The Anticoagulation Dilemma: Concurrent Traumatic Brain Hemorrhage and Cerebral Venous Thrombosis in a Blast Injury Patient.

Blast injuries produce complex multi-system trauma through primary barotrauma, secondary penetrating projectiles, tertiary displacement forces, and quaternary mechanisms. Among the most challenging complications is the concurrent presentation of traumatic intracranial hemorrhage and cerebral venous sinus thrombosis (CVST), which creates a profound therapeutic dilemma regarding anticoagulation management. We present a 26-year-old male who sustained injuries from a high-order explosive device that resulted in 19 fatalities. He presented 16 h post-injury with a Glasgow Coma Scale score of 10/15 a

Transorbital Penetrating Intracranial Injury by Wooden Foreign Body: A Rare Case Report.

Transorbital penetrating intracranial injury (TOPI) is a rare form of traumatic brain injury, often caused by high-velocity projectiles. However, non-missile penetrating injuries, particularly from wooden foreign bodies, are uncommon and pose diagnostic and therapeutic challenges due to their radiolucency and high risk of infection. We report the case of a 58-year-old Nepali male who sustained a TOPI after falling from a tree, with a wooden foreign body penetrating his right orbit and extending intracranially. The patient presented 6 days post-injury with persistent pain and bleeding but no ne

Thoracic and lumbar spine trauma classification systems fail to predict post-traumatic kyphotic deformity.

BACKGROUND: Post-traumatic kyphosis of the thoracic and lumbar spine can lead to pain and decreased function. MRI has been advocated to assess ligament integrity and risk of kyphosis. METHODS: All thoracic and lumbar spine MRI performed for evaluation of trauma over a 3-year period at a single institution were reviewed. Patients were included if there was an MRI showing a vertebral body fracture and follow-up radiographs. Two observers retrospectively reviewed all radiographs, CT and MRI scans, and classified injuries based on the Denis, TLICS, AO and load sharing classification systems. Chan

The Talar Body Prosthesis: Results at Ten to Thirty-six Years of Follow-up.

BACKGROUND: Satisfactory results of implantation of the talar body prosthesis were reported in 1997, although some complications associated with the initial design were noted. The present study evaluated outcomes of treatment with a modified talar body prosthesis. METHODS: Of the thirty-six talar body prostheses implanted with use of a transmalleolar surgical approach from 1974 to 2011, thirty-three were available for follow-up at ten to thirty-six years or had failed prior to that time. The indication for implantation had been osteonecrosis in twenty-three patients, a comminuted talar fractu

Recurrent, locked posterior glenohumeral dislocation requiring hemiarthroplasty and posterior bone block with humeral head autograft.

This article describes the case of a 77-year-old man with a recurrent posterior shoulder dislocation, treated with humeral hemiarthroplasty and reconstruction of a large posteroinferior glenoid defect with a bone block created from humeral head autograft. On examination, the patient's left arm was held in internal rotation against his stomach, with minimal swelling about the deltoid. His shoulder was nontender to palpation; however, he had substantial motion restrictions, and attempted motion was painful. He could forward elevate to 30°, extend to 0°, internally rotate to belly, and passive ex

Radial head implant arthroplasty.

Radial head arthroplasty has developed into a reliable procedure to address fractures of the radial head not amenable to reconstruction, particularly when associated with an unstable elbow or forearm. Good to excellent results can be anticipated when used for the correct indications and when care is taken to understand and address the concomitant injuries. This article will discuss the technique of radial head arthroplasty in the context of the injuries that often accompany radial head fractures.

Serial nasal bone reduction: a new approach to the management of nasal bone fracture.

Nasal bone fracture is the most common type of facial bone fracture. Although these injuries often do not seem to be severe, undertreatment of nasal trauma could lead to significant long-term problems. The postreduction incidence of nasal deformities requiring subsequent rhinoplasty or septorhinoplasty ranges from 9% to 50% in different studies. A clinical trial study on 330 cases with simple nasal bone fracture was performed. These patients were divided into 2 groups randomly. In the case group, serial nasal bone reduction was performed, and in the control group, classic nasal bone reduction

Ankle arthrodesis following bone transport as a definitive limb salvage strategy after multiple failed microsurgical reconstructions for infected tibial pilon fracture nonunion: a case report.

BACKGROUND: Reconstruction of large segmental tibial bone defects remains one of the most challenging problems in orthopedic trauma surgery, particularly when complicated by infection and soft tissue compromise. Various techniques, including vascularized fibular grafting, induced membrane formation, and bone transport, have been described for limb salvage in such complex situations. Bone transport based on distraction osteogenesis enables biological reconstruction of bone defects while simultaneously improving the local biological environment. CASE PRESENTATION: A 39-year-old male constructio

Comparative Study of Minimally Invasive Plate Osteosynthesis Versus Open Reduction and Internal Fixation in the Treatment of Distal Fibular Fractures.

INTRODUCTION: Minimally invasive plate osteosynthesis (MIPO) has been proven to be better than open reduction and internal fixation (ORIF) for treating various long bone fractures. However, there are not many studies on distal fibula fracture treatment by MIPO. This study compares the clinical and radiological outcomes and quality of life of the MIPO with ORIF in the treatment of fractures of the distal fibula within a 6-month follow-up. MATERIALS AND METHODS: Patients undergoing MIPO (n = 27) and ORIF (n = 30) for distal fibula fractures (Danis-Weber type B and C) treatment from 2023 to 2024

Temporal map of the pig polytrauma plasma proteome with fluid resuscitation and intravenous vitamin C treatment.

BACKGROUND: Fluid resuscitation plays a prominent role in stabilizing trauma patients with hemorrhagic shock yet there remains uncertainty with regard to optimal administration time, volume, and fluid composition (e.g., whole blood, component, colloids) leading to complications such as trauma-induced coagulopathies (TIC), acidosis, and poor oxygen transport. Synthetic fluids in combination with antioxidants (e.g., vitamin C) may resolve some of these problems. OBJECTIVES: We applied quantitative mass spectrometry-based proteomics [liquid chromatography-mass spectrometry (LC-MS/MS)] to map the

Distal clavicle fracture as a complication of arthroscopic distal clavicle resection.

Arthroscopic resection of the distal clavicle has been advocated as a surgical treatment option for acromioclavicular (AC) joint pathology. To our knowledge, iatrogenic fracture of the distal clavicle during distal clavicle resection has never been reported. This report describes distal clavicle fracture as a complication of misidentification of the AC joint and subsequent aggressive burring of the distal clavicle during shoulder arthroscopy. This case is further complicated by the development of symptomatic delayed union and adhesive capsulitis. Ultimately, a revision distal clavicle resectio
