Wide-Awake Wrist Arthroscopy and Open TFCC Repair.

The wide-awake approach to hand surgery entails the use of local infiltration anesthesia using lidocaine with epinephrine and no tourniquet. The technique provides practitioners with an option to perform advanced hand surgical care in an ambulatory setting, without the need for general or regional anesthetics. We present our results using wide-awake approach in wrist surgery, both open and arthroscopic. Between June and August 2011, the wide-awake approach was used in nine elective wrist surgery cases; three arthroscopic procedures, four open triangular fibrocartilage complex (TFCC) repairs, a

The SmartHand transradial prosthesis.

BACKGROUND: Prosthetic components and control interfaces for upper limb amputees have barely changed in the past 40 years. Many transradial prostheses have been developed in the past, nonetheless most of them would be inappropriate if/when a large bandwidth human-machine interface for control and perception would be available, due to either their limited (or inexistent) sensorization or limited dexterity. SmartHand tackles this issue as is meant to be clinically experimented in amputees employing different neuro-interfaces, in order to investigate their effectiveness. This paper presents the d

Effect of epidural stimulation of the lumbosacral spinal cord on voluntary movement, standing, and assisted stepping after motor complete paraplegia: a case study.

BACKGROUND: Repeated periods of stimulation of the spinal cord and training increased the ability to control movement in animal models of spinal cord injury. We hypothesised that tonic epidural spinal cord stimulation can modulate spinal circuitry in human beings into a physiological state that enables sensory input from standing and stepping movements to serve as a source of neural control to undertake these tasks. METHODS: A 23-year-old man who had paraplegia from a C7-T1 subluxation as a result of a motor vehicle accident in July 2006, presented with complete loss of clinically detectable

Differences between naïve and expert observers' vergence and accommodative responses to a range of targets.

PURPOSE: Vergence and accommodation studies often use adult participants with experience of vision science. Reports of infant and clinical responses are generally more variable and of lower gain, with the implication that differences lie in immaturity or sub-optimal clinical characteristics but expert/naïve differences are rarely considered or quantified. METHODS: Sixteen undergraduates, naïve to vision science, were individually matched by age, visual acuity, refractive error, heterophoria, stereoacuity and near point of accommodation to second- and third-year orthoptics and optometry underg

Distribution and immunohistochemical characterization of primary afferent neurons innervating the levator ani muscle of the female squirrel monkey.

OBJECTIVE: This study was undertaken to examine the neurofilament and neurochemical composition of subpopulations of primary afferent neurons innervating the levator ani muscle by combining retrograde tracing and triple labeling immunofluorescence in the female squirrel monkey. STUDY DESIGN: Cholera toxin B subunit (CTB) was injected unilaterally into the levator ani muscle of 3 monkeys to identify primary sensory neurons in the dorsal root ganglia (DRG) and their central projections in the spinal cord. L7-S2 DRG were processed for dual or triple labeling immunofluorescence 3 days after injec

[Ocular neuromyotonia].

BACKGROUND: Ocular neuromyotonia is a rare ocular motility disorder characterized by involuntary contractions of one or several ocular motor muscles. In this report a typical case is presented. PATIENT AND METHODS: A 61-year-old female patient was referred with persistent diplopia despite previous surgical treatment for right 6 (th) nerve palsy. The palsy was caused by a meningeoma of the petrous bone, which had been incompletely resected. Subsequently, the patient had received radiation therapy. Strabismological and neuroophthalmological examinations were performed. The results coincide with

Neck muscle fatigue affects postural control in man.

We hypothesised that, since anomalous neck proprioceptive input can produce perturbing effects on posture, neck muscle fatigue could alter body balance control through a mechanism connected to fatigue-induced afferent inflow. Eighteen normal subjects underwent fatiguing contractions of head extensor muscles. Sway during quiet stance was recorded by a dynamometric platform, both prior to and after fatigue and recovery, with eyes open and eyes closed. After each trial, subjects were asked to rate their postural control. Fatigue was induced by having subjects stand upright and exert a force corre

Spatial frames of reference and somatosensory processing: a neuropsychological perspective.

In patients with lesions in the right hemisphere, frequently involving the posterior parietal regions, left-sided somatosensory (and visual and motor) deficits not only reflect a disorder of primary sensory processes, but also have a higher-order component related to a defective spatial representation of the body. This additional factor, related to right brain damage, is clinically relevant: contralesional hemianaesthesia (and hemianopia and hemiplegia) is more frequent in right brain-damaged patients than in patients with damage to the left side of the brain. Three main lines of investigation

Vision does not always help stroke survivors compensate for impaired limb position sense.

