BACKGROUND: Post-lung transplant infection is one of the leading causes of morbidity and mortality. The cause and incidence are similar in many series; however, infections such as Mycobacterium tuberculosis are influenced by the epidemiologic situation. The authors present a prospective and observational study to define the incidence, clinical presentation, and course of tuberculosis in a cohort of lung transplant patients at a single center in Spain.
METHODS: Between 1990 and 2002, cutaneous delayed-type hypersensitivity testing and pathologic and microbiologic study of explanted lungs were

A 15-year-old man, who was a classmate with the index case of a large outbreak of tuberculosis in a junior high school in Kochi city, showed strongly positive reaction to tuberculin skin test in March 1999. After taking a chest X-ray film, which showed no abnormal finding, chemoprophylaxis with isoniazid was started in April 1999. He was compliant with therapy, and had no symptom until July 1999, when he was checked again by chest X-ray and abnormal finding was suspected. He visited a hospital for further examinations, and chest X-ray revealed cavitary lesion and sputum smears showed acid-fast

The morbidity from juvenile tuberculosis in the GDR amounts presently to 0.6 per 100.000 in the age group 0 to 15 years. The GDR's centre fo … artments for lung diseases and tuberculosis or with other surgically orientated departments. Whilst tuberculin tests with 2 TE on children with lung tuberculosis gave a clear diagnosis, the same tests proved to be negative in nearly all cases with histologically confirmed lymphatic tuberculosis. Tuberculous bacteria could be detected in only 15 from 29 cases of l …

Screening for latent tuberculosis infection (LTBI) is indicated before immunosuppressive therapies but is complicated by lack of a gold standard and limited by, e.g., immunosuppression. This study aimed to investigate a series of patients diagnosed with LTBI during screening before immunosuppressive therapy, describing how the use of diagnostic tests and treatment evolved over time. This retrospective cohort study included all individuals diagnosed with LTBI during screening before intended immunosuppressive therapy in a tertiary care hospital between January 2000 and December 2017. Evidence f

Purpose: To study the risk factors for visual loss in presumed tuberculosis-related uveitis (TRU).Methods: Retrospective observational cohort study of patients with TRU, either treated or not for tuberculosis, from January 2005 to January 2017. Clinical and demographic variables were recorded. Main outcome measure was a loss of visual acuity (VA) of ≥2 Snellen lines. A Generalized Estimation Equation was used to control between-eyes bias. A backward stepwise logistic regression multivariate analysis was conducted to elucidate independent risk factors. Results: One hundred and thirty-eight eyes

BACKGROUND: Tuberculin skin test (TST) and interferon gamma release assays (IGRAs) are both recommended for routine screening of healthcare workers (HCWs) in low tuberculosis (TB)-burden countries. More recently, based on scarce data, the World Health Organization strongly recommended that IGRA should not be used for occupational screening in high-burden settings.
OBJECTIVE: To assess the prevalence of latent tuberculosis infection (LTBI) determined among highly exposed HCWs and low-exposed medical students in Johannesburg, South Africa. Methods. We performed a cross-sectional study using bot

The interferon gamma release assay (IGRA) has emerged as a useful tool for identifying latent tuberculosis infection (LTBI). This assay can be performed through testing platforms such as the QuantiFERON-TB Gold Plus (QFT-Plus) assay. This in vitro test has been incorporated into several guidelines worldwide and has recently been considered by the World Health Organization (WHO) for the diagnosis of LTBI. The possibility of systematically implementing IGRAs such as the QFT-Plus assay in centers that perform LTBI screening has been accelerated by the decreased availability of the tuberculin skin

OBJECTIVES: To estimate the sensitivity and specificity of childhood pulmonary tuberculosis (CPTB) diagnostic tests, including Xpert MTB/RIF and alternative samples such as nasopharyngeal aspirate and stool. We used latent class analysis to overcome the lack of reference standards.
METHODS: We included 1165 children suspected of having CPTB in a prospective cohort study conducted in the Ivory Coast, Madagascar, and Cameroon. We used Bayesian latent class analysis to evaluate the performance of multiple diagnostic tests for CPTB: smear microscopy, mycobacterial culture, and Xpert carried out o

Serological diagnosis of Mycobacterium bovis infection in badgers (Meles meles) has relied primarily on antibody recognition of MPB83, a sero-dominant antigen of M. bovis. Most vaccine studies in badgers to date have used the Bacille Calmette-Guerin (BCG) Danish strain, a low producer of MPB83. Due to a supply shortage of the BCG Danish strain, the BCG Sofia SL222 strain has been considered as an alternative vaccine. This strain is a high producer of MPB83 raising the possibility that vaccinated animals will test sero-positive in diagnostic assays that use this antigen. In this study we vaccin

BACKGROUND: India has one of the highest tuberculosis (TB) burdens globally. However, few studies have focused on TB in young children, a vulnerable population, where lack of early diagnosis results in poor outcomes.
METHODS: Young children (≤ 5 years) with suspected TB were prospectively enrolled at a tertiary hospital in Pune, India. Detailed clinical evaluation, HIV testing, mycobacterial cultures, and drug susceptibility testing were performed.
RESULTS: 223 children with suspected TB were enrolled. The median age was 31 months, 46% were female, 86% had received BCG, 57% were malnourished
