Differences in antibiotic prescribing patterns for children younger than five years in the three major outpatient settings.

OBJECTIVES: To perform a comprehensive analysis of the use of antibiotics in three major sites for outpatient care: private office-based clinics, emergency departments (ED), and hospital-based clinics. STUDY DESIGN: Data from the National Ambulatory Medical Care Survey (NAMCS) and National Hospital Ambulatory Medical Care Survey (NHAMCS) were used to determine visit rates and antibiotic prescribing patterns for the three major outpatient care settings from 1994 to 2000 for children <5 years of age. RESULTS: Antibiotic prescription rates declined from 1405 to 1088 per 1000 children over the s

Leukocyturia in women with diabetes and its clinical implications.

BACKGROUND: Urinalysis is one of the most common studies performed on the diabetic patient at every visit. The presence of leukocyturia is relatively common but it is not clear what the attitude of the physician toward this particular finding should be. The main objective of the present study was to investigate the clinical significance of leukocyturia in diabetic women. METHODS: Ninety-eight diabetic women (84.7% type 2) aged 57 +/- 13 years who were being seen at the diabetic out-patient clinic were randomly selected. All patients underwent a clinical and gynecologic examination and a urina

Community-acquired Staphylococcus aureus bacteraemia in patients who do not abuse intravenous drugs.

Despite advances in antimicrobial therapy and intensive care support, Staphylococcus aureus continues to cause significant morbidity and mortality. We studied community-acquired S. aureus bacteraemia in a population where intravenous drug abuse is extremely uncommon, prospectively reviewing all such patients (n = 113) admitted to Groote Schuur Hospital from February 1986 to January 1991. Overall mortality was 35%. Factors associated with poor outcome were: confusion on presentation, failure to mount a febrile response, acute renal failure, adult respiratory distress syndrome, shock, endocardit

[Bilateral hydronephrosis secondary to ileocolic Crohn's disease].

Unilateral hydronephrosis can complicate a far from negligible number of patients with Crohn's disease. Bilateral hydronephrosis associated with Crohn's disease is a much more unusual entity. In the other hand, the treatment for this condition is still controversial. We present the case of a 44 year old female with several bowel obstruction episodes caused by ileocolic Crohn's disease. During the last episode she was also diagnosed of bilateral hydroureter and hydronephrosis. After the resolution of the bowel obstruction and the placement of a double J catheter in both ureters, the patient was

Vesicoureteric reflux in children.

Vesicoureteric reflux is defined as the retrograde passage of urine from the bladder into one or both ureters and often up to the kidneys, and mainly affects babies and infants. In severe cases dilatation of the ureter, renal pelvis, and calyces might be seen. Traditionally it was thought that only a low percentage of children have vesicoureteric reflux, but studies have suggested as many as 25-40% are affected. Guidelines recommend that the number of investigations for vesicoureteric reflux in children who have had a febrile urinary tract infection be reduced, but this approach is controversi

Postoperative bacteriuria, pyuria and urinary tract infection in patients with an orthotopic sigmoid colon neobladder replacement.

The purpose of this study is to investigate the prevalence of postoperative bacteriuria, pyuria and urine culture in patients with an orthotopic sigmoid colon neobladder replacement. Urine samples for bacteriuria, pyuria and urine culture, if necessary, were collected at 1, 3, 6, 9 and 12 months after surgery and the presence of blood culture and antibiotic-resistant strains, and their treatments on positive urine culture cases were investigated. Of 209 for bacteriuria and 207 for pyuria urine samples with evaluable data, 95 (45.5%) were positive for bacteriuria and 76 (36.7%) had pyuria (10 o

Changes in the incidence and antimicrobial susceptibility of healthcare-associated infections in a New York hospital system, 2006-2012.

INTRODUCTION: National efforts to curtail healthcare-associated infections (HAI) proliferated recently, though data detailing progress over time are limited. This retrospective cohort study aims to describe changes in incidence and antimicrobial susceptibility of HAI in four New York City hospitals over seven years. METHODS: Electronic data were collected retrospectively for all patients discharged from 2006 through 2012. Previously validated computerized algorithms based on National Healthcare Safety Network criteria detected bloodstream infections, pneumonia, surgical site infections, and u

Interventions for preventing the progression of autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the leading inherited cause of kidney disease. Clinical management has historically focused on symptom control and reducing associated complications. Improved understanding of the molecular and cellular mechanisms involved in kidney cyst growth and disease progression has resulted in new pharmaceutical agents targeting disease pathogenesis and preventing disease progression. However, the role of disease-modifying agents for all people with ADPKD is unclear. This is an update of a review first published in 2015. OBJECTIVES: We

Non-carbapenem therapy of urinary tract infections caused by extended-spectrum β-lactamase-producing Enterobacteriaceae.