BACKGROUND: Position sense is commonly impaired after stroke. Traditional rehabilitation methods instruct patients to visualize their limbs to compensate for impaired position sense. OBJECTIVE: Our goal was to evaluate how the use of vision influences impaired position sense. METHODS: We examined 177 stroke survivors, an average of 12.7 days (+/- 10 days (SD)) post-stroke, and 133 neurologically-intact controls with a robotic assessment of position sense. The robot positioned one limb (affected) and subjects attempted to mirror-match the position using the opposite limb (unaffected). Subject

Modulatory effect of repetitive peripheral magnetic stimulation on skeletal muscle tone in healthy subjects: stabilization of the elbow joint.

To investigate the role of repetitive peripheral magnetic stimulation (RPMS) on the postural component of motor performances, the long-lasting modulatory effect of RPMS on the stabilization of the elbow joint was examined in 13 healthy subjects. The resistance against very slow passive movements in the relaxed state was recorded simultaneously with the electromyogram (EMG) of the forearm extensor and flexor muscles. The experiments show that RPMS performed on the forearm flexor muscles increased the degree of stabilization of the elbow joint, whereas RPMS on the forearm extensor muscles caused

Needle bevel-guided standardisation of scleral tunnel formation in intrascleral haptic fixation: a quantitative geometric analysis.

BACKGROUND: Scleral tunnel formation during Yamane double-needle intrascleral haptic fixation (ISHF) depends on proprioceptive estimation, producing variable tunnel lengths that contribute to intraocular lens (IOL) malalignment. The aim of this study was to evaluate a needle bevel-guided technique using the innate 2 mm bevel of a 26-gauge needle as a visual endpoint for standardising intrascleral tunnel length, and to assess postoperative IOL positional stability using a quantitative geometric landmark framework. METHODS: Retrospective consecutive case series of 24 eyes undergoing ISHF in the

Bad Ragaz Ring Method Versus Halliwick Method in Verbally Cooperative Children with Spastic Cerebral Palsy (GMFCS Levels I-II): A Pilot Comparison Study.

Background/Objectives: Aquatic therapy is widely used for children with cerebral palsy (CP), but existing approaches such as the Halliwick method focus primarily on postural control and mobility with limited active resistance exercise. The Bad Ragaz Ring Method (BRRM) integrates proprioceptive neuromuscular facilitation (PNF) diagonal patterns with closed kinetic chain (CKC) exercise in the aquatic environment, yet its direct application to children with spastic CP has rarely been examined. This pilot study compared the effects of BRRM and the Halliwick method on muscle thickness, strength, an

Intercostal electromyographic responses to multi-site chest wall vibration at an optimal frequency.

BACKGROUND: High-frequency mechanical vibration facilitates respiratory muscle activity via muscle spindle afferents. Although synchronised multi-site chest wall vibration enhances ventilation, the associated intercostal electromyographic (EMG) responses and their contribution to ventilatory changes remain unclear. This study investigated the effects of multi-site chest wall vibration on intercostal muscle activation and ventilation in healthy adults. METHODS: Twenty healthy adults underwent phase-synchronised chest wall vibration delivered to four or eight intercostal sites during inspiratio

Optimizing Vestibular Rehabilitation: From Neuroplastic Mechanisms to Multimodal Therapeutic Strategies.

Peripheral vestibulopathy (PV) is a common disorder that causes dizziness and balance impairment, substantially affecting patients' quality of life. When symptoms persist, they frequently lead to anxiety, depression, and an increased risk of social isolation. Although central vestibular compensation (CVC) often promotes functional recovery, current pharmacological options remain limited and are primarily aimed at symptom control. Consequently, they should be considered adjuncts rather than alternatives to rehabilitation-based strategies. Vestibular rehabilitation (VR) remains the cornerstone o

Cross-sport patterns of health-related conditions in Chinese young athletes based on a comprehensive motor function assessment.

BACKGROUND: The increasing global participation in youth sports has been accompanied by a concerning rise in sports-related injuries among children and adolescents, highlighting the need for more comprehensive assessment approaches. Current methods often fail to account for the complex interplay of biomechanical, physiological, and developmental factors unique to young athletes. METHODS: This study employed an integrated motor function assessment framework combining sport-specific risk evaluation, Functional Movement Screening (FMS), metabolic-nutritional profiling, and biomechanical analysis

Engineering a functional tendon-bone interface: mechanisms and prospects of magnesium alloys in promoting neurotized regeneration via modulating stem cell-nerve interplay.

OBJECTIVE: Conventional strategies for tendon-bone interface (TBI) repair primarily focus on structural healing, often overlooking the essential processes of neural regeneration and proprioceptive recovery required for functional restoration. This review aims to explore the potential of biodegradable magnesium (Mg) alloys and the released magnesium ions (Mg2 +) in establishing a "Mg2 +-Stem Cell-Nerve" axis as a novel strategic foundation for achieving neurotized regeneration at the TBI. METHODS: This review provides a narrative synthesis of the existing literature on the roles of Mg2 + in re
