Severe neck infections that require wide external drainage: clinical analysis of 17 consecutive cases.

Infections in the neck layers and spaces are potentially life-threatening diseases causing further complications, like mediastinitis, airway obstruction, or sepsis. Despite of the need for a conservative approach, they still regularly require surgical intervention. Records of 17 patients with severe neck infections that were treated by wide external incision and open wound management were retrospectively analyzed. The aim of the study was to clinically characterize these most serious neck infections. The most common presenting symptoms were neck pain and tense neck mass (94-94%) regularly with

Necrotizing soft-tissue infections in obstetric and gynecologic patients.

For the clinician, necrotizing soft-tissue infections have remained a daunting opponent since the first writings on the subject over 2000 years ago. Early disease may be incorrectly diagnosed as cellulitis, and this delay in correctly diagnosing and expeditiously proceeding to radical surgical debridement may lead to a high degree of mortality. Although several inciting events and risk factors have been described that allow for the development and progression of this disease, the diagnosis is still made clinically. Only aggressive surgical management in combination with broad-spectrum antibiot

A case of streptococcal toxic shock syndrome due to Group G streptococci identified as Streptococcus dysgalactiae subsp. equisimilis.

A 79-year-old man with a 3-month history of lymphedema of the lower limbs, and diabetes mellitus, was admitted to our hospital for suspected deep venous thrombosis. Several hours after admission, leg pain and purpura-like skin color appeared. On the 2nd hospital day, he was referred to our department for possible acute occlusive peripheral artery disease (PAD) and skin necrosis with blisters; however, computed tomography with contrast showed no occlusive lesions. He had already developed shock and necrotizing deep soft-tissue infections of the left lower leg. Laboratory findings revealed renal

Outcomes from treatment of necrotizing soft-tissue infections: results from the National Surgical Quality Improvement Program database.

BACKGROUND: Necrotizing soft-tissue infections (NSTIs) are a group of uncommon, rapidly progressive, potentially fatal disorders. The National Surgical Quality Improvement Program (NSQIP) Registry was used to determine current data on the incidence, treatment, and outcomes of NSTIs. METHODS: There were 688 NSTI cases identified for years 2005 to 2008. Ten control patients for each NSTI patient were also selected. Demographic, laboratory, and outcome data were collected to compare both groups. RESULTS: Evidence of systemic inflammatory response syndrome (SIRS), sepsis, or septic shock occurre

Traumatic rectal perforation presenting as necrotising fasciitis of the lower limb.

Necrotising fasciitis is a life-threatening soft tissue infection that is associated with high mortality and morbidity. It has been described in the form of Fournier's gangrene following rectal perforations related to colorectal cancer. In these rare instances, spontaneous perforation of locally-advanced rectal carcinoma provides an entry point for bacterial seeding to the surrounding soft tissues, resulting in Gram-negative sepsis of the perineum. To our knowledge, necrotising fasciitis extending beyond the perineum due to rectal perforation has not been previously described. We report an unu

Neglected peritonsillar abscess resulting in necrotizing soft tissue infection of the neck and chest wall.

Necrotizing soft tissue infection (NSTI) of the neck and chest wall resulting from neglected peritonsillar abscess is a relatively rare but highly lethal surgical condition which has received little attention in the literature. The case of a 54-year-old male patient affected by this unusual infection is reported. Our recent experience and literature data suggest that NSTI resulting from peritonsillar abscess is rapidly spreading and life threatening. High index of suspicion, early diagnosis, broad-spectrum antibiotics and aggressive surgical debridement are essential to its successful treatmen

A case of acute onset postoperative gas gangrene caused by Clostridium perfringens.

BACKGROUND: Gas gangrene is a necrotic infection of soft tissue associated with high mortality rates. We report a case of postoperative gas gangrene with very acute onset and rapid progression of symptoms. To our knowledge, this case is the most acute onset of postoperative gas gangrene ever reported. CASE PRESENTATION: A 65-year-old Japanese female patient developed a shock state 16 h after radical cystectomy with ileal conduit reconstruction. Two days after the operation, she was transferred to the intensive care unit because of deterioration in her respiratory and circulatory condition. So

Prognostic value of the shock index in Fournier's gangrene: a retrospective cohort study comparing established mortality scoring systems.

BACKGROUND: Fournier's gangrene is a rapidly progressive, life-threatening necrotizing infection of the perineal and genital re-gions, associated with persistently high mortality despite advances in surgical and intensive care. Early and reliable prognostic assess-ment is essential for improving patient outcomes. This study aimed to evaluate the prognostic performance of the Shock Index (SI) in predicting mortality in patients with Fournier's gangrene and to compare its discriminatory ability with established scoring systems. METHODS: This retrospective cohort study included adult patients di

Case Report: Total atherosclerotic occlusion of perforators in anterolateral thigh flap.

