Over an 8-month period (July 1990-to February 1991), we explored 21 women presenting with a clinical and laboratory profile of prolactin-secreting microadenoma of the pituitary gland. Magnetic resonance imaging (MRI) is undoubtedly the most efficient method to explore microadenomas, especially when carried out in the absence of any treatment. In 8 cases, MRI was performed in the absence of medical treatment and gave a positive result, i.e. always showed a focal lesion. In the remaining 13 cases the patients had been treated before the exploration, and MRI detected a microadenoma in only 4 case

Thirty-six proven cases of medulloblastoma were reviewed by serial CT follow-up examinations from 4 months to 10 years, 2 months after the initial diagnosis, with a mean follow-up time of 3 years, 9 months. The children ranged in age from 10 months to 16 years, 7 months at the time of follow-up. The tumor recurred at the primary site in 20 cases (56%). Leptomeningeal metastasis was demonstrated on CT in 14 cases (39%); seven of these patients also presented with solid subarachnoid metastases. Thirteen patients (36%) showed evidence of severe brain atrophy, which was confined to the posterior f

Glioblastoma multiforme (GBM) is typically characterized by a locally infiltrative presentation; secondary involvement of the dura mater and leptomeningeal disease are infrequent. A rare diagnostic finding is the isolated "dural tail sign" on contrast-enhanced MRI. While this sign is frequently considered reactive, histopathological findings in highly proliferative instances with increased extracerebral tropism confirm direct neoplastic invasion, justifying intensified surveillance in patients diagnosed with GBM. We present a case of glioblastoma, IDH-wildtype, CNS WHO grade 4, in a patient wh

Meningiomas usually arise from arachnoid cap cells and present as intracranial, dural-based lesions. Extracranial extension into the temporalis muscle, infratemporal fossa, orbit, and skull base foramina is uncommon and can mimic primary soft-tissue or peripheral nerve sheath tumors. We report a 54-year-old female who presented with a slowly progressive right-sided facial and temporal swelling over two years, associated with intermittent pain and mild proptosis but no focal neurological deficit. Computed tomography demonstrated a small extra-axial minimally enhancing right anterior temporal le

OBJECTIVES: Venous compression is an underrecognized cause of trigeminal neuralgia (TN) and remains controversial due to its subtle imaging manifestations, variable anatomy, and uncertain optimal surgical management. This study aimed to clarify the anatomical spectrum of venous offenders, evaluate surgical outcomes, and explore the role of adjunctive trigeminal nerve combing (NC) in selected cases.
METHODS: We retrospectively analyzed 31 patients with primary TN who underwent microvascular decompression (MVD) between 2017 and 2023 and were confirmed intraoperatively to have pure venous compre

Cerebral venous sinus thrombosis (CVST) can increase intracranial pressure and cause secondary brain injury. There is uncertainty in the literature regarding the risk of traumatic CVST (tCVST) following fractures over a major sinus, the need and timing for venous imaging, and treatment. The current study aims to review our experience with tCVST when treating patients with isolated linear fractures over the major sinuses. Methods. This is a retrospective study of 95 adult patients, treated between January 2017 and March 2023, with mild, blunt, isolated traumatic head injury (GCS 13-15) and cran

BACKGROUND: In Nepal, a developing country in South East Asia, neurointerventional procedures are still in early stages. Despite progress in noninvasive neurovascular imaging, digital subtraction angiography (DSA) remains the most precise technique for diagnosing vascular disorders within the central nervous system. This article aims to assess the safety of the first 1000 diagnostic procedures conducted after the introduction of interventional neuroradiology practice in a tertiary care setting in Nepal.
METHODS: A retrospective cross-sectional study included consecutive diagnostic digital sub

Analyze the influence of tumor vascularization and intraoperative bleeding on surgical outcomes of Grade IV Vestibular Schwannoma (VS). This single-center analytical study included patients with sporadic Grade IV VS operated on by the same surgeon in Argentina between 2011 and 2023. Clinical, imaging, surgical and pathology variables were prospectively analyzed. Tumor vascularization was assessed via preoperative perfusion MRI and correlated with intraoperative bleeding, microvascular density (CD34), and VEGF expression. Associations with facial nerve (FN) outcomes and extent of resection (EOR

