Implementing Internet-Delivered Cognitive Behavioral Therapy for Depression and Anxiety in Adults: Systematic Review.

BACKGROUND: Scientific implementation findings relevant to the implementation of internet-delivered cognitive behavioral therapy (iCBT) for depression and anxiety in adults remain sparse and scattered across different sources of published information. Identifying evidence-based factors that influence the implementation of iCBT is key to successfully using iCBT in real-world clinical settings. OBJECTIVE: This systematic review evaluated the following: (1) aspects that research articles postulate as important for the implementation of iCBT and (2) aspects relevant to the day-to-day running of i

Your period and your pregnancy, a cohort study of pregnant patients investigating the associations between menstruation and birth outcomes in Australia: study protocol.

INTRODUCTION: Early pregnancy care involves the screening and identification of women with risk factors for adverse pregnancy outcomes, including stillbirth or preterm birth, to tailor pregnancy care and interventions accordingly. Most stillbirths and approximately two-thirds of preterm births, however, occur in the absence of evident risk factors. The majority of stillbirths occur in the preterm period, yet there are few interventions targeting this period, and progress to reduce stillbirth rates remains slow. Placental dysfunction is a major contributor to stillbirth, particularly, preterm s

Effect of desflurane anesthesia on flash visual evoked potential monitoring in patients undergoing spine surgery: study protocol for a randomized controlled trial.

BACKGROUND: Flash visual evoked potentials (FVEPs) are a reliable method for protecting visual function during spine surgery in prone position. However, the popularization and application of FVEPs remain limited due to the unclear influence of various anesthetics on FVEPs. Exploring the effects of anesthetic drugs on FVEP and establishing appropriate anesthesia maintenance methods are particularly important for promoting and applying FVEP. According to the conventional concept, inhaled narcotic drugs significantly affect the success of FVEP monitoring, FVEP extraction, and interpretation. None

Effects of positive end-expiratory pressure on intracranial pressure, cerebral perfusion pressure, and brain oxygenation in acute brain injury: Friend or foe? A scoping review.

BACKGROUND: Patients with acute brain injury (ABI) are a peculiar population because ABI does not only affect the brain but also other organs such as the lungs, as theorized in brain-lung crosstalk models. ABI patients often require mechanical ventilation (MV) to avoid the complications of impaired respiratory function that can follow ABI; MV should be settled with meticulousness owing to its effects on the intracranial compartment, especially regarding positive end-expiratory pressure (PEEP). This scoping review aimed to (1) describe the physiological basis and mechanisms related to the effec

HIV-Related Intersectional Stigma and Discrimination Measurement: State of the Science.

Background. Across settings, individuals from populations that are multiply stigmatized are at increased risk of HIV and experience worse HIV treatment outcomes. As evidence expands on how intersecting stigmatized identities and conditions influence HIV outcomes, researchers have used diverse quantitative approaches to measure HIV-related intersectional stigma and discrimination. To date, no clear consensus exists regarding how to best quantitatively measure and analyze intersectional stigma and discrimination. Objectives. To review and document existing quantitative measures of HIV-related in

Application of the COM-B model to barriers and facilitators to chlamydia testing in general practice for young people and primary care practitioners: a systematic review.

BACKGROUND: Chlamydia is a major public health concern, with high economic and social costs. In 2016, there were over 200,000 chlamydia diagnoses made in England. The highest prevalence rates are found among young people. Although annual testing for sexually active young people is recommended, many do not receive testing. General practice is one ideal setting for testing, yet attempts to increase testing in this setting have been disappointing. The Capability, Opportunity, and Motivation Model of Behaviour (COM-B model) may help improve understanding of the underpinnings of chlamydia testing.

Leadership-as-practice to improve hand hygiene: a mixed-methods study in a Nepal emergency department.

PURPOSE: This study aims to examine the application of a leadership-as-practice (L-A-P) approach to improve hand hygiene compliance in a resource-limited emergency department (ED) in Nepal. Poor hand hygiene remains a major contributor to health care-associated infections, and traditional hierarchical leadership approaches often fail to achieve sustained behavioural change. By conceptualising leadership as a shared, practice-based process, this study aimed to embed hand hygiene into everyday clinical work, promote shared accountability and strengthen patient safety culture within a high-pressu

Facilitators and barriers to implementing AI-assisted diagnostic systems in primary healthcare services in low-resource settings: a mixed-methods study protocol.

