Morphometric evaluation of screw fixation in atlas via posterior arch and lateral mass.

STUDY DESIGN: This study is a result of manual and radiologic measurements of 50 isolated anatomic specimens of C1 and five patients of atlantoaxial instability treated by screw fixation via posterior arch and lateral mass. OBJECTIVES: To investigate the feasibility of screw placement via posterior arch and lateral mass in atlas. SUMMARY OF BACKGROUND DATA: Several types of posterior approaches have been adopted for stabilization and fusion of atlantoaxial complex. Before this study, Gallie in 1939 gave a report on a posterior wiring technique that depended on a structural bone graft. Since

Comparison of Autogenous and Alloplastic Cranioplasty Materials Following Impact Testing.

Alloplastic materials are often used when significant defects exist. Benefits include no donor site morbidity, relative ease of use, limitless supply, and predictable durability. Depending on the type of alloplast, limitations include a persistent risk of extrusion and infection. Of particular interest in relation to cranioplasties is the ability of the material to provide neuroprotection. The integrity and neuroprotective properties of autologous bone flaps, polymethylmethacrylate (PMMA), and high-density porous polyethylene (PP) were evaluated following impact testing. A Novel Scoring System Can Estimate the Risk of Posttraumatic Osteonecrosis After Femoral Neck Fracture Fixation in Adults Younger Than 65 Years.

BACKGROUND: Femoral neck fractures in patients under age 65 years remain challenging because up to 21% of them will develop osteonecrosis of the femoral head (ONFH) after internal fixation, which can be painful and may result in conversion to arthroplasty. This complication is particularly burdensome for patients who still are employed. However, preoperative parameters associated with ONFH remain unclear, and no reliable risk-stratification tool is currently available. QUESTIONS/PURPOSES: (1) Which preoperative clinical and radiographic variables were retained in a risk-stratification model?

Trans-iliosacral plating for vertically unstable fractures of sacral spine associated with spinopelvic dissociation: A cadaveric study.

BACKGROUND: The treatment algorithm for sacral fracture associated with vertical shear pelvic fracture has not emerged. Our aim was to study a new approach of fixation for comminuted and vertically unstable fracture pattern with spinopelvic dissociation to overcome inconsistent outcome and avoid complications associated with fixations. We propose fixation with well-contoured thick reconstruction plate spreading across sacrum from one iliac bone to another with fixation points in iliac wing, sacral ala and sacral pedicle on either side. Present biomechanical study tests the four fixation patter

Supplemental Bone Grafting in Giant Cell Tumor of the Extremity Reduces Nononcologic Complications.

BACKGROUND: Giant cell tumors (GCTs) are treated with resection curettage and adjuvants followed by stabilization. Complications include recurrence, fracture, and joint degeneration. Studies have shown treatment with polymethylmethacrylate (PMMA) may increase the risk of joint degeneration and fracture. Other studies have suggested that subchondral bone grafting may reduce these risks. QUESTIONS/PURPOSES: Following standard intralesional resection-curettage and adjuvant treatment, is the use of bone graft, with or without supplemental PMMA, (1) associated with fewer nononcologic complications

Mandibular fracture severity and patient health status are associated with postoperative inflammatory complications.

PURPOSE: To identify risk factors associated with postoperative inflammatory complications (POICs) after treatment of mandibular fractures. PATIENTS AND METHODS: The investigators designed a case-control study and enrolled a sample of patients treated for mandibular fractures at Massachusetts General Hospital between August 2004 and January 2010. Subjects who developed POICs after fracture management were categorized as cases. A POIC was defined as 1) recurrent swelling, fever, increased pain, or trismus; 2) wound dehiscence with purulent drainage; 3) exposed or infected hardware; 4) abscess

Intramedullary Nailing Technique for Proximal Humeral Fractures Using a Straight Antegrade Nail with Locking Tuberosity Fixation.

BACKGROUND: Intramedullary straight nail fixation of proximal humeral fractures using a locking mechanism provides advantages compared with plating, including (1) less soft-tissue dissection, which preserves periosteal blood supply and soft-tissue attachments; (2) improved construct stability for comminuted fractures or osteopenic bone; and (3) shorter operative time for simpler fractures. DESCRIPTION: The patient is placed in the beach-chair position with the head of the bed elevated approximately 45°. The fracture is reduced with use of closed or percutaneous methods, ideally, or with an op

Zygoma reconstruction.