Atherosclerosis and calcification of vessels, commonly encountered in diabetic patients, is a well-known complicating factor in microsurgical reconstruction. However most studies on this problem have focused on axial vessels and their major branches. We present a case of total atherosclerotic occlusion of perforators rising from intact lateral circumflex femoral artery branches in bilateral anterolateral thigh (ALT) flap elevation. A 51-year-old male with a history of diabetes mellitus, ischemic heart disease and kidney transplantation presented with necrotizing fasciitis of the left foot. Ser

Predicting the Global Distribution of Vibrio vulnificus Under Changing Climate Scenarios.

Vibrio vulnificus is a Gram-negative bacterium and an emerging pathogen globally distributed in brackish and marine waters. It can be transmitted to humans through contaminated food, which causes primary septicemia, or through wound exposure, causing necrotizing fasciitis. Increased abundance and range expansion have recently been associated with climate change. Further understanding of the spatial epidemiology of V. vulnificus is necessary, and species distribution modeling is a technique to correlate all the factors related to the spread of V. vulnificus. Through meta-analysis aided by Rayya

When delay costs a limb: Severe chronic osteomyelitis in a patient with intravenous drug use.

INTRODUCTION: Intravenous drug use (IVDU) is strongly associated with severe infections including abscesses, endocarditis, necrotizing fasciitis, and osteomyelitis. When untreated, acute osteomyelitis can progress to chronic disease with biofilm-mediated antimicrobial resistance, soft tissue necrosis, and eventual limb loss. Reports of extreme cases requiring major amputation remain uncommon in the literature. CASE PRESENTATION: We report the case of a 32-year-old female with a history of substance use disorder who presented with a chronic right forearm wound after more than one year without

Eravacycline: From broad-spectrum coverage to novel clinical applications.

Eravacycline is a novel, fully synthetic fluorocycline antibiotic with broad-spectrum activity against Gram-positive, Gram-negative, aerobic, and anaerobic pathogens, including multidrug-resistant (MDR) strains. It maintains efficacy despite common tetracycline resistance mechanisms, such as efflux pumps and ribosomal protection proteins. Its pharmacokinetic profile is characterized by extensive tissue penetration, particularly into the epithelial lining fluid and alveolar macrophages. Eravacycline is particularly active against MDR pathogens such as carbapenem-resistant Enterobacterales and A

Infection risk and management in patients with cirrhosis: A critical overview.

In this paper, we analyze the article published by El Labban et al, which explores the impact of cirrhosis on patients with necrotizing fasciitis. The authors conclude that cirrhosis is a significant risk factor for increased in-hospital morbidity and mortality in this patient population. Building upon their final observation regarding the importance of understanding this association, we will delve into the topic of infections in patients with liver cirrhosis. These patients exhibit intrinsic characteristics that make them particularly susceptible to infections, both bacterial and fungal. This

Cellulitis in Pediatric Patients: Recognition and Management in the Era of Evolving Resistance.

Cellulitis, a bacterial skin infection most frequently caused by group A streptococci (Streptococcus pyogenes) and less so by Staphylococcus aureus, commonly occurs in pediatric patients. The non-specific clinical presentation of poorly demarcated, expanding erythema, and warmth is common to a multitude of similarly presenting conditions, contributing to challenges in accurate diagnosis. There is also no gold standard diagnostic test for cellulitis, as laboratory assessments, tissue and blood cultures, and imaging studies have not been helpful. These adjunctive studies may be useful, however,

Point-of-care ultrasound in skin and soft tissue infections.

BACKGROUND: Skin and soft tissue infections (SSTIs) are commonly encountered in clinical practice. Point-of-care ultrasound (POCUS) is becoming an increasingly valuable tool in hospital medicine, especially with advancements in ultrasound technology that make it easier to perform. POCUS can augment the history and physical exam in patients with suspected SSTIs. POCUS can detect deeper infections, such as abscesses, and expedite time to surgical debridement for life-threatening infections such as necrotizing fasciitis (NF). OBJECTIVES: This review provides an introduction on how to perform and

The CRP/PAB ratio outperforms the LRINEC score in early diagnosis of Fournier's gangrene.

BACKGROUND: Fournier's gangrene (FG) is scarce and potentially fatal disease. Although the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score was established in 2004, its reliability as a diagnostic tool to differentiate between FG and perianal abscess is still debated. The objective of this study was to assess the reliability of the LRINEC score and other relevant inflammatory markers. The diagnostic effectiveness of these inflammatory factors was evaluated and compared. METHODS: Retrospective observational study of patients with FG or with perianal abscess. Fifty-two patient

The burden of group A Streptococcus (GAS) infections: The challenge continues in the twenty-first century.

Streptococcus pyogenes is a Gram-positive bacterium, also known as Group A Streptococcus (GAS), that has become a significant threat to the healthcare system, infecting more than 18 million people and resulting in more than 500,000 deaths annually worldwide. GAS infection rates decreased gradually during the 20th century in Western countries, largely due to improved living conditions and access to antibiotics. However, post-COVID-19, the situation has led to a steep increase in GAS infection rates in Europe, the United States, Australia, and New Zealand, which triggers a global concern. GAS in
