Fungal infections of the central nervous system (CNS) frequently occur in the immunocompromised or debilitated host. Imaging findings are non-specific but may be organized into extra-axial, parenchymal, and vascular categories. Furthermore, knowledge of fungal morphology may predict the imaging manifestations with large, hyphal species having a predilection for brain parenchymal involvement, while small, unicellular organisms typically result in meningitis. Advanced imaging techniques such as diffusion-weighted imaging, MR perfusion and MR spectroscopy, when combined with clinical findings, ma

This article attempts to cover the most common problems likely to present in neonatal crias and to give guidance on how to approach these cases, particularly how to stabilize the acute presentation of a sick neonate. It is impossible to cover every eventuality and it is important to keep an open mind when evaluating and treating sick neonatal camelids. Despite the often moribund presentation of sick neonatal crias, they may be highly rewarding to treat, although hospitalization and 24-hour care is recommended.

A total of 450 brain examinations were performed using computed tomography before and after intravenous infusion of methylglucamine diatrizoate 30%. In 110 cases involving a documented intracranial lesion, the scans were analyzed to determine the degree and usefulness of contrast enhancement. Enhancement was achieved in a wide variety of neoplastic and non-neoplastic lesions and was found to be of considerable help in detection of abnormality as well as in defining tumor morphology.

BACKGROUND: Data on Computed tomographic (CT) pattern of stroke abound in literature but they are predominantly from studies done in the Western world. Though most of the reports agree that ischaemic rather than haemorrhagic stroke is prevalent in most climes,no strictly radiology based study can be quoted in the Port Harcourt area. Thus there is the need for accurate data on stroke pattern in Nigerians of the South-South region.
AIMS: This study was done to establish the CT pattern of stroke seen in University of Port Harcourt Teaching Hospital (UPTH) and to determine the relationship betwee

Infective endocarditis is a sudden illness that rapidly causes cardiac and extracardiac injury. Embolic material travels into the arterial circulation causing embolic events in 20-50% of patients. The brain is one of the most frequent sites of embolism that potentially interferes with treatment options. Neurologic complications are the presenting symptom in 20% of the cases being associated with poor prognosis (45% of deaths versus 24% in patients without these complications). This is the case of a 63-year-old male patient presenting with main clinic of stroke. Multifocal signs and past aortic

OBJECTIVE: To assess clinical profile and short term outcome of patients with meningitis related ventriculitis.
METHODS: Case records of 47 patients admitted with a diagnosis of ventriculitis (complicating meningitis) over 3 y were analysed retrospectively.
RESULTS: The mean age of patients was 4.96 mo (20 d - 42 mo). The common symptoms were fever (27/47), enlarging head (23/47) and seizures (12/47) with signs of raised intracranial pressure (29/47). Ventricular cerebro-spinal fluid (CSF) showed mean cells - 237 cells/μL, sugar - 36 mg/dL and protein - 568 mg/dL. Gram stain and/or culture w

OBJECTIVES: Squamous Cell Carcinoma (SCC) is among the most frequently reported malignancies in XP; however, intracranial extension is exceedingly rare and may pose a significant diagnostic challenge when radiologic findings mimic those of cerebral abscess.
METHOD: The case of a 7-year-old male child with XP who developed an ulcerative lesion on the scalp and underwent surgical excision is reported. Xeroderma Pigmentosum (XP) is a rare autosomal recessive genodermatosis characterized by extreme sensitivity to ultraviolet radiation and an increased predisposition to cutaneous malignancies..

OBJECTIVE: To present a unique case of a brainstem Curvularia fungal infection and review the diagnosis and management of this rare phenomenon.
METHODS: A 33-year-old immunocompetent African American male presented with 2 weeks of headache, nausea, and vomiting in a setting of a recent 20-lb weight loss. Neurological examination was positive for multiple cranial nerve palsies, hemisensory loss, and gait instability. Magnetic resonance imaging demonstrated an enhancing medullary lesion.
RESULTS: Metastatic and infectious workup revealed a left lung lesion, which on subsequent biopsy was posit

We reviewed the neuropathologic characteristics of 52 cases of acquired immune deficiency syndrome (AIDS) at autopsy. Histologically significant neuropathologic lesions were found in 38 cases. We believed that infection was the predominant pathologic process in 26 cases; occasionally, multiple infectious agents were present. This included Toxoplasma encephalitis (n = 16), fungal abscess (n = 1), tuberculous abscess (n = 1), progressive multifocal leukoencephalopathy (n = 2), cysticercosis (n = 1), and Escherichia coli meningoencephalitis (n = 1). Microglial nodules or perivascular inflammation

The first case of cerebral paragonimiasis was reported by Otani in Japan in 1887. This was nine years after Kerbert's discovery of the fluke in the lungs of Bengal tigers and seven years after a human pulmonary infection by the fluke was demonstrated by Baelz and Manson. The first case was a 26-year-old man who had been suffering from cough and hemosputum for one year. The patient developed convulsive seizures with subsequent coma and died. The postmortem examination showed cystic lesions in the right frontal and occipital lobes. An adult fluke was found in the occipital lesion and another was

OBJECT: Awake craniotomy is currently a useful surgical approach to help identify and preserve functional areas during cortical and subcortical tumor resections. Methodologies have evolved over time to maximize patient safety and minimize morbidity using this technique. The goal of this study is to analyze a single surgeon's experience and the evolving methodology of awake language and sensorimotor mapping for glioma surgery.
METHODS: The authors retrospectively studied patients undergoing awake brain tumor surgery between 1986 and 2014. Operations for the initial 248 patients (1986-1997) wer

Stroke-like Migraine Attacks after Radiation Therapy (SMART) syndrome is a rare, delayed complication of cerebral irradiation characterized by stroke-like symptoms, seizures, and/or headaches, with mean latency period of 11.2 years. It is most associated with gliomas and medulloblastomas and diagnosis is challenging due to symptom overlap with recurrent neurologic deficits. We present a 59-year-old male with history of right frontal convexity anaplastic meningioma complicated by seizure disorder and Todd's paralysis, status post tumor resection, adjuvant radiation to 6000cGy, craniotomy and cr

BACKGROUND: Varicella-zoster virus infection is typically self-limited in children but can rarely lead to severe neurological and thrombotic complications. Post-infectious immune dysregulation may contribute to conditions such as cerebral venous sinus thrombosis (CVST) and autoimmune encephalitis. Reports describing the coexistence of these complications, particularly with anti-GAD65 antibodies, are exceedingly rare.
CASE PRESENTATION: An 8-year-old boy developed seizures, headache, and right-sided hemiplegia twelve days after varicella infection. Neuroimaging revealed CVST with intracranial

OBJECTIVES: Before molecular taxonomy, Nocardia species infections were predominantly attributed to Nocardia asteroides; now, more than 50 species are known to infect humans. To better study the range of associated clinicopathologic features, Nocardia species can be grouped into complexes based on gene sequencing, antibiotic susceptibilities, and matrix-assisted laser desorption/ionization with time-of-flight mass spectrometry profiling.
METHODS: A 20-year retrospective study of laboratory-detected Nocardia species cases from 2 US tertiary-care academic medical centers was performed by review
