Hospital Peshawar. All newly diagnosed patients having CSOM with complications were enrolled. The complications were treat

Intracranial complications of sinusitis: what is the role of endoscopic sinus surgery in the acute setting.

OBJECTIVE: Intracranial complications of sinusitis (ICS) are uncommon in the antibiotic era. The role of neurosurgical drainage of ICS is fairly well established, but the role for acute surgical intervention for the sinus disease is not well defined. We retrospectively reviewed our experience with ICS to see if we could identify the role and effectiveness of endoscopic sinus surgery (ESS) in the acute setting of ICS. METHODS: The study used a retrospective review of patients presenting to a tertiary care academic medical center for a 6-year period. RESULTS: Twenty-three patients were identif

Auditory auras in patients with postencephalitic epilepsy: case series.

Many previously healthy patients who present with new seizures or status epilepticus appear to have encephalitis for which no specific pathogen is identified. A significant portion of these patients develop epilepsy. We describe five patients with adult-onset, medically intractable, postencephalitic epilepsy characterized by auditory auras ranging from unformed buzzing to structured language. Auras of any type were common among patients with postencephalitic epilepsy in our referral database: 62% experienced at least one type of aura. The proportion of postencephalitic patients with auditory a

Intracranial vasculitis and multiple abscesses in a pregnant woman.

Cerebral vasculitis is an unusual disorder with many causes. Infectious causes of cerebral vasculitis are predominantly bacterial or viral in nature. Purulent bacterial vasculitis is most often a complication of severe bacterial meningitis. The patient is a 25-year-old African American female, 25 weeks pregnant, who presented to the neurology service after a consult and referral from an outside hospital. She had a 1-month history of right sixth nerve palsy. Initial workup included a negative lumber puncture and a noninfused magnetic resonance imaging (MRI). Three days later, the patient develo

Neuroimaging of central nervous system tuberculosis in HIV-infected patients.

The recent resurgence of tuberculosis in the United States is in part attributable to the AIDS epidemic. Central nervous system tuberculosis has fairly specific radiographic features, and thus the alert radiologist may be the first to suggest this diagnosis. This article reviews the neuroimaging characteristics of central nervous system tuberculosis in HIV-seropositive patients, including intracranial manifestations, tuberculous spondylitis, and tuberculous arachnoiditis.

Case report: arachnoiditis following intracranial 'Thorotrast'.

A patient is reported with a painless, progressive cauda equina lesion due to arachnoiditis, the result of the contrast medium 'Thorotrast' (thorium dioxide) introduced into the brain over 30 years previously. Contrast medium introduced into the lumbar spine can give rise to aseptic adhesive inflammation (arachnoiditis). It rarely gives rise to clinical problems but, when it does so, is usually associated with back pain and only very rarely with progressive neurological deficit.

Complications of mastoiditis with special emphasis on venous sinus thrombosis.

Two children with intracranial venous sinus thrombosis complicating mastoiditis prompted review of the experience with these disorders at the The Children's Memorial Hospital in Chicago over the past decade. Fifteen patients, 1.5 to 14 years of age, with mastoiditis were identified, including nine cases categorized as acute and six as chronic, the latter without fever but with chronic otorrhea. All six patients with chronic, but none with acute, mastoiditis were found to have cholesteatomas. Venous sinus thrombosis developed in four children and intracerebellar abscess in one child. Contrast-e

Factors relating to intelligence in treated cases of spina bifida cystica.

Analysis of results on 83 survivors of spina bifida cystica showed the following: (1) in the seven children who had had central nervous system (CNS) infection, intelligence was impaired, six being severely retarded. (2) In the nine children who did not suffer CNS infection or require a shunt, intelligence was normal. The need for a shunt was related to radiological appearance (craniolacunae) and to the sensory level at birth. (3) In the 67 children who did not suffer CNS infection but did require a shunt, intelligence was related to sensory level found at birth and to thickness of the pallium
