For postmenopausal females, the prevalence of osteoporosis was significantly lower in the hyperuricemia group compared with the normouricemic group (P=4.65E-06). Cognitive impairment has been described in elderly subjects with high normal concentrations of serum uric acid. However, it remains unclear if gout confers an increased poorer cognition than those in individuals with asymptomatic hyperuricemia. The present study aimed at evaluating cognitive function in patients suffering from gout in an elderly male population, and further investigating the genetic contributions to the risk of cognitive function. This study examined the cognitive function as assessed by Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA) in 205 ma…

Allopurinol, a common drug for treating hyperuricemia, is associated with cutaneous adverse drug reactions ranging from mild maculopapular exanthema to life-threatening severe cutaneous adverse reactions, including drug reaction with eosinophilia and systemic symptoms, Stevens-Johnson syndrome, and toxic epidermal necrolysis. We have previously reported that HLA-B*58:01 is strongly associated with allopurinol-induced severe cutaneous adverse reactions in Han Chinese, but the associations of the HLA-B*58:01 genotype in an allopurinol-induced hypersensitivity phenotype remain unclear. To investi…

Since the association of serum uric acid and kidney transplant graft outcome remains disputable, we sought to evaluate the predictive value … e episode of acute rejection and diabetic issue were associated with a higher mean uric acid level. Hyperuricemia was significantly an independent predictor of pure graft failure (hazard ratio=4.01, 95% CI: 1.25-12.91, P=0.02) after adjustment. But it was no longer an independent risk factor for graft loss after adjustment. Interestingly, higher triglyceride le …

INTRODUCTION: Previous studies have shown an association between gout and/or hyperuricemia and a subsequent increase in cardiovascular disease (CVD) outcomes. Allopurinol reduces vascular oxidative stress, ameliorates inflammatory state, improves endothelial function, and prevents atherosclerosis progression. Accordingly, we tested the hypothesis that a positive association between allopurinol therapy in gout patients and future cardiovascular outcomes is present using a population-based matched-cohort study design.
METHODS: Patients aged ≥40 years with newly diagnosed gout having no pre-exis…

CONCLUSIONS: Initial kidney function, proteinuria, renal size, hypercholesterolemia, hyperuricemia, and SBP are the factors that influence CKD progression in ADPKD. SBP and younger age at diagnosis are the only factors that maintain their independent predictive value in a multivariant analysis.

BACKGROUND: Lower dosages of allopurinol are recommended to avoid toxicity in gout patients with impaired renal function. This often has resulted in inadequate control of hyperuricemia. The optimum therapeutic range of plasma oxypurinol concentrations in gout patients with chronic kidney disease has never been investigated. This study was performed to examine the relationships between plasma oxypurinol concentrations and the changes in serum urate level and renal function after taking a standard dose of allopurinol, 300 mg daily, in gout patients with renal insufficiency.

Hyperuricaemia is a metabolic disorder characterized by elevated serum urate levels. Uric acid production primarily results from purine metabolism, with xanthine dehydrogenase (XDH) serving as a key enzyme determining uric acid synthesis. RNA interference technology has emerged as a powerful tool for gene regulation in recent years. Specifically, N-acetylgalactosamine (GalNAc)-modified small interfering RNA (siRNA), hereafter referred to as GalNAc-siRNA, can selectively deliver siRNAs to hepatocytes, achieving highly efficient knockdown of liver target genes. This study employed GalNAc-siRNA t

OBJECTIVE: Asymptomatic hyperuricaemia (AH) is an independent risk factor for chronic kidney disease, cardiovascular events, and metabolic disorders. However, controversies persist regarding the clinical use of urate-lowering therapies (ULTs). This study employs a network meta-analysis (NMA) to systematically evaluate the efficacy and safety of ULTs in AH across seven key endpoints: serum uric acid (SUA), estimated glomerular filtration rate (eGFR), high-sensitivity C-reactive protein (hs-CRP), systolic blood pressure (SBP), major adverse cardiovascular events (MACE), composite renal events (C

BACKGROUND AND AIM: Prior research has established a relationship between homocysteine (Hcy) and serum uric acid (SUA) levels; however, the precise mechanisms underlying this association remain unclear. The objective of this research was to explore the correlation between SUA and Hcy and to evaluate the possible role of kidney function as a mediator in the connection between SUA and Hcy.
METHODS AND RESULTS: Consecutive enrollment of 16870 participants aged 20-60 years was conducted at the Third Xiangya Hospital of Central South University. Self-reported health questionnaires were used to gat

Recently, the US Food and Drug Administration (FDA) issued a public safety alert, responding to the results of the now-published Cardiovascular Safety of Febuxostat and Allopurinol in Patients With Gout and Cardiovascular Morbidities (CARES) trial. The CARES trial showed no significant difference between allopurinol and febuxostat in the primary composite end point of cardiovascular (CV) events in subjects with gout and established CV comorbidities at baseline. However, there was a significantly increased risk of CV and all-cause mortality with febuxostat. Urate-lowering therapy (ULT) is centr

Epidemiological and experimental studies have shown that hyperuricaemia and gout are intricately linked with hypertension, metabolic syndrome, chronic kidney disease and cardiovascular disease. A number of studies suggest that hyperuricaemia and gout are independent risk factors for the development of these conditions and that these conditions account, in part, for the increased mortality rate of patients with gout. In this Review, we first discuss the links between hyperuricaemia, gout and these comorbidities, and present the mechanisms by which uric acid production and gout might favour the
