Assessment of pain-related behavior and pro-inflammatory cytokine levels in the rat rotator cuff tear model.

The cause of pain following rotator cuff tear has not been fully elucidated. The purpose of this study was to evaluate behavior and inflammatory cytokines in a rat unstabilized rotator cuff defect (UCD) model. Forty-five Sprague-Dawley rats were divided into three groups: sham; UCD; and stabilized rotator cuff defect (SCD). Gait analysis was examined using CatWalk. Tumor necrosis factor (TNF)-α, interleukin(IL)-1β, and IL-6 were measured within the subacromial bursa and the glenohumeral joint synovium at 21 and 56 days after surgery using an enzyme-linked immunosorbent assay (ELISA). Internal impingement of the shoulder.

Internal impingement of the shoulder is a pathologic condition characterized by excessive or repetitive contact of the greater tuberosity of the humeral head with the posterosuperior aspect of the glenoid when the arm is abducted and externally rotated. This arm positioning leads to rotator cuff and glenoid labrum impingement by the bony structures of the glenohumeral joint. Although some degree of contact between these structures occurs under normal conditions, to date most of the orthopaedic literature has focused on internal impingement as a disease state that affects overhead athletes and

Biceps tendinitis in chronic rotator cuff tears: a histologic perspective.

Patients with chronic rotator cuff tears frequently have anterior shoulder pain attributed to the long head of the biceps brachii (LHBB) tendon. In this study, tenodesis or tenotomy samples and cadaveric controls were assessed by use of immunohistochemical and histologic methods to quantify inflammation, vascularity, and neuronal plasticity. Patients had moderate pain and positive results on at least 1 clinical test of shoulder function. The number of axons in the distal LHBB was significantly less in patients with biceps tendinitis. Calcitonin gene-related peptide and substance P immunostaini

Arthroscopy of the shoulder.

The operative technique and indications for shoulder arthroscopy have been reviewed, with a description of proper technique and instrumentation. Visualization within the joint is usually superior to visualization with arthrotomy. With the refinement of techniques and equipment, it is expected that the frequency of transarthroscopic procedures will increase in the near future.

The shoulder: facts, confusions and myths.

The purpose of this paper is to address some of the myths, facts and confusions about the shoulder. Myth: Recurrent subluxation or dislocation of the shoulder requires surgery. Fact: The most important shoulder muscle is the anterior deltoid. Myth: You can diagnose shoulder problems with two anterior/posterior x-rays. Fact: You must keep track of the pins you insert for shoulder problems. Confusion: The position of the shoulder for arthrodesis: which to choose? Confusion: What if the arthrodesis is painful? Myth: You don't need the clavicle and can resect it without any problems. Fact: All ste

Clinical Challenges in Managing Shoulder Osteoarthritis and Suprascapular Neuropathy.

Chronic shoulder pain is a common and multifactorial condition. Suprascapular nerve neuropathy, although relatively uncommon, may lead to pain, weakness, and muscle atrophy as a result of extrinsic compression, trauma, or inflammation. When it occurs in the presence of glenohumeral osteoarthritis, overall shoulder function may be further compromised, complicating both diagnostic assessment and therapeutic decision-making. We report the case of a 56-year-old man with a three-year history of chronic left shoulder pain and progressive proximal weakness, despite multiple courses of physiotherapy.

The superior-medial shoulder arthroscopy portal is safe.

PURPOSE: The superior-medial (SM) shoulder arthroscopic portal (Neviaser portal) is the portal anatomically closest to the suprascapular nerve, and any potential benefits of this portal would be mitigated if risk of suprascapular nerve injury were significant. The purpose of this study is to determine the safety of the SM arthroscopic shoulder portal. We hypothesize that the SM shoulder arthroscopic portal is safe. METHODS: Twelve fresh cadaveric shoulders were securely positioned to simulate shoulder arthroscopy in the beach-chair position with the arm at the patient's side in neutral rotati

Biomechanical analysis of latissimus dorsi tendon transfer with and without superior capsule reconstruction using dermal allograft.

BACKGROUND: Irreparable rotator cuff tears (ICTs) remain a challenging treatment dilemma. Superior capsule reconstruction (SCR) acts as a static stabilizer to decrease superior humeral head migration. Latissimus dorsi tendon transfers (LDTs) dynamically decrease superior humeral head migration and improve external rotation. We hypothesized that the dynamic stabilizing effect of the latissimus transfer plus the static stabilizing effect of SCR would improve shoulder kinematics in shoulders with ICTs. MATERIALS AND METHODS: Eight fresh-frozen cadaveric shoulders were tested in 5 conditions: (1)

Does Pure Platelet-Rich Plasma Affect Postoperative Clinical Outcomes After Arthroscopic Rotator Cuff Repair? A Randomized Controlled Trial.

BACKGROUND: The exact role of platelet-rich plasma (PRP) in rotator cuff tendon reconstruction remains unclear. PURPOSE/HYPOTHESIS: This study investigated whether an intraoperative pure PRP injection, compared with a local anesthetic injection, improves patient-reported outcomes at 3 and 6 months after arthroscopic rotator cuff repair. The hypothesis was that pure PRP improves patient-reported outcomes (Oxford Shoulder Score [OSS]) at 3 and 6 months after surgery and has the same pain-reducing effect compared with a postoperative subacromial local anesthetic (ropivacaine) injection. MR arthrography of the shoulder: rethinking traditional imaging procedures to meet the technical requirements of MR imaging guidance.

