Medulloblastoma in childhood: results of radical resection and low-dose neuraxis radiation therapy.

This review concerns 22 children who were treated from 1980 through 1983 for medulloblastoma in the posterior fossa. Treatment included attempts at radical resection of the tumor and postoperative craniospinal radiation therapy, with 5000 to 5500 rads directed to the posterior fossa and 2500 rads to the remaining craniospinal axis. This lower radiation dose to the neuraxis was used to avoid late adverse effects upon the growing central nervous system of the children. Gross confirmation of total resection was obtained in 13 patients (the "total resection group"); however, nine patients had a su

[Sequential changes of vascular intimal ultrastructure in experimental cerebral vasospasm induced by oxyhemoglobin. Scanning electron microscopic study].

Organic changes of vessel wall may play an important role in the pathophysiology of vasospasm after subarachnoid hemorrhage. Sequential changes of intimal ultrastructure in experimental vasospasm induced oxyhemoglobin were investigated by scanning electron microscope. Thirty-four cats were used for this study. The basilar artery was exposed by transcervico-transclival approach. Oxyhemoglobin solution was applied under the arachnoid membrane around the artery in 16 cats at hourly intervals. Of 16 cats, 3 were sacrificed at 1 hour after the first application of the solution, 3 at 3 hours, 3 at 6

FXTAS presents with upregulation of the cytokines IL12 and TNFα.

INTRODUCTION: Fragile X Tremor and Ataxia Syndrome is a progressive neurodegenerative disorder that develops in some FMR1 premutation carriers. The objective of this study is to characterize how cytokine levels are altered in the FXTAS brain. METHODS: Fresh frozen cerebellar tissue from FXTAS cases and controls was homogenized and analyzed for 12 different cytokines using a commercially available ELISA panel. RESULTS: Relative to controls, FXTAS cases showed large and significant increases in the cytokines IL-12 and TNFα. There were large but non-significant increases in the levels of IL-2,

Propriospinal myoclonus upon relaxation and drowsiness: a cause of severe insomnia.

Three patients, aged 71, 50, and 41 years, presented with myoclonic activity arising in the relaxation period preceding sleep onset and causing severe insomnia. Polygraphic studies showed that the myoclonic activity began in spinally innervated muscles, propagating at low speed to rostral and caudal muscular segments. Myoclonus arose whenever patients relaxed mentally and showed diffuse electroencephalographic alpha activity and was independent of posture. The jerks disappeared during sleep. Clonazepam afforded symptomatic improvement. Propriospinal myoclonus arising from a spinal generator ma

Electrophysiological technique for evaluating lesions of the conus medullaris and cauda equina.

The authors describe the use of evoked electromyographic responses recorded in the anal sphincter induced by stimulation of the bladder wall and urethra in evaluating lesions of the conus medullaris and cauda equina in 110 patients. This reflex response took effect by way of the pelvic nerves and cauda equina to the sacral cord where the pudendal nerve nucleus was activated, resulting in a contraction of the external anal sphincter. Various lesions along this pathway have been shown to produce either increased latencies and depressed responses or complete loss of response depending on the exte

[A case of IgG4-related hypertrophic pachymeningitis and leptomeningitis presenting with cognitive impairment].

A 68-year-old female was admitted to our hospital because of a right-sided headache since one year, an inability to perform housework, and a decrease in speech since March of that year. Upon admission, higher brain dysfunctions, including attention disorder, aphasia, and apraxia, were observed. Contrast-enhanced MRI revealed dural thickening in the bilateral frontal to parietal lobes and FLAIR imaging revealed high signal intensities in the bilateral frontal brain sulcus. Blood tests revealed a high IgG4 level (273 mg/dl). Dural biopsy revealed infiltration of IgG4-positive plasma cells into t

Waardenburg syndrome: a rare cause of inherited neuropathy due to SOX10 mutation.

Waardenburg syndrome (WS) is a rare disorder comprising sensorineural deafness and pigmentation abnormalities. Four distinct subtypes are defined based on the presence or absence of additional symptoms. Mutations in six genes have been described in WS. SOX10 mutations are usually associated with a more severe phenotype of WS with peripheral demyelinating neuropathy, central dysmyelinating leukodystrophy, and Hirschsprung disease. Here we report a 32-year-old man with a novel heterozygous missense variant in SOX10 gene, who presented with congenital deafness, Hirschsprung disease, iris heteroch

Growing skull fracture stages and treatment strategy.

OBJECT: A growing skull fracture (GSF) is a rare but significant late complication of skull fractures, usually occurring during infancy and early childhood. Delayed diagnosis and improper treatment could exacerbate this disease. The aim of this study was to introduce a new hypothesis about, describe the stages of, and discuss the treatment strategy for GSF. METHODS: The authors performed a retrospective review of 27 patients with GSF, who were grouped according to 3 different GSF stages. RESULTS: Over a period of 20 years, 27 patients with GSF (16 males and 11 females) were treated in the au

Intrathecal chemotherapy delivered by a lumbar-thecal catheter in metastatic medulloblastoma: a case illustration.

BACKGROUND: Medulloblastoma is the most common malignant brain tumour in children. Despite recent advances, the prognosis in high risk patients remains poor. Further improvement in survival is dependent upon the development of strategies to attack the tumour more effectively, but with less toxicity. Intrathecal chemotherapy, is an ideal but currently underused method of directly targeting residual tumour within the area of resection and the leptomeningeal disease commonly associated with this tumour. METHOD: We describe the case of a 12 yr old child with metastatic medulloblastoma, who receiv

Magnetic resonance imaging of syrinx associated with intramedullary metastases and leptomeningeal disease.

Intramedullary spinal cord metastasis with an associated syrinx diagnosed by magnetic resonance imaging (MR) is described. The patient had documented simultaneous leptomeningeal spread of malignant cells and intramedullary spinal cord metastasis with hyalinized blood vessels, venous dilatation, and cavitation detected by autopsy. Metastasis to the spinal cord is unusual, but well described. Syrinx associated with intramedullary spinal cord metastasis has been detected rarely. MRI of syrinx and intramedullary spinal cord metastasis, and the possible pathogenesis of these lesions are discussed.

High-dose contrast-enhanced computed tomography (CECT) with iopamidol in the detection of cerebral metastases. Tolerance of the contrast agent.

Intravenously administered iodinated contrast media have been demonstrated, since early experience with computed tomography of the brain, to improve clinical value of the procedure for detecting intracranial lesions. There is no universal agreement about the amount and the method of administration of the contrast medium. Many authors maintain that the use of large doses gives better results for the diagnosis of tumors and metastases. The purpose of this paper is to evaluate the tolerance of iopamidol administered by rapid intravenous infusion in a large number of patients undergoing contrast e

[Six cases of hydroencephalodysplasia (author's transl)].

Six cases of hydroencephalodysplasia were evaluated from the points of clinical signs and symptoms and neuroradiological procedures and diagnostic problems were discussed. All of our cases were characterized by extensive dysplasia of the brain and increased cerebrospinal fluid pressure. Deformity of the head and developmental arrest of the psychomotor function were observed in five cases. The side of the skull deformity was consisted with that of the brain defect. Pneumoencephalography and cerebral angiography are characteristic in this disease, but these procedures have to be carried out with