Anterior intradural extramedullary meningiomas are intimidating, especially in the thoracic spine due to the spinal cord impeding conventional surgical corridor, restricted space, and precarious cord vascularity.1,2 This is possibly the first report in the literature of a minimally invasive surgery: far-lateral hybrid exoscopic and angled endoscopic approach for an anterior thoracic spine meningioma. A 60-year-old woman presented with an anterior T1-2 meningioma with a flattened cord draping over the tumor more toward the right, leaving a potential corridor on the left lateral aspect (Video 1)

BACKGROUND: Chiari malformation is characterized by inferior displacement of the cerebellar tonsils through the foramen magnum, frequently resulting in strain related headaches, and motor/sensory dysfunction. Chiari decompression technique varies significantly, possibly contributing to frequent revisions. We reviewed revision Chiari decompressions at our institution to determine the primary indications for revision and outcomes after revision.
METHODS: We retrospectively reviewed patients who underwent revision of Chiari decompression at our institution from 2005 to 2020. Demographics, indica

BACKGROUND AND OBJECTIVE: There are many surgical approaches for execution of a thoracic corpectomy. In cases of challenging deformity, traditional posterior approaches might not be sufficient to complete the resection of the vertebral body. In this technical note, we describe indications and technique for a transdural multilevel high thoracic corpectomy.
METHODS: A 25-year-old man with a history of neurofibromatosis type 1 presented with instrumentation failure after a previous T1-T12 posterior spinal fusion, extensive laminectomy, and tumor resection. The patient presented with progressive

Cerebral amyloid angiopathy (CAA) is a small vessel disease, causing spontaneous intracerebral hemorrhage (ICH) in the elderly. It is strongly associated with Alzheimer disease (AD), as most CAA patients show deposition of Aβ-i.e. the basic component of parenchymal Alzheimer amyloid deposits-in the cerebral vessels. Iatrogenic early-onset CAA has been recently identified in patients with a history of traumatic brain injury or other cerebral as well as extra-cerebral lesions that led to neurosurgery or other medical procedures as intravascular embolization by cadaveric dura mater extracts many

Meningitis caused by bacteria, which is an inflammation of the meninges affecting the pia, arachnoid, and subarachnoid space, is still one of the leading causes of death and morbidity in infants and young children. Neisseria meningitidis, group B streptococcus (GBS), Haemophilus influenzae type B (Hib), Listeria monocytogenes, and Streptococcus pneumoniae have been found to be the most frequent causative agents. Infants and children can have modest, fluctuating, non-specific, or even absent clinical signs of bacterial meningitis. They may include bulging fontanelles, vomiting, diarrhea, respir

OBJECTIVE: Minimally invasive spine surgery is rapidly gaining popularity because of its versatile nature. Traditionally, prolapsed disc has been the most common disease targeted using this technique. However, its usefulness for various other diseases has also been shown in studies. We present our experience of using this technique for various spinal diseases apart from prolapsed discs.
METHODS: This is a retrospective study in which patients operated on by a single surgeon from January 2019 to April 2023 were included. Cases that required conversion to open technique were excluded. Intraoper

Multiple cranial nerve palsies frequently accompany hypoglossal nerve palsy, potentially indicating malignancy, such as lymphoma, nasopharyngeal carcinoma, or metastases. However, when solely the hypoglossal nerve is affected, the causes may involve Chiari malformation, arachnoid cyst, or infectious mononucleosis, suggesting a positive prognosis. Craniocervical junction tuberculosis (TB), is an uncommon cause of isolated hypoglossal nerve palsy and has been reported infrequently in the literature. Craniocervical junction tuberculosis accounts for only 0.5% of TB cases overall and 6% of extra-p

BACKGROUND: MN1 C-terminal truncation (MCTT) is a rare syndrome; only 27 cases have been reported. We report the first case of an 8-year-old girl with MCTT syndrome complicated with moderate obstructive sleep apnea (OSA).
METHODS: MCTT syndrome was diagnosed by whole-exome sequencing (WES) and validated by Sanger sequencing. The patient received 2 years of treatment with continuous positive airway pressure (CPAP) to relieve sleep apnea and hypoxia, and a reverse sector fan-shaped expander for maxillary expansion.
RESULTS: WES revealed a de novo MN1 variant, c.3760C>T (p.[Q1254*]). An arachno