OBJECTIVE: The purpose of this study was to determine the feasibility of and the appropriate technique for performance of MR imaging-guided arthrography of the shoulder. SUBJECTS AND METHODS: Thirty-eight MR imaging-guided glenohumeral joint punctures were performed using an open C-arm scanner with a vertically oriented magnetic field, adapted for interventional procedures. Two different approaches to the shoulder were used: a modification of the traditional anterior approach (seven procedures), and an anterosuperior approach (31 procedures) mimicking the anterior arthroscopy portal. Stemless vs. stemmed reverse total shoulder arthroplasty as primary treatment in the elderly: A meta-analysis.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is widely used for cuff-deficient and degenerative shoulder conditions in older adults. While stemmed implants offer long-term success, stemless designs have gained attention due to bone preservation and ease of revision, but evidence remains limited. METHODS: A systematic search was performed in PubMed, Scopus, Cochrane Library, and Google Scholar till June 2025. Five studies met the inclusion criteria. Key outcomes included satisfaction, Visual Analog Scale for pain, functional scores, shoulder range of motion (ROM), fracture-related co

The Swimmer's Shoulder: Multi-directional Instability.

PURPOSE OF REVIEW: Swimmer's shoulder is the term used to describe the problem of shoulder pain in swimmers. Originally described as supraspinatus tendon impingement under the coracoacromial arch, it is now understood that several different pathologies can cause shoulder pain in competitive swimmers, including subacromial impingement syndrome, overuse and subsequent muscle fatigue, scapular dyskinesis, and laxity and instability. RECENT FINDINGS: Swimmers may develop increased shoulder laxity over time due to repetitive use. Such excessive laxity can decrease passive shoulder stability and le

IgG4 related lung disease extending to the thoracic vertebrae.

IgG4-related disease (IgG4-RD) is a fibroinflammatory condition that can affect practically every organ. Although it was first identified in pancreas and salivary glands, major organs like liver, biliary tree, kidney, thyroid glands and lungs are commonly involved, sometimes resulting in organ failure. We describe a case of an 41-year-old man presented with back pain after a rotator cuff injury. A Computed Tomography (CT) revealed incidentally a right lower lobe paravertebral lesion extending across the T5 and T6 vertebral levels and invading into the adjacent pleural surface. The laboratory f

Hemiplegic shoulder pain: implications for occupational therapy treatment.

BACKGROUND: Hemiplegic shoulder pain is common after stroke causing hemiplegia. It adversely affects the recovery of arm function and independence in activities of daily living. Subluxation, abnormal tone and limited range of motion or capsular constrictions have been reported as potential causes. Other factors such as rotator cuff tears, brachial plexus injury, shoulder-hand syndrome and other pre-existing pathological conditions may also be associated with hemiplegic shoulder pain. The etiology remains unclear, but hemiplegic shoulder pain may result from a combination of the above factors.

A randomised controlled trial of intravenous dexamethasone combined with interscalene brachial plexus blockade for shoulder surgery.

We recruited patients scheduled for shoulder rotator cuff repair or subacromial decompression under general anaesthesia and interscalene brachial plexus blockade (30 ml ropivacaine 0.5%). We allocated 240 participants into four groups of 60 that were given pre-operative saline 0.9% or dexamethasone 1.25 mg, 2.5 mg or 10 mg, intravenously. We recorded outcomes for 48 h. The median (IQR [range]) time to first postoperative analgesic request after saline was 12.2 (11.0-14.1 [1.8-48]) h, which was extended by intravenous dexamethasone 2.5 mg and 10 mg to 17.4 (14.9-21.5 [7.2-48]) h, p < 0.0001, an

A multimodal spatio-temporal graph neural network framework for fatigue detection in tennis serving.

BACKGROUND: The tennis serve is a complex, high-velocity motion dependent on the efficient transfer of energy through the kinetic chain, from the lower extremities to the racquet. Fatigue-induced alterations in this chain are primary precursors to overuse injuries, such as rotator cuff tendinopathy and elbow medial collateral ligament stress. However, current wearable monitoring solutions predominantly rely on unimodal kinematic data, failing to capture the neuromuscular compensatory mechanisms that precede mechanical performance degradation. METHODS: This study introduces a novel multimodal

Impact of 30 years' high-level rock climbing on the shoulder: an magnetic resonance imaging study of 31 climbers.

BACKGROUND: Rock climbers are particularly susceptible to shoulder injuries due to repetitive upper-limb movements on vertical or overhanging terrain. However, the long-term effects of prolonged climbing on the shoulder joints are still unknown. PURPOSE: The purpose of this study was to analyze the prevalence of pain and degenerative changes in the shoulder joints after high-level rock climbing over at least 25 years. We hypothesized that specific climber-associated patterns of degeneration would be found. METHODS: Thirty-one adult male high-level rock climbers were compared to an age- and s
