The unstable knee in congenital limb deficiency.

PURPOSE: Instability of the knee is a common finding in patients with congenital limb deficiency. The instability can be attributed to soft tissue abnormalities, frontal, sagittal or rotational deformity of the lower limb and bony dysplasia of the patella or of the femoral condyles. In most of the cases, these pathomorphologic changes stay asymptomatic in daily activity. However, instability can appear during deformity correction and bone-lengthening procedures, leading to flexion contracture or subluxation of the knee. METHODS: A review of pediatric orthopaedic literature on different factor

Cystic Shape Cemento-Ossifying Fibroma of Ethmoid Sinus.

Cemento-ossifying fibromas are a group of rarely occurring benign tumours, developing from the periodontal membrane and varying considerably in appearance and in the progress of the disease. Their common feature is higher or lower production of cemental tissue. In most cases the tumours are small because their cementoma mature quickly and become inactive, which causes the tumour to stop growing. They develop most frequently in the mandible and also in the maxilla. Other sites, such as paranasal cavities, soft tissues and bones of the head, are extremely rare. The case of a cemento-fibrosing tu

Clinical characteristics and surgical decision making for infants with metopic craniosynostosis in conjunction with other congenital anomalies.

BACKGROUND: Metopic craniosynostosis can occur in isolation or in conjunction with other congenital anomalies. The surgical decision making … patients' CT scans revealed 13 with classic trigonencephaly, 3 with microcephaly, and 3 with narrow frontal bones, abnormal orbits, and small anterior fossa. Patients (90%) with isolated metopic craniosynostosis underwent cranial vault expansion, whereas only 63% of the complex group did so. The complex metopic group had a longer hospital stay (5 d vs 3.4 d), …

In situ free-floating craniectomy for traumatic cerebral decompression in an infant: A field hospital solution.

BACKGROUND: Despite refinements in neurotrauma care, the morbidity and mortality of severe traumatic brain injury (TBI) in pediatric patients remains high. We report a novel approach to the surgical management of increased intracranial pressure in severe TBI utilizing an in situ free-floating craniectomy technique, which was originally devised as a creative solution to the unique challenges in a Haitian field hospital following the 2010 earthquake. CASE DESCRIPTION: A 13-month-old Haitian boy presented to Project Medishare field hospital in Port-au-Prince with left hemiplegia, a bulging fonta

EphA4 as an effector of Twist1 in the guidance of osteogenic precursor cells during calvarial bone growth and in craniosynostosis.

Heterozygous loss of Twist1 function causes coronal synostosis in both mice and humans. We showed previously that in mice this phenotype is associated with a defect in the neural crest-mesoderm boundary within the coronal suture, as well as with a reduction in the expression of ephrin A2 (Efna2), ephrin A4 (Efna4) and EphA4 in the coronal suture. We also demonstrated that mutations in human EFNA4 are a cause of non-syndromic coronal synostosis. Here we investigate the cellular mechanisms by which Twist1, acting through Eph-ephrin signaling, regulates coronal suture development. We show that Ep

Computer image-guided surgery for total maxillectomy.

In total maxillectomy, the entire upper jaw including the tumor is removed en bloc from the facial skeleton. An intraoperative computed tomographic guidance system (ICTGS) can improve orientation during surgical procedures. However, its efficacy in head and neck surgery remains controversial. This study evaluated the use of an ICTGS in total maxillectomy. Five patients with maxillary sinus neoplasms underwent surgery using a StealthStation ICTGS. The headset was used for anatomic registration during the preoperative CT scan and surgical procedure. The average accuracy was 0.95 mm. The ICTGS pr

The unicompartmental knee: design and technical considerations in minimizing wear.

Unicompartmental knee arthroplasty is an alternative to total knee arthroplasty for patients with unicompartmental tibiofemoral noninflammatory disease. Careful patient selection and newer instrumentation has reduced the progression of arthrosis in the other compartment and tibial loosening, leaving polyethylene wear as the predominant failure mechanism in more contemporary designs. Increased wear increases the debris volume at the bone-implant interface, and wear particles will generate osteolysis leading to component loosening and unreplaced compartment degeneration. The design-related facto

The use of greater omentum vascularized free flaps for neurosurgical disorders requiring reconstruction.

The heterotopic transfer of composite tissue, with microvascular anastomosis of an arterial supply and venous drainage to locally existing vessels, has received considerable attention in the plastic surgical literature. The use of latissimus dorsi musculocutaneous free flaps has been reported in the repair of large defects of the scalp, cranium, and dura following resection of invasive neoplasms or trauma. When the defect involves primarily subcutaneous tissue loss resulting in abnormal contour, omental free flaps provide effective coverage and restoration of contour. In addition, the inherent

Relationship with sex-based anatomical differences to lower extremity injury severity in frontal crashes.

OBJECTIVE: Multiple studies have demonstrated an increased risk of lower extremity injuries for females in frontal crashes. This study aimed to investigate whether sex-based anatomical differences, as measured on computed tomography (CT) scans of the abdomen and pelvis, contribute to lower extremity injury risk. METHODS: The Crash Injury Research and Engineering Network (CIREN) database (2017-2023) was queried for frontal collisions. Cases were included if the occupant was an adult (≥18 years), seated in the front row, properly restrained, and had an analyzable CT scan of the abdomen and pelv

Biomechanical Evaluation of a Novel Stepped Bone Reconstruction Structure for the Mandible Under Traumatic Impact Loading.

OBJECTIVES: In daily life, following mandibular reconstruction surgery, accidental events such as falls and impacts may lead to fracture, displacement, or failure of the reconstructed site. This study investigates the biomechanical behavior of a Stepped Bone Reconstruction Structure (SBRS) when subjected to transient traumatic loads postoperatively. METHODS: On the basis of CT scans, an SBRS model was established. Two magnitudes of impact loads were simulated at 5 different locations: frontal symphyseal, left parasymphyseal, right parasymphyseal, left mandibular angle, and right mandibular an

The Nightmare of AVM Surgery: Early Rupture of the Venous Drainage-Lessons from Personal Experience and a Review of the Literature.

We describe the case of a 72-year-old man who presented with signs of increased intracranial pressure, right-sided motor deficit, and repeated episodes of epilepsy due to a left frontal arteriovenous malformation (AVM) with a large superficial draining vein. Despite great efforts to protect the vein from the start, it ruptured shortly after we removed the bone flap. This required rigorous hemorrhage control, which in turn led to profuse bleeding from the nidus throughout the process of the dissection and coagulation of the arterial feeders. The postoperative course was initially uneventful; ho

Superior orbital fissure syndrome after deep lateral orbital wall decompression in Graves' ophthalmopathy.

PURPOSE: The superior orbital fissure contains cranial nerves III, IV, VI, and V1 with their three branches: frontal, lacrimal, and nasociliary. Superior orbital fissure syndrome (SOFS) is rare and can occur as a result of compression of these nerves due to trauma, bleeding, or inflammation in the retrobulbar space, but no cases of SOFS after deep lateral orbital wall decompression (DLOWD) have been reported. The aim of this paper is to describe this pathology, its possible causes, management, and outcome. METHODS: Retrospective study of 575 DLOWD in patients with disfiguring exophthalmos due