INTRODUCTION: Although AI-assisted diagnostic systems are increasingly deployed in primary care globally, evidence from multiple countries indicates that deployment does not necessarily ensure clinical uptake. Resource-constrained and dispersed regions face implementation challenges, yet the mechanisms remain unclear. This mixed-methods protocol employs the Consolidated Framework for Implementation Research (CFIR) to systematically identify barriers and facilitators to AI implementation in primary care in such settings. METHODS AND ANALYSIS: This study adopts a sequential mixed-methods design

Progression of intermittent exotropia in surgical and non-surgical patients: a retrospective cohort study.

PURPOSE: Intermittent exotropia (IXT) is a common form of childhood strabismus, and greater exodeviation or poorer control at presentation are known factors influencing the decision to pursue surgery. However, existing studies have not tracked pre-operative progression longitudinally or directly compared these changes between patients who undergo surgery and nonsurgical controls. This study aimed to compare the clinical features and pre-operative progression of IXT between surgically and nonsurgically managed children matched by duration of follow-up. METHODS: The medical records of patients

Factors influencing surgical residents attrition: a mixed-methods study from a low-middle-income country.

OBJECTIVE: Premature loss of residents prior to completion of training, is common among surgical specialties, negatively impacts other trainees and program leadership, and disrupts the continuity of care. Developing countries face various challanges which may impact this attrition, however, there is a lack of literature on the rate of resident attrition in low-middle-income countries (LMICs). We aimed to identify the rates and predictors of surgical residents' attrition at a single institution. METHODOLOGY: We conducted a mixed-methods observational study; single center retrospective review a

Perceived barriers and facilitators to conventional cervical screening: a qualitative study among women, primary health workers and community representatives in Uganda.

INTRODUCTION: Cervical cancer (CC) is the most common cancer among women in Uganda, with many being diagnosed when the disease is in an advanced stage. Visual Inspection with Acetic acid (VIA) remains the most readily available CC screening method in most health facilities in Uganda, but with low screening coverage. This study aimed to understand the barriers and facilitators to CC screening in Uganda. METHODS: A qualitative study was conducted between March 2022 and March 2023 in Mukono, and Wakiso. Through Focus Group Discussions (FGDs) data was collected on perceived barriers and facilitat

AI-Guided Cognitive Behavioral Therapy for Depression and Anxiety: Bridging the Mental Health Treatment Gap Through Digital Psychiatry.

Background: Depression and anxiety disorders remain among the leading contributors to global disability and represent a major public health challenge. Although evidence-based psychotherapies are available, access to treatment remains limited due to structural, economic, geographical, and workforce-related barriers. Digital mental health interventions have emerged as scalable approaches to reducing this treatment gap, with artificial intelligence (AI)-guided cognitive behavioral therapy (CBT) representing a rapidly developing and clinically relevant extension of digital psychotherapy. Development of IgE autoantibodies in newborns with atopic dermatitis (DIANA): protocol of a prospective, non-interventional, observational birth cohort.

INTRODUCTION: Atopic dermatitis (AD) is a chronic, pruritic inflammatory skin disorder that typically begins in early childhood and affects up to 20% of children. Infants with a family history of allergic diseases or with elevated total IgE levels are at increased risk of developing AD. Emerging evidence shows that a subgroup of AD patients produce IgE autoantibodies against skin self-peptides, which are suggested to play a role in disease pathophysiology and predict the development of allergic diseases later in life. Notably, such autoantibodies have been detected in up to 15% of infants with

Mathematical Prediction Models for Sentinel Node Status in Early-Stage Breast Cancer: Protocol for a Systematic Review.

BACKGROUND: The status of the axilla remains a significant prognostic factor and influences adjuvant systemic and locoregional treatment choices in early-stage breast cancer (EBC). Sentinel node (SN) biopsy continues to be the preferred technique for establishing axillary nodal status in clinically node-negative EBC. A multivariable prediction model with adequate accuracy and generalizability has been explored as a potential alternative to SN. OBJECTIVE: This systematic review aims to evaluate the predictive performance, methodological quality, and risk of bias associated with the available m

The impact of magnesium levels in pregnancy on adverse birth outcomes: a protocol for a systematic review and meta-analysis.

BACKGROUND: Magnesium plays a critical role in maternal and fetal physiology, influencing vascular tone, inflammation, and cellular metabolism. Despite its established biological importance, evidence linking maternal magnesium status to adverse pregnancy outcomes remains inconsistent, partly due to heterogeneity in measurement methods, reference ranges, and study populations. These uncertainties are of significant public health importance globally but are of particular relevance in low- and middle-income countries (LMICs), where comorbidities such as HIV infection, anaemia, and obesity are pre
