New antibiotics and antimicrobial combination therapy for the treatment of gram-negative bacterial infections.

PURPOSE OF REVIEW: Increasing rates of life-threatening infections due to multidrug-resistant (MDR) gram-negative bacteria, such as carbapenemase-producer strains, as well as pathogens that are resistant to all current therapeutic options, have been reported. The number of compounds that are currently being developed is still insufficient to control this global threat. We have reviewed the current available options for the treatment of MDR gram-negative infections, including combination regimens employing older antimicrobials and new compounds. RECENT FINDINGS: A limited number of large trial

Identification of the most common pathogenic bacteria in patients with suspected sepsis by multiplex PCR.

INTRODUCTION: Staphylococcus aureus, coagulase-negative staphylococci, Enterococcus spp., Enterobacteriaceae, Pseudomonas aeruginosa, and Acinetobacter baumanii have been found to be the most prevalent bacteremia-causing bacteria in patients with septicemia. Early detection of bloodstream infection (BSI) is crucial in the clinical setting. A multiplex PCR method for identification of these agents in clinical samples has been developed in parallel by conventional microbiological methods. METHODOLOGY: The target genes selected for each of the organisms were very specific for designing primers.

Riboswitch control of aminoglycoside antibiotic resistance.

The majority of riboswitches are regulatory RNAs that regulate gene expression by binding small-molecule metabolites. Here we report the discovery of an aminoglycoside-binding riboswitch that is widely distributed among antibiotic-resistant bacterial pathogens. This riboswitch is present in the leader RNA of the resistance genes that encode the aminoglycoside acetyl transferase (AAC) and aminoglycoside adenyl transferase (AAD) enzymes that confer resistance to aminoglycoside antibiotics through modification of the drugs. We show that expression of the AAC and AAD resistance genes is regulated

Membrane interactions and biological activity of antimicrobial peptides from Australian scorpion.

UyCT peptides are antimicrobial peptides isolated from the venom of the Australian scorpion. The activity of the UyCT peptides against Gram positive and Gram negative bacteria and red blood cells was determined. The membrane interactions of these peptides were evaluated by dye release (DR) of the fluorophore calcein from liposomes and isothermal titration calorimetry (ITC); and their secondary structure was determined by circular dichroism (CD). Three different lipid systems were used to mimic red blood cells, Escherichia coli and Staphylococcus aureus membranes. UyCT peptides exhibited broad

Unorthodox Parenteral β-Lactam and β-Lactamase Inhibitor Combinations: Flouting Antimicrobial Stewardship and Compromising Patient Care.

In India and China, indigenous drug manufacturers market arbitrarily combined parenteral β-lactam and β-lactamase inhibitors (BL-BLIs). In these fixed-dose combinations, sulbactam or tazobactam is indiscriminately combined with parenteral cephalosporins, with BLI doses kept in ratios similar to those for the approved BL-BLIs. Such combinations have been introduced into clinical practice without mandatory drug development studies involving pharmacokinetic/pharmacodynamic, safety, and efficacy assessments being undertaken. Such unorthodox combinations compromise clinical outcomes and also potent

Molecular detection of carbapenemase genes in WHO priority pathogens from blood cultures of sepsis patients in Pakistan.

AIMS: This study aimed to assess the antibiogram patterns of CR-GNB and to characterize carbapenemase-encoding genes among isolates obtained from patients with sepsis. METHODS: A total of 3,746 blood culture samples were collected from patients with suspected septicemia and samples were processed using the BACTEC blood culture system, and all positive cultures were sub-cultured onto blood agar and MacConkey agar plates for isolation. Bacterial identification and antimicrobial susceptibility testing were performed using the VITEK 2 compact system following CLSI guidelines. Detection of antimic

Pseudomonas aeruginosa, Klebsiella pneumoniae, and Acinetobacter baumannii were all found to have an increased resistance to CHX over time with the most profound change detected in A. baumannii. Additionally, subpopulations with log-normal distributions were found consistent with the presence of a baseline subpopulation of …

Easy-To-Access Quinolone Derivatives Exhibiting Antibacterial and Anti-Parasitic Activities.

The cell wall of Mycobacterium tuberculosis (Mtb) has a unique structural organisation, comprising a high lipid content mixed with polysaccharides. This makes cell wall a formidable barrier impermeable to hydrophilic agents. In addition, during host infection, Mtb resides in macrophages within avascular necrotic granulomas and cavities, which shield the bacterium from the action of most antibiotics. To overcome these protective barriers, a new class of anti-TB agents exhibiting lipophilic character have been recommended by various reports in literature. Herein, a series of lipophilic heterocyc

Lipid-tail modification of the antimicrobial peptide P7 enhances membrane disruption and antibacterial activity against Klebsiella pneumoniae.

Antimicrobial peptides (AMPs) are short, host-defense peptides with broad-spectrum antibacterial activity and a low propensity for inducing resistance, positioning them as promising alternatives to conventional antibiotics. The α-helical peptide P7 (KIAKRIWKILRR) previously demonstrated potent antibacterial activity against drug-resistant Salmonella enterica serovar Typhimurium with minimal toxicity. However, its efficacy against ESKAPE pathogens (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter spp.) has not …

In silico DFT, ADMET and molecular docking studies of novel coumarin-linked pyrazole and quinoline derivatives as antimicrobial and antioxidant agents.

Ten novel coumarin-based hybrids comprising coumarin-pyrazole (7a-e) and coumarin-quinoline (11a-e) derivatives were synthesized via multistep reactions and characterized spectroscopically. Solvent optimization identified DMF: Ethanol (1:1) with H2SO4 as optimal conditions, achieving 88% yield in 2-3 h. Antimicrobial evaluation against four bacterial strains (Bacillus subtilis, Staphylococcus aureus, Escherichia coli, Acinetobacter baumannii) and one fungal strain (Aspergillus niger) revealed compound 7a as the most potent (MIC: 25 μg/mL), superior to standard drugs.