PURPOSE: We determined the prevalence of ESBL Enterobacteriaceae in urinary tract infections among inpatients, identified risk factors of acquisition, and evaluated the effectiveness of alternatives to carbapenems. METHODS: The clinical, microbiological, and therapeutic data as well as the outcomes were recorded for all ESBL-E positive urine samples for three months. RESULTS: Thirty-one (4%) of the 762 Enterobacteriaceae positive cultures were ESBL producers. The predisposing conditions for being infected with those strains were: immunodepression (61%), recent hospitalization (52%), recent a

Comparative study of antibiotic resistance and virulence profiles of exoU and exoS Pseudomonas aeruginosa isolates from urinary tract infections and microbial keratitis in Bangladesh and India.

Pseudomonas aeruginosa is an important opportunistic pathogen in urinary tract infections (UTIs) and microbial keratitis (MK), but differences in resistance and virulence between exoU and exoS P. aeruginosa isolates across regions and infection sites remain poorly understood. This study compared antibiotic resistance and virulence profiles of exoU and exoS P. aeruginosa isolates from Bangladeshi and Indian UTIs, and from Indian UTIs and MK collections. A total of 23 Bangladeshi UTI isolates were analyzed by broth microdilution for susceptibility to ciprofloxacin, levofloxacin, gentamicin, and

Dynamics of urinary tract infections: a comprehensive study on antimicrobial susceptibility, virulence profiling and molecular epidemiology of uropathogenic Escherichia coli from Pakistan.

BACKGROUND: Urinary tract infections (UTIs), primarily caused by Escherichia coli, pose a global health challenge owing to rising antimicrobial resistance (AMR), particularly carbapenem-resistant (Carb-RT) and extended-spectrum beta-lactamase (ESBL)-producing uropathogenic E. coli (UPEC). This study analyzed AMR trends, virulence determinants, and molecular epidemiology of UPEC isolates from community and hospital settings in Pakistan. METHODS AND RESULTS: A total of 2700 urine samples were screened in this study. UPEC isolates underwent antimicrobial susceptibility testing, PCR screening for

Fosfomycin resistance in extended-spectrum beta-lactamase producing Escherichia coli isolated from urinary tract-infected patients in a tertiary care hospital.

Introduction. Urinary tract infections (UTIs) are a significant global health concern, with Escherichia coli being the predominant pathogen responsible for uncomplicated and complicated cases. Fosfomycin has emerged as a promising oral treatment option for multidrug-resistant UTIs, particularly those caused by extended-spectrum β-lactamase (ESBL)-producing E. coli. However, fosfomycin resistance has been paralleled by its irrational use and the emergence of enzymes that modify fosfomycin in ESBL-producing Enterobacteriaceae, especially in Asia.Hypothesis/Gap Statement. There is limited data on

Prevalence and risk factors for extended-spectrum β-lactamase producing antimicrobial-resistant E. coli in urinary tract infections among inpatients in the tertiary hospitals in Zanzibar (Tanzania): a prospective cross-sectional study.

INTRODUCTION: Extended-spectrum β-lactamase (ESBL) production among Enterobacteriaceae, such as E. coli, has been increasing worldwide, which causes treatment failure for urinary tract infections. Therefore, this study aimed to determine the prevalence and risk factors for the production of ESBL in E. coli from patients with urinary tract infections (UTI) in Zanzibar. METHODS: a prospective cross-sectional study was conducted from January 2018 to December 2021 in Zanzibar. Data were retrieved from a routine bacteriological laboratory culture report from urine samples of 4306 patients at the L

Adjuvant corticosteroids for prevention of kidney scarring in children with acute pyelonephritis: a systematic review and meta-analysis.

BACKGROUND: Acute pyelonephritis in children may result in permanent kidney scarring that is primarily caused by inflammation during acute infection. Antibiotic therapy alone is not enough to significantly reduce kidney scarring, and adjuvant corticosteroid therapy has shown a significant reduction in inflammatory cytokines in urine prompting its evaluation in randomised controlled trials. A few clinical trials showed a trend towards a reduction in renal scarring but did not have an adequate sample size to show a significant effect. Therefore, we planned to synthesise the available evidence on
