[Open double-row rotator cuff repair using the LASA-DR screw].

OBJECTIVE: Safe and cost-effective rotator-cuff repair. INDICATION: All types of rotator cuff lesions. CONTRAINDICATIONS: Frozen shoulder, rotator cuff mass defect, defect arthropathy. SURGICAL TECHNIQUE: Extensive four-point fixation on the bony footprint is performed using the double-row lateral augmentation screw anchor (LASA-DR) with high biomechanical stability. Following mobilization of the tendons, these are refixed in the desired configuration first medially and then laterally. To this end, two drilling channels (footprint and lateral tubercle) are created for each screw. Using the

The duration of symptoms does not correlate with rotator cuff tear severity or other patient-related features: a cross-sectional study of patients with atraumatic, full-thickness rotator cuff tears.

HYPOTHESIS: The purpose of this cross-sectional study is to determine whether the duration of symptoms influences the features seen in patients with atraumatic, full-thickness rotator cuff tears. Our hypothesis is that an increasing duration of symptoms will correlate with more advanced findings of rotator cuff tear severity on magnetic resonance imaging, worse shoulder outcome scores, more pain, decreased range of motion, and less strength. METHODS: We enrolled 450 patients with full-thickness rotator cuff tears in a prospective cohort study to assess the effectiveness of nonoperative treatm

Suprapectoral or subpectoral position for biceps tenodesis: biomechanical comparison of four different techniques in both positions.

HYPOTHESIS: The aim of this study was to compare different techniques for tenodesis of the long head of biceps tendon (LHB) in the suprapectoral and subpectoral position to test the hypothesis that using shorter screws at the subpectoral position would achieve a similar primary ultimate failure load (UFL) as the longer screws at the suprapectoral position, that both types of tenodesis screws achieve comparable UFL, and that knotless suture anchor techniques can be performed at the subpectoral position in cortical bone and reach a UFL similar to tenodesis screws. METHODS: On 42 fresh frozen hu

[MRI after reconstruction of the supraspinatus tendon: MR-tomographic findings].

AIM: The value of MRI for pre-operative evaluation of the rotator cuff is proven. In spite of its clinical relevance, there have been only few studies on MR imaging of patients who have already been operated. Thus, the question rises to what extent the post-operative MRI can contribute to the evaluation of the integrity of the rotator cuff. METHOD: At least 10 months after successful open repair of the supraspinatus tendon, mostly combined with Neer's anterior acromioplasty, an MR scan was performed on 33 shoulders. Only patients satisfied with the outcome of surgery and showing a Constant sc

SMARTWheel: From concept to clinical practice.

Wheelchair prescription is complex with thousands of choices and options. Theoretically, a higher quality or innovative wheelchair that is appropriately matched to the user and their unique needs will increase participation. It is well accepted that there is an alarmingly high incidence of carpal tunnel syndrome, and rotator cuff injuries among manual wheelchair users. Since the initial conceptualization, the SMART(Wheel) was intended to better understand the physiological and physical effects of wheelchair propulsion on the body. Initially, little was known about wheelchair propulsion and the…

Elevated body mass index and clinical outcomes after surgical stabilization for shoulder instability: A systematic review.

BACKGROUND: Obesity is understood to be a potential risk factor that negatively impacts outcomes across various orthopaedic procedures including weightbearing joint arthroplasty, and even in non-weightbearing joints, such as total shoulder arthroplasty and rotator cuff repair. However, there is limited understanding of the role obesity plays in the incidence and outcomes of shoulder instability. HYPOTHESIS/PURPOSE: The purpose of this study is to characterize the relationship between body mass index (BMI) and shoulder instability outcomes following operative stabilization. STUDY DESIGN: This

Simple, Cost-Effective Technique to Create a Double-Loaded Suture Anchor from a Non-loaded Suture Anchor.

INTRODUCTION: Suture anchors are used in the repair of rotator cuff repair, bankart's repair, SLAP repair, tendoachilles avulsion type injury, bony avulsion of medial collateral ligament of the knee, etc. The cost of preloaded suture anchors varies from INR 4500 to 10,000. Double-loaded suture anchors are better than single-loaded suture anchors in terms of having more hold in the soft tissues and having multiple anchoring points. Non-loaded screw type suture anchors are available with their inserter devices through which one suture stranded can be passed to create single-loaded suture anchor.

Glucocorticoids induce specific ion-channel-mediated toxicity in human rotator cuff tendon: a mechanism underpinning the ultimately deleterious effect of steroid injection in tendinopathy?

BACKGROUND: Glucocorticoid injection (GCI) and surgical rotator cuff repair are two widely used treatments for rotator cuff tendinopathy. Little is known about the way in which medical and surgical treatments affect the human rotator cuff tendon in vivo. We assessed the histological and immunohistochemical effects of these common treatments on the rotator cuff tendon. STUDY DESIGN: Controlled laboratory study. METHODS: Supraspinatus tendon biopsies were taken before and after treatment from 12 patients undergoing GCI and 8 patients undergoing surgical rotator cuff repair. All patients were s

CRITICAL IMPLICATIONS OF FEMALE BONE METABOLISM IN ORTHOPEDICS: STATE OF THE ART.

Female bone metabolism is a dynamic and tightly regulated process that plays a pivotal role in musculoskeletal health and orthopedic outcomes across the lifespan. Bone remodeling depends on the balance between osteoblastic bone formation and osteoclastic bone resorption and is strongly influenced by hormonal status, nutrition, genetics, physical activity, and systemic disease. In women, this balance is profoundly affected by reproductive and aging-related transitions, including adolescence, pregnancy and lactation, perimenopause, menopause, and postmenopause. Maximal peak bone mass is typicall

Partial tear at the musculotendinous junction of the teres major after anterior latissimus dorsi and teres major tendon transfer: associated factors and clinical impact in 256 patients.

BACKGROUND: Anterior latissimus dorsi and teres major (aLDTM) tendon transfer is an established procedure for isolated irreparable subscapularis tendon tear and anterosuperior irreparable rotator cuff tears. However, partial tear at the musculotendinous junction (MTJ) of the teres major (TM) has been observed on post-operative magnetic resonance imaging (MRI). The associated factors and clinical significance of these lesions remain unclear. This study aimed to identify factors associated with these lesions and to evaluate their impact on clinical outcomes. METHODS: We retrospectively reviewed

Comparison of ultrasound and MRI shows equivalent accuracy and reliability in acromial index measurement.

The acromial index, a radiological parameter assessing the subacromial space, is vital for diagnosing conditions like rotator cuff tears and subacromial impingement syndrome as the acromial Index influences the choice of therapy. Traditionally determined via X-ray, AI is increasingly assessed using MRI due to its superior image quality. However, MRI's cost and limited availability highlight the need for alternative methods. Ultrasound offers a promising, cost-effective alternative, providing real-time imaging without radiation exposure. This study aims to compare the accuracy and reliability o

An additional rehabilitation program to improve postoperative outcomes in patients with rotator cuff tear and scapular dyskinesis: a propensity score-matched study.

BACKGROUND: General scapular exercises have been included in traditional postoperative rehabilitation for rotator cuff tears, but patients with concomitant scapular dyskinesia do not have identical scapular muscle imbalances, and general scapular exercises make it difficult to improve scapular movement. We hypothesized that identifying weak scapular muscle groups and strengthening training would improve scapular movement. METHODS: A total of 60 rotator cuff tear patients with scapular dyskinesia were included in the study, 20 in the experimental group and 40 in the control group. Patients in