Ideal reconstruction of the zygoma position is essential in restoring facial width, projection, and symmetry. Reconstruction should be focused on the zygoma's 4 articulations and restoring the vertical and horizontal pillars of the facial skeleton. This article describes the applied surgical anatomy as it relates to zygomatic deformities, surgical approaches, and reconstruction. The basis for diagnosing and classifying zygoma deformities as they relate to severity of injury and associated displacement, comminution, and comorbidities is also discussed. Traditional and contemporary concepts in p

Hawkins Award 2025: clinical implications of radiological findings associated with radial head replacement. A long-term follow-up study.

BACKGROUND: Radial head replacement (RHR) is indicated in comminuted fractures and those associated with elbow instability. Despite satisfactory clinical outcomes, some patients present with concerning radiographic findings. Our aim was to compare the long-term radiological results of 2 RHR designs and to correlate them with clinical findings. METHODS: A prospective observational cohort study including 134 patients who received an RHR (84 spacer designs and 50 anatomic models) for acute elbow instability was conducted. Patients were prospectively followed for a mean of 88 months (range, 24-20

Locking compression plate distal ulna hook plate fixation versus intramedullary screw fixation for displaced avulsion fifth Metatarsal Base fractures: a comparative retrospective cohort study.

BACKGROUND: Intramedullary screw (IMS) fixation was wildly used in fifth metatarsal base fractures (FMBFs) and the results were satisfactory. However, in the comminuted osteoporosis or small displaced avulsion FMBFs, anatomical reduction and stable fixation could not be achieved with IMS. The Locking Compression Plate (LCP) distal ulna hook plate fixation was a novel alternative fixation method. The aim of this retrospective cohort study was to determine if LCP distal ulna hook plate fixation resulted in improved outcomes compared to the traditional IMS fixation in displaced avulsion FMBFs. Distal short segment fixation of thoracolumbar and lumbar injuries.

Fifteen patients with thoracolumbar and lumbar spine injuries were treated between November, 1993, and April, 1996, using a posterior approach that involved short segment distal fixation at or one level below the level of injury. Medical records and radiographs were reviewed for complications, maintenance of correction, repeat surgeries and functional status. Indications for this technique included fractures and ligamentous injuries that did not require direct canal decompression and were without severe comminution. The level of injury was T12 in two patients, L1 in seven patients, L2 in two p

Postoperative subacute static progressive stretch does not increase the risk of distal lower limb venous thromboembolism.

PURPOSE: Static progressive stretch (SPS) can be applied to treat chronic joint stiffness. However, the impacts of subacute application of SPS to the distal lower limbs, where deep vein thrombosis (DVT) is common, on venous thromboembolism remain unclear. This study aims to explore the risk of venous thromboembolism events following subacute application of SPS. METHODS: A retrospective cohort study was conducted on patients diagnosed with DVT following a lower extremity orthopedic surgery before being transferred to the rehabilitation ward from May 2017 to May 2022. Patients with unilateral l

A systematic review and meta-analyses on animal models used in bone adhesive research.

Currently, steel implants are used for osteosynthesis of (comminuted) fractures and intra-articular bone defects. These osteosyntheses can sometimes be complicated procedures and can have several drawbacks including stress shielding of the bone. A bone glue might be a safe and effective alternative to current materials. Despite numerous animal studies on bone adhesives, no such material is clinically applied yet. We have conducted a systematic review to summarize the evidence in experimental animal models used in research on bone adhesive materials for trauma and orthopedic surgery. Additional

[Recurrent purulent bacterial meningoencephalitis].

During the period of 25 years there were 55 patients treated in our Institute because of recurrent purulent bacterial meningoencephalitis(rpbme). This group consisted of 42 males (76%) and 13 (24%) females, the prevalent number (53%) of patients being under 21 years of age. The diagnosis of rpbme was based on the commonly accepted criteria and confirmed by the laboratory results of CSF examination. The cause of the recurrences was established considering the skull X-ray examination, CT and MRI. The evaluation of the clinical status was based on the Glasgow Coma Score (GCS). During the first ho
